REPORTABLE ONLY DUE TO SUTURES - Employee had put e-bolt in and was under the shield on the longwall when a rock fell glancing off an employee's hard hat striking his left hand causing laceration requiring sutures.

Employee's shift started on 6/15/15 at 11:00 PM. Employee was pulling shield #42 up and as it set he moved to pull shield #41. Rocks fell between shields and cut the right arm right above the elbow. Sent out for treatment and received 15 stitches.

Degas crew was moving the degas drill. As the employee was loosening the jack a rock fell hitting his right shoulder knocking him down and he fell back on his ankle causing fracture to right ankle.

Employee was blowing up a shield and the tip of the shield was under the previous shield. When the shield tip slid from under the other shield, a rock fell either off the shield or from the top striking employee on the right hand. Employee had surgery on 5/29/15.

SUTURES ONLY - Employee was standing between shields 3 and 4 behind the dog house operating tailgate end of shear, when the shear entered the stump of coal a rock hit the employee on the face causing laceration requiring sutures.

Employee was operating a continuous miner on 3 section and was cutting #2 entry, standing at the back of the miner loading a shuttle car when a rock fell out of the top from between the row of pins striking him on the head and shoulders causing fracture to collar bone and broken teeth.

Employee was operating a Continuous Miner when a piece of rock fell out from between two roof bolt channels striking employee on top of left foot causing contusion.

Employee was on top of roof bolter cleaning the top off to load up material when a rock fell from between two rows of bolts and roof channel striking him on his right lower leg causing fracture to lower leg.

Employee was taking down a 4X4 timber and curtain which caused a loose rock to fall from the top striking employee on head and shoulders resulting in a strain to his neck.

Employee was helping set a sand jack, when the jack touched the roof a small piece of scale rock fell hitting his right wrist causing laceration requiring sutures.

Employee was operating the shear on the longwall when a rock fell out of the roof on the face side of the shear and bounced off top of the shear, landing on employee's leg and foot causing fracture to ankle.

REPORTABLE ONLY DUE TO SUTURES - Employee was taking down cap block material at the top of brattice when a rock became dislodged and fell striking employee in the face causing sutures to his lip.

Rock fell in between two shields at the tailgate striking employee on the foot. Employee's first date of loss is 3/8/2014.

Employee was struck on the hand by a rock that fell out between the shields. Stitches were required. Fx to the 5th finger and a partial tendon tear. He is back to work.

Employee was on ladder installing spads when he began to drill, a piece of rock broke and fell with the drill still in it pulling employees arm down with it causing pain in shoulder. Employee has been working since date of accident and has now been taken off work. Employee did not start losing time until 2/18/14.

Employee was stacking roof plates on the roof bolter tray when a piece of rock fell striking employee on right hand causing a fracture.

Employee was walking off face when a rock struck him in the mouth, breaking two teeth.

REPORTABLE ONLY DUE TO CHIPPED TOOTH - Employee was installing two sticks of resin in the hole when a small piece of rock flaked off falling between the roof bolter canopy striking employee in mouth causing small chip on front tooth.

Employee was standing at the front of the roof bolter scaling down loose top when rock rolled off rib striking employee causing him to fall and twist left knee. Roof bolter was located in the # 4 entry. Torn ACL and meniscus. Referred to Dr.

EE was observing the roof bolting crew installing wire screen on the rib when a piece of rock measuring 39" x 37" x 6" fell out from between bolts in the mine roof and struck him on top of his right foot causing fractures to 3 of his toes.

EE was moving C.M. cable out of the haulage roadway when a piece of rock fell out from between bolts in the mine roof and struck him on top of his left arm causing a laceration that required staples to close. Returned to work but dirt inside the opening caused him to return to hospital where he has been taken off work at this time. This is a LTA.

Top was hanging about 50' behind trailer and 50' or more behind line shields. Top bumped and broke and fell taking out supports on crib line striking employee in his ribs and head. The top took out 3 complete cribs and approximately 60 crib blocks. Blew crib block from 5' to 9' from where they were built.

Employee was struck in the hand with a rock while riding in a mantrip.

EE was performing routine maint. repairs on C.M. when he removed his left glove to take out planetary plug to install fluid, a piece of rock meas. 4"X 7"X 1/4" thick fell out from between bolts and hit him on his left hand causing a laceration which required 6 sutures to close. EE has been released to full duty on 3/6/12.

Employee was supplying roof bolter when a piece of rock fell out from between bolts and struck him on his left shoulder and arm causing a laceration to his left arm which required 5 sutures to close and he was released back to full duty on 2-3-12.

Employee was standing on right rib, behind miner, waiting on shuttle car when a rock fell from the top striking employee on right ear and shoulder.

Employee was helping with belt repair when rock fell out of top hitting employee in the head.

While lifting a rock with comealongs, a rock fell and struck employee on the hand

REPORTABLE ONLY DUE TO FRACTURE AND SUTURE. Employee was cleaning shields off and a rock fell and struck him on the left hand causing laceration and fracture to middle finger.

Employee stated that while working on the face at the new longwall, the gob came down and a piece struck his bicep. Employee first lost work day did not occur until 06/16/2011.

Employee was walking around roof bolter where a piece of rock had just been pulled on the opposite side of the machine when a piece of rock measuring 24" x 36" x 1.5" fell out from between rows of bolts and struck him on top of the head. Bolts were 3'4" from rib and 4' from row to row. Put on restricted days 5/4/11 - 5/7/11.

REPORTABLE ONLY DUE TO STITCHES: Employee had finished pinning and was under supported top and lowered the canopy to allow for advancement and a rock slid off the canopy and struck him on the right side neck.

Employee was raising canopy up to top and rock fell flipping back under canopy and struck employee in head, fracture to nose.

REPORTABLE ONLY DUE TO STITCHES. Employee was doing ventilation repair and loose rock hit him on the left lower arm causing a laceration requiring stitches.

REPORTABLE ONLY, NO LOST TIME. Pulling loose rock down and a piece fell and hit his face. Sutures required on his nose. NO LOST TIME, REPORTABLE ONLY.

While an employee was scaling roof, a rock fell striking the employee on the left middle finger. Sutures required. Injury Only. No Lost Work Days.

REPORTABLE ONLY: A rock fell between the shields and bounced off the pan and struck the employee in the mouth cutting his lip and breaking his front tooth.

REPORTABLE ONLY. Employee was riding on a manbus and a small rock fell out of top and hit employee in the mouth chipping two teeth.

REPORTABLE ONLY DUE TO STITCHES AND FRACTURE TO TOOTH. A rock fell from between channels and hit employee on the head and neck and pushed his head into the bolter tray, cutting his lip and breaking his tooth and cutting his chin.

REPORTABLE ONLY. NO LOST TIME. Pinning roof, rock fell from top, hitting his left lower arm. Sutures required. NO LOST TIME.

Working on suction line when a rock (26"x27"x2") fell from roof hitting his lower back.

Employee was operating manbus and rocks fell and struck him on the head EMPLOYEE DID NOT START LOSING TIME UNTIL 4/5/2010 DUE TO SURGERY.

While cleaning shields off to pull to track to loadout, a rock from top hit shield and fell on employee.

While working on Cowl blade, a rock approximately 6" X 5" fell on back of his head and neck and drove him to the ground.

Employee was pinning in #1 Entry Face and a rock approximately 12" X 4" fell from the top and struck the employee on the right shoulder. EMPLOYEE DID NOT START LOSING TIME UNTIL 7/16/09.

Employee was sitting on the manbus and a rock fell out of the top and struck him on the head. REPORTABLE ONLY DUE TO STITCHES
