The employee was installing roof bolts in the #3 right crosscut (3rd bolt in the 3rd row) when a kettle bottom measuring 70" X 57" X 0 to 8.5" fell & struck him on the left side & trapped his left leg under the rock. A co-worker removed the rock from his leg. First aid was given and emp. was transported to the hospital.

EE was drilling a bolt hole for the 3rd bolt in the 1st row of a cut just taken out in the no. 2 heading. After drilling approx. 8 - 10" into the roof the drill steel hit a crack in the roof. The rock infront if him started to work and he started to run back to the back of the machine. A slick ensided slip fell from the roof. A piece of this rock hit the side of his foot.

Piece of draw rock fell, striking EE on right hand while shoveling belt. Rock size 17" long, 8" wide and 1' thick.

The employee was operating the continuous miner by remote when some draw rock fell on the miner. The employee backed the miner out of the place in order to remove the drawrock. As he was pulling on a large piece of drawrock, a piece 37" long x 14" wide x 0" - 3.5" thick broke out and slid down onto his little finger and outside of his right hand.

The employee stated he was shoveling and rock dusting around the R-1 Belt Drive. He stated he hit the roof at the edge of the drive hole with the shovel handle and when he bent over to shovel, a slug of draw rock struck him in the back and slid down his right shoulder.
