The injured was bolting in the 7 right crosscut. After he stacked his pusher and finishing piece of steel together and was drilling to his required depth, he went to get a bit out of the tray to change on his starter steel and a piece of rock fell on top of his canopy, and continued to fall on his left hand.

Individual was relocating fly pads when a rock fell from the mine roof striking him in the head then while the rock was sliding down his back he received a laceration that required four sutures to close the wound

Employee was taking a guard off of a take-up and while pushing up on the guard, it hit the roof dislodging a piece of draw rock (16" x 16" x 2")that hit employee on the left leg and ankle.

Individual was drilling to shoot an overcast. As he was marking his hole spacing a rock fell from the top striking him in the face.

While roof bolting along a clay slip a piece of shale fell and struck him on the left foot causing a contusion to the fifth toe. He continued working but after pain persisted he sought treatment at St. Johns ER Springfield where he was diagnosed with a fracture to the toe. After follow up he was released to unrestricted duty.

A piece of rock struck the employee while bolting. His hand was setting on the tray of the machine when the rock fell and hit his hand.

EE was kneeling down to work on a power center when a rock from the top fell striking EE on the head above the left eye. EE continued to work the rest of his shift and went to the doctor and received 9 stitches to close the wound.

Individual was standing on the walk way of the bolter attempting to pry a loose rock from the roof when the rock fell. It struck the canopy of the bolter and slid inward toward the walk way striking the individual on the left foot.

As the employee was installing roof bolts a piece of rock fell and hit his head.

The injured was pushing his drill steel against the mine roof to start a hole, when a piece of loose rock fell and hit on the arm.

While attempting to clear rocks off a continuous miner was inadvertently struck by a sliding slab of shale out of his view. The piece hit him above the boot line at his right shin leaving a laceration that required staples to close. Treated and released at St. John's ER Springfield, IL.

While operating a Continuous Mining Machine on MMU 001-0 section in #8 right x-cut (punch thru), EE was struck on top of his shoulders with a piece of mine roof that appeared to be secured to the mine roof, became loose while mining #8 x-cut into #9 entry. The piece of rock fell from between permanent roof support and the coal rib.

While attempting to install a roof bolt, a piece of draw rock, slick sided and sharp, fell striking employee on his left hand causing a laceration to the middle finger requiring stitches.

While roof bolting and installing an 8' bolt with the plate on the bend hit a 4' brow with the plate and chipped off a small rock shooting it towards him and striking him above his upper lip leaving a laceration that required sutures to close. He was released after treatment to unrestricted duty.

While setting a prop and standing at the base of the prop, a chunk of shale fell and struck him on the right forearm, leaving an area of abrasion with several small lacerations. Transported by private vehicle to Springfield where the wound was cleaned and sutured. Released to unrestricted duty.

The injured was installing a roof bolt, as he pushed the bolt up, a piece of loose rock fell and cut him on the arm.

While operating the roof bolter, the employee was resting his hand on the drill head, when a rock from the roof fell striking his left hand and dislocating his left ring finger and causing a laceration which took sutures to close.

The employee was operating the continuous miner on the number 1 section in the number 8 heading. A rock fell striking the employee in the back. The employee was seen by a physician on 3-16-10 and was placed on light duty.

The employee was roof bolting when a piece of roof rock fell, striking him on the left hand, fracturing the 3rd metacarpil bone and lacerating the index finger.

While bolting and as he was inserting an extension to complete a 6 foot hole, a piece of clay fell from the roof and struck his right thumb, leaving a laceration that required sutures to close. X-rays also revealed a fracture of the thumb. Treated and released at Springfield Memorial ER to return to work without restriction.

Employee was operating a roof bolter on the right side. He had completed drilling a hole and was bringing the drill steel out of the hole when a small piece of roof rock fell and struck him on the right side of the upper lip. The wound required 3 sutures to close. He returned to work on his next scheduled shift without restrictions.

He went to back of miner to check his center line. He got down on his knees and a piece of rock fell from the top, 2'x 2'x 2", which punctured his boot and cut his ankle. Injured received 5 or 6 stitches to close wound.

The injured was operating a radio remote controlled continuous miner when a rock fell inby the last row of roof bolts, it rode through the last row in a manner that it struck the injured as he truned to get back, resulting in an injury to his neck and a fracture to his left ankle.

While roof bolting and setting a pin beneath a brow leaned out from beneath the canopy to watch the bolt and was struck on the upper lip by a small piece of shale (diameter of a softball) leaving a laceration that required sutures to close. Treated and released to unrestricted duty returning to work his next scheduled shift.

EE was traveling to section on mantrip when a piece of draw rock fell and hit him over the right eye brow. Accident happened half way up #4 roadway. The piece of rock was 12 X 32 X 1 thick.

While walking around a ram car was struck on the head and left shoulder by a piece of falling shale. Transported to St. John's Hospital in Springfield and released to see her own physician Tuesday, April 28 pending a release to return to work. This became a lost time after her personal physician ordered no work until results of an X-ray are determined.

While roof bolting and withdrawing an extension from the roof a piece of shale fell from the roof striking him on the head, neck, right shoulder and right arm causing pain and an abrasion. A secondary consultation diagnosed a cervical strain and placed him on a weight and lifting restriction.

While roof bolting and drilling a hole to install a rib pin a small piece of shale fell hitting the forward edge of the canopy and striking him in the left upper lip leaving a small laceration in the lip that required sutures to close.

Employee was installing a roof bolt in 2LT x-cut. When he pushed the bolt up a piece of rock was dislodged from the top(measuring 16" long, 8" wide & 6" thick tapering to a point)& hit him in the face lacerating his upper lip (left side) & breaking one of his teeth. The injury required 12 stitches to close.

While bolting a chunk of shale struck him on the left foot causing pain. He was turning to run and was unsure if the rock bounced off the canopy or hit his foot directly. Transported by private vehicle to Memorial ER where X-rays revealed no fracture. Treated and released with a "sitting only" restriction and returned to unrestricted work 10/17/2008.

While loading glue into bins on his bolter and walking thru a curtain he was struck on the right wrist by a piece of shale falling between bolts causing sharp pain and abrasions to the wrist. Transported to Memorial ER where x-rays revealed no fracture, released to his own DR. where he was placed on a "no use of right hand" restriction. He has not returned to full duty.

Changing batteries on Ram Car at charging station when the operator went to unhook jumper cable from the batteries. A piece of draw rock fell hitting him on his left shoulder and back. The roc k measures 15 inches wide, 37 inches long and 0-2 1/2 inches thick.

While bolting a rock fell from the roof striking his right little finger while it was on top the wrench. X-rays revealed a fracture at the last knuckle. He initially returned to work without restriction but a follow up visit scheduled with a surgeon advised installing pins in the joint placing him on a limited work restriction 10/6/2008.

A rock fell from between bolts and scraped down injured's back. Examined by ER and no treatment given. Injured was unable to RTW next scheduled shift due to ER Dr.'s refusal to grant full release to RTW and injured being unable to see family physician (who granted full release to RTW) until 2 days later.

While operating a continuous miner, a piece of shale slid off the miner and struck the operator on the left foot. Transported by private vehicle to St. John's Hospital Springfield where an X-ray revealed a fracture to his big toe. Released with minimal restrictions and did not miss any work.

While bolting the operator had his left hand and arm on top of the canopy holding a 4? guide bolt. As he looked behind him a piece of rock and shale fell from the roof striking his arm leaving a puncture and laceration. Transported by ambulance, X-rays revealed no fractures. The wound was not sutured due to the severity of the swelling.Began missing work 9/23/2008.

Rock fell, striking the individual in the lip.

While bolting, shale and clay fell from the roof, striking the operator on top the hard hat. He was brought to the surface and transported to Memorial Medical Center in Springfield. A follow-up visit the next day prescribed therapy for a cervical strain and he was placed on a ten pound weight restriction with no overhead work.

Injured was scaling mine roof, while doing so another rock fell and struck EE's little finger. Tip of finger was lacerated and required 6 stitches.

He was reaching for a wrench during roof bolting and had his hand positioned on the pot of the roof bolter, when a piece of rock fell striking his hand.

Employee was operating a Fletcher Roof Ranger on the left side. Upon completion of drilling he was in the process of removing the drill steels from the hole when a small rock fell from the roof and struck him atop the left hand causing a fracture to the metacarpal bone.

While bolting the 6R blow-thru and at the last short row before entry 7 EE was inserting a bolt, when a rock fell from the brow, striking him on the head and right cheek. A puncture on the cheek required sutures to close. He was released to return to work without restriction.

Employee was traveling from #3 belt line to #4 head drive on a Johnson Industries 2 man carrier, when a piece of rock fell from in-between the roof bolts hitting on the Johnson carrier. The employee had his right hand on the steering wheel and the rock struck his right hand and left leg. The rock measured 48" long 24" wide and 5" thick.

Roof bolter operator was injured while drilling bolt hole, a piece of draw rock fell, hitting him on right hand. Had to have stitches in hand.

A piece of shale fell out while he was removing a drill steel from the roof, landing on the drill mast then falling over and hitting his right thumb against the steel. The thumb swelled and was diagnosed as a small fracture. Released to work without restriction.

While examing the faces before beginning work on 4/15/2008 the employee was struck on the right shoulder and neck by falling shale. He did not seek treatment until 5/6/2008 and was diagnosed with a stretched tendon and his work activity was restricted.

While inserting a drill steel into the mine roof, a piece of shale approximately 3' X 8" fell and struck the operator on the top of the left foot. Diagnosed as a fracture of the big toe. Employee returned to work with no restrictions.

While scaling down loose shale, a piece fell, hit canopy of bolter and bounced off and struck EE on upper lip. Resulting laceration required two sutures to close, after treatment EE was realeased to return to work
