Employee was cleaning on the west 4 belt at cross cut 73 when a bad bottom 72" belt roller that was against the rib slid down hitting employee on the top of right foot.

Mostrando los 13Knuckle boom truck operator was removing the bullet from D11 roller frame. The bullet rolled off the track toward the truck as the operator was walking to the back of the truck. Operator jumped the bullet. Bullet hit the steer tire on boom truck and rolled back toward D11 hitting the left leg of operator. Operator was knocked under boom truck.
Employee had thrown cable over out of the way and when he turned he walked into a bit bucket bolted to the roof.
Employee was sitting on the bench in the washhouse changing clothes when he said a basket fell and struck him on the back of his head. He has continued to work and was taken off on 06/03/2010. MRI's of the neck and back were done and showed Multilevel degenerative areas and Minor Lumbar degenerative changes. Spurring was also found.

He put his hand down on a block as he was going thru curtain and a block that was left hanging in the roof fell and struck his ring finger right hand.
Man was working on elevator when metal object fell from overhead striking his head. Man was tied off and had hard hat which took the blow.
The employee stated he was beside the roof bolt machine when the operator started it and walked to the back of the machine and started to cross the trailing cable on the machine when the operator acitivated the cable take-up and the cable caused him to set down on his left ankle.
HELPING CO-WORKERS SPLICE THE #4 UNIT FACE BELT, HE WAS LAYING UNDER THE BELT ON HIS STOMACH DRIVING A BELT SPLICE PIN THROUGH A NEW SET OF BELT CLIPS. THE BELT CHAIN HOLDING THE BELT STRUCTUR E SLIPPED FROM THE FRAME SLOT ALLOWING THE STRUCTURE TO FALL WITHOUT WARNING ONTO HIS BACK. THE FORCE OF THE STRUCTURE STRIKING HIS BACK RESULTED IN A HAIRLINE FRACTURE TO 1 RIB, RIGHT CHEST.
EMPLOYEE REPORTED: RIDING IN MANTRIP AT END OF SHIFT, SMALL PARTICLES FELL FROM MINE ROOF, CAMEOVER HIS SAFETY GLASSES AND LODGED IN HIS EYE. WHEN TRYING TO REMOVE THE FOREIGN PARTICLES FROM HIS EYE, HE SCRATCHED HIS EYE BALL RESULTING IN PAIN AND IRRITATION. HE CONTINUED TO WORK UNTIL MONDAY SEPT. 17, 2001 AND ON THIS DATE STARTED LOOSING SCHEDULED WORK DAYS.
