JV 18 Mine Coal

This former employee was terminated at 4:20 PM on this date. As they were leaving they asked another employee who was unaware of the termination for a radio. At approx 4:40PM they used the radio requesting an ambulance, alleging that they had fallen down the stairs, hurting their back, leg, arm & ankle. Upon examination by a doctor, no injury was found about their body.

Employee was loading scoop bucket w/cinderblock when EE's thumb got caught between blocks injuring EE's right thumb.

EE was making a splice with knife & knife slipped & cut left hand between thumb and pointer finger.

Employee was cutting heel from boot with a knife. The knife slipped & cut wrist. EE received 1 stitch & returned to regular shift later that night.

EE pulled scoop up to load & turned scoop off & dropped bucket on ground. When exiting scoop began to roll back causing ee to twist and fall. EE complained of pain in left knee. EE has not returned to work since the day of the DOI, 11/6/17.

walking behind machine & it slewed left & edge of canopy hit EE in the chest. EE's momentum placed EE close to the canopy. EE was briefly pinned against the rib.

During on-shift exam of the section, the employee noticed a substantial piece of draw rock hanging in the #6 entry. They pulled the rock down with slate bar. As rock fell, sharp edge of rock caught/struck them on right forearm. Resulted in 4" laceration requiring 7 stitches. They did not have any lost time.

EE & another employee were replacing top roller at mainline #4 belt drive. EE bent over attempting to lift roller up on belt rail when EE strained EE's back. EE went to local hospital and was diagnosed with pulled muscle.

Employee was re-hanging cable on off-side(blind spot) of shuttle car in face(#4 entry cut), when the loaded shuttle car moved & bumped them. They did not seek medical attention until 07/10/2017, stating their back was sore. Employee has lost time, but returned to work on 07/12/2017 regular/full duty.

Employee was taking guard off scoop charger w/help of another employee. They were trying to lift it off when injured employee dropped EE's end catching EE's 3rd finger on left hand between guard & charger. Went to ER received 2 stitches, return to work next day. NO loss time.

Employee was traveling from #3 Break to the continuous miner area when a coal/rock rib roll occurred. The roll struck the employee about waist high, pinning employee down to the mine floor. Rock was approximately 26"x27"x10".

Putting panel board cover on electrical box on section Joy Continuous Miner #2, cover was caught & employee pulled on it, heard something pop & felt pain in lower back & right leg.

Employee was moving miner cable and injured EE's back.

Employee was pulling miner cable slack. A shuttle car came around the corner catching EE's left ankle between the shuttle car and a small rock.

The roof bolter operators left hand was caught in a pinch point when the offside operator engaged the control to lower the ATRS. This resulted in fractures to the ee's left hand.

On 6/26/13 Dominion Coal received notification from MSHA that the ee had exercised his option to work in a low dust area.

The Human Resource Department received notification from the U.S. Dept. of Labor that the ee had participated in a medical examination authorized by the Federal Mine & Safety Act and this examination indicated that the ee has a sufficient degree of pneumoconiosis.

The superintendent was given letter of correspondence by employee from the dr. on 1/14/13. The employee has been diagnosed as having CWP.

On 7/5/2012 Dominion Coal received notification from MSHA that the ee had exercised his option to work in a low dust area.

The tram stopped at the #4 belt drive to give the belt man a ride to the #3 drive. As the tram left the #4 drive the hood of the belt man's jacket caught on a roof bolt & he was pulled over the back of the operator's compartment of the tram. This injury resulted in a laceration requiring 16 stitches to the employee's chin from his jacket zipper.

The ee alleges an injury to his back while dismounting a personnel carrier. On August 18th the ee denied any event that caused injury. The ee left the mine and mine property on his own without medical assistance.

An ignition occurred at the continuous miner in the face of the #1 heading on B-Section. The miner operator was cutting on the left side of the heading when he observed a flash near the right side of the ripper head of the machine.

The employee was using a scoop to transport his tool box to a different location. The incident has been investigated and the employee has been interviewed but the cause is unknown. The ee was taken to the hospital for examination and was released to full duty but did not work. The mine was notified on 7/29/11 that the employee was being treated for a head injury.

Employee had been called out early to repair an electrical problem. When he arrived the shift elect. had repaired the problem. He began working on the communication & tracking cable when he called for help due to chest pains & breathing difficulty. He was taken to the surface & was being transported by ambulance to the hospital when he experienced a massive heart attack.

The steel cable for the B-2 belt drive take-up unit had traveled outside of the wheel guides. The injured employee was attempting to return the cable into the guides when the trapped cable became free and his left hand was caught between the steel cable and the wheel. This resulted in a laceration to the employee's index finger on his left hand.

The employee was checking the amount of supplies on the truss bolting machine. He was positioned on the operator side of the machine with his left foot near the front tire. As the bolter was trammed forward, the employee's foot was caught-under the machine tire resulting in a fracture to the employee's foot.

Employee was closing a swinging air lock door when the ventilation current pulled the door from his control resulting in the employee twisting his knee.

The section mechanic had completed repairing the water supply line to the continuous miner which was located in the #4 right crosscut. As he was traveling through the crosscut to the #5 heading to return his tools to a personnel carrier he tripped on a rock measuring 2.5 x 5 x 6 inches and injured his left knee area.

Employee was uncoupling hitch from supply car. When the coupling pin was removed the hitch caught the employee's finger between the hitch and the supply car-resulting in a laceration to his index finger on his right hand.

A 7X16X2.25 deteriorated section of the roof strata became dislodged around the permanent roof bolts and fell on the manbus trackway. The area had been bolted in the mid 90's using 6 foot conventional bolts.

The emp. was operating a track manbus traveling to the #4 Belt Drive to begin cleaning belt. Approaching the track switch at the #4 belt drive, another emp. was at the switch to throw it when the injured went through it. The injured was looking & talking to others as he was moving through the switch & caught his head against the roof & jerking his head backward.

EE trammed the dual head roof bolting machine from No. 6 heading to No. 5 heading. He backed the machine up into No. 5 heading to turn around & take bolter to No. 1 heading. As he was backing up the bolter-cable fell off the back of the bumper. EE attempted to put the cable back on the bumper with the bolter still moving & caught his hand between bolter & coal lying on rib.

The fan blades on the exhaust mine fan broke out. A piece of a fan blade came through a small opening between the fan housing shroud & the connecting tubing & struck the contractor employee on the left foot behind his big toe causing a laceration that cut tendons & fractured his foot.

High voltage cable was being pulled back and loaded into a rubber tired flat car. The car was being pulled by a scoop and as the car was moved forward, the EE had his left foot in front of the wheel and the wheel of the car ran over his foot.

Employee was roof bolting back area. Installing roof bolt and had to bend the roof bolt to place bolt into the head of the roof bolter. Holding the roof bolt he raised the head up to the bolt. As he placed the roof bolts head into the head of the bolter, he accidentally hit the rotation lever instead of the up lever to raise the head. His glove caught on the bolt and his fingers

The employee was helping another employe put a compartment cover back on the roof bolting machine. The cover had been removed to wash the compartment out. The employees lifted the cover back on to the roof bolter and were sliding the cover in place when the employee caught his finger between the cover and the frame of the machine.

EE performing a preshift examination of section & observed a loose rib in the #4 heading near the pillar line. He obtained a pry bar & took down the loose rib. Additional material came off the rib behind the material he had just pulled. Some of the material brushed his left leg & he fell forward. Additional rock & coal fell on his forearm.

EE as positioned approx 2-3' from the coal rib, when a section of the rib collapsed and struck him on the back. The EE continued to work and perform his normal job duties. He went to the doctor on 12/16/08 and was referred to a specialist by the doctor. On 12/22/08 we were informed the EE has a compression fracture.

While performing a belt conveyor move the employee was attempting to route a flexible water line under the conveyor when his rescuer became hung on the belt structure of the idled belt conveyor. While attempting to free the rescuer the employee felt a pain in his back.

The EE was bolting the No. 2 left crosscut and had drilled the last roof bolt hole in the place. The coal height was too low in the area to put the 42" resin roof bolt in the hole. As he was bending the roof bolt slightly to get the bolt in the hole, he felt a pain in the right side of his lower back, near his rib cage.

EE was shoveling belt with his foot, positioned between two plastic water lines,when he twisted his right knee.(EE had returned to work on 4/3/08 from an accident on 2/29/08 concerning his knee,worked 9hrs on 4/30/08 and 4hrs on 5/1/08,then came out of mine and reported to supervisor that his knee was bothering him again.

The production crew was leaving the section at the end of the shift. As they were traveling to the end of the track in the bucket of a battery powered scoop, the employee apparently lost his balance and it is believed he struck his head on the cap lamp of the employee next to him. The blow cut his ear.

The EE was attempting to put a wheel unit on a shuttle car. He was pulling the wheel unit onto the shuttle car with a come-a-long, when unit turned over. He placed one end of a 48" piece of drill steel through one of the lug nut holes and attempted to upright the wheel unit. As he was prying, he stood up and the drill steel came out and he fell against the shuttle car.

Employee was putting a conveyor chain on a shuttle car when a piece of metal came off the chain and went into his left hand.

EE was bolting the top and the bolt slipped off the head and hit his left hand.
