Employee suffered injuries to the top of EE's left foot and toes when a piece of stone fell from the rib and penetrated EE's leather mining boot just behind the steel toe.

Employee was working on cleaning up a rib roll when more of the rib rolled out hitting the employee on the right shoulder and lower back. Employee continued to work until 8/3/2026 when EE was taken off work.

Shuttle car driver was entering EE's car when a piece of coal rib struck EE in the back and the right ankle. Started missing work on 7/25/2025 for inflammation.

EE walked beside the bolter in the bolted entry to throw cable out of the way.A rock fell from the rib 8' high measuring 16" long by 2.5" wide.The rock hit the EE on the left forearm then the left boot.When it struck EEs boot, the rock went through the top of the LaCross Metatarsal guard mining boot causing a laceration to the top of EEs foot around the toes.EE got 2 pins/sutures.

Miner operator was starting to load a car when the face fell in leaving a pile of rock just inby the last row of bolts. A rock then fell out just inby the last row of bolts landing on the pile of rock causing the rock to slide in on the operator's left ankle. Resulting in a fracture the miner operator started missing work 1/24/2025.

While setting in the shuttle car at the feeder the corner of the rib rolled off and some of the coal fell into the cab pinching the skin on the back of the right arm causing two small lacerations. 3 sutures in one and 5 sutures in the other.

Employee was bolting with a single head bolter and a piece of rib rolled out, smashing EE's right hand against the body of the bolter.

The roof bolter operator was cleaning the dust box on the opposite side of the bolter. While squatting down a rock struck the roof bolter operator on top of the head causing a headache. The rock fell between the rib and the rib pin. The roof bolter operator was taken off work starting 6/17/2024.

The miner crew was mining crosscut 3 to 2. The back bumper of the miner was positioned near the inby corner of the crosscut. As the employee reached for the lever to lower the ATRS jack, the rib rolled off the corner and a rock struck employee's left forearm, pinching it between the miner frame and the rock.

Employee was in the process of cleaning the dust box on the roof bolter when a piece of coal rib popped off behind the employee striking the right calf, resulting in a fractured fibula.

Employee had just installed cable bolts. EE was at the inch tram, when EE dropped the ATRS, a piece of rib rolled off knocking EE against the bolter. This resulted in a fracture to EE's lower back and a broken bone in EE's upper back.

Employee was sitting in a battery scoop when a piece of rib fell off and hit EE on EE's right cheek resulting in 5 stitches. EE received a full duty release the same day.

Rib roll on foot

Employee was picking up trash and when employee went to get back in the scoop a piece of rib rolled off and struck employee's arm around the elbow. X-rays was negative. Employee was taken off work pending an MRI.

Employee was on knees shoveling belt when a piece of rock rolled off the rib (13"X13"X3") and struck employee on the top of shoulder.

Employee was working on a brattice. A piece of the rib fell off causing a laceration on EE's finger that required 2 stitches.

Employee had got out of the deck of the bolter, walked around the off side, grabbed a crosser to cross the miner cable. After the employee took the crosser to the miner operator to put on the cable. EE was beside the bolter when the rib rolled off catching the lower part of EE's leg resulting in a fracture between the knee and ankle.

Employee was pinning in a header hole when a rock came out of the rib and struck employee on the back pushing employee into the canopy, cutting forehead.

The employee was running a CM. As EE was finishing punching a crosscut, a sharp rock fell out between the rib and roof bolt and struck EE on the back of EE's left hand which was on the remote box causing a laceration. (9) sutures were required to close the wound.

Employee was roof bolting and a piece coal came off the coal rib and struck them on the back of calf just above ankle. Employee has soft tissue damage.

Employee was spotting face chain links. Employee was sitting on the tailgate drive housing with left leg hanging over the edge. A rock rolled out of the face/top, catching employee's leg between the rock and the tailgate drive housing.

Pulling draw rock.

Employee was kneeling when a coal rib popped out causing debris to fly into left eye, causing an abrasion to the cornea. Employee started missing work on 3/26/2021.

Employee was capping off the side of a brattice, when piece of coal rib fell off and smashed EE's right thumb into the block. The abrasion took four sutures.

Employee was roof bolting a right turnout. Due to the mining height being somewhat lower, ee was standing with feet spread out wide to lower body under the canopy. This exposed feet and legs out from under the canopy. A rock fell inby location from the unsupported face and struck ee on the right foot resulting in a fracture. EE started missing work on 10/2/18

Employee was working on a piece of equipment when EE was struck by a piece of coal rib in the left index finger.

EE was moving a tailpiece on a unit. As EE placed material on the tailpiece, a section of the rib broke off & hit EE from behind knocking EE to the mine floor. EE was taken to the hospital to be examined and later released to return to work. EE continued to work until an MRI revealed that EE had a torn ACL & Meniscus. EE started missing work on 04/24/18, waiting for surgery.

Employee was installing a roof bolt in place of a loose pin, due to scaling rock. When rib and a section of the roof rolled off striking the employee from behind on left lower leg.

Employee was installing wire mesh when a rock fell out between rib and wire mesh hitting employee on the left foot and ankle.

Employee was positioning a section rock duster machine (Bantam type duster), to load into a scoop bucket, at which time a piece of coal block rib fell out striking the employee, and knocking the employee down.

Employee and coworker were working on the continuous miner, when a section of the rib rolled off and struck the employee in the back right side of head causing a laceration, that required stitches.

Employee and coworker were working on the continuous miner, when a section of the coal rib rolled off striking employee in left cheek and shoulder. The laceration on the cheek required stitches.

Employee was pinning in the 2L entry, when the coal rib rolled off hitting employee in the right shoulder & back area.

Employee was holding a piece of wire mesh that was being installed unto the coal rib. A small rock came out of the coal rib striking the employee in the upper lip; this caused a laceration that required four stitches to the up lip.

EE was installing a concrete brattice wall and during this process, a piece of loose rib rolled striking EE on the left leg/ankle, pushing EE into the partially built brattice. EE's injury resulted in a fracture to the fibula (lower leg - small bone) and EE began missing time on 11-16-16.

Employee was pinning in #1 entry, the canopy of the bolter hit the rib. After placing the first pin EE was drilling EE's second hole and the drill steel stopped up, the EE went to the middle of the bolter to retrieve a new drill steel, when a rock from the rib fell down, an hit the wire mesh that was leaned up next to the rib and hit EE in the back of the right leg.

Employee was wet dusting a punch cut between #6 & #7 entries. His scoop was parked on the inby corner of the #7 intersection, he attempted to reenter the scoop on the rib side, a piece of loose rib rolled off the corner, striking his back and pushing his body into the scoop.

EE was scooping out by #2 unit when he noticed the rib he had just scooped was loose, so he jumped out of the way and the rib caught him on the back of his lower legs. He only had contusion to both legs but was not given a full duty release by the doctor.

Employee was starting a brattice on #1 unit and when he bumped the rib with a block, the top of the rib sluffed out and hit him on his shoulder.

Employee was shoveling belt when a rock fell out between the rib and first pin, striking employee on the right arm and left butt cheek.

He was driving shuttle car, he had arm laying on outside of cab, rib rolled off and struck his right arm.

Employee was sitting on his golf cart at #3c belt sub when a piece of rock fell out striking him on his right knee. He is off waiting to get results of CT scan.

Employee was retrieving a mine de-watering pump, when the rib sloughed/rolled out and struck the employees hand against the pump, causing injury to his thumb.

Loose gob fell out from between the rib pin and the rib, striking him on the right forearm, causing a laceration. ***Stitches***

Employee was pinning ribs and rib came off and struck his hand against truss bolter jaws.

Employee was pinning in #4 right enrty when a rock fell from rib hitting him in lower back.

After cutting a header hole a piece of rib came loose and struck employee on the lower left leg.

Employee was changing cutting bits on the shear head gate drum. While doing so a section of the face slide down and part of it landed on his left foot. The employee was examined at the local ER and given a full release to return to work. Employee continued to have foot pain and sought medical attention and is off work as of 07/16/2012 with a deep contusion injury to the left foot.

Employee was pinning 3 right when a slip in rib fell out and landed on employee's left leg.

While bolting #5 entry of #1 Unit, a rib rolled in and pushed his head into the tray of the bolter. ***He received 9 stitches on his forehead.***
