EE was standing 10' inside #5 kiln when 10 lb. piece of coating which had been deemed stable, fell hitting his hard hat resulting in a concussion, teeth damage, and two small superficial scratches on his head. Lost time case.

EMP WAS WALKING OUT OF BURN BLDNG. UNDER REXNOUD WHEN A PIECE OF 3'X 3' PLYWOOD FELL FROM TOP OF REXNOUD STRIKING HIM ON RT SHOULDER, ELBOW AND KNEE. EMP CONTINUED TO WORK UNTIL 10-27 WHEN HE WENT TO DR. RX THERAPY UNTIL 11-2 THEN RELEASED.

EMPLOYEE WAS TRAPPED INSIDE TOW ER BETWEEN 2 LADDERS. EMPLOYEE WAS HELPED OUT OFTOWER WITH PAIN IN KNEE. MRI TEST SHOWED EMPLOYEE HAD TORN CARTILIDGE. OPERATED ON KNEE MAY 9, 1990.
