Injured was moving the fork of the telehandler and pinched EE's finger between the fork and the headache bar of the machine. Injured suffered a laceration as a result of this and received stitches for this injury.

Injured was reclaiming belt structure and smashed the tip of finger causing a laceration and a fracture.

Injured got a foreign body in right eye and had to have it surgically removed. Injured also had prescription prescribed for the injury.

Injured bent over to pull a piece of steel out of a scoop bucket. As EE lifted the steel EE felt pain in right knee/leg. EE followed up with the Doctor on 9/20/2017 and it was determined EE strained lateral hamstring. EE was placed on restricted duty and will have 4-6 weeks of physical therapy.

Injured reported on 9/7/2017 that the injured had strained back either on 9/5 or 9/6 while hanging a curtain. Injured was sent to Doctor and was prescribed a muscle relaxer and steroid making this a reportable incident

Injured strained a muscle in their back while performing routine maintenance on the roof bolter. The injured was seen on 8//28/2017 by doctor and prescribed a script for a muscle relaxer for the muscle.

Injured broke the tip of 4th finger on right hand, after hitting it with a miner bit. While trying to clean out the clip area for holding the miner bits in the insert, the injured hit finger with a bit causing a small fracture on the tip of finger.

Injured strained back on 8/4/2017 and continued to work on 9/28/2017 injured had an MRI performed and it was discovered injured had a herniated disc in back.

injured was hanging a curtain and felt a pain in shoulder area, but continued to work. Pain continued to progress and injured had an MRI scheduled and on 8/16/2017 injured was diagnosed with a tear in the muscle in the shoulder and placed on restrictions.

Injured was installing a thumb on a 345 excavator. As injured was moving a weldment a piece of steel pivoted and cut their arm. 8 Sutures were required to close the wound.

Injured was cleaning rocks of the top of the miner and as employee was pushing the rocks off the employee lacerated the right hand and received 5 stitches.

Injured was lifting a cable and strained shoulder. Continued to work but had discomfort on 12-23-2016 injured had MRI performed and on 12-28-2016 was informed that injured had a torn rotator cuff in the shoulder.

The injured was in the process of setting jacks when the bands on a new bundle of jacks were cut off the load of the jacks shifted and the injured attempted to stop the load when EE's finger was caught between two jacks causing a laceration. The injured received stitches to EE's finger.

The injured was in the process of removing a gear box from a bin roller when the gear box cleared the shaft it made an upward motion pinching index finger between the motor pedal and the motor causing a laceration. The injured was seen by a doctor and received stitches.

The injured was installing a bottom roller at the M7 drive. Upon lifting the roller the injured felt a pop in stomach just above belly button. Upon being seen by a doctor, was diagnosed with a hernia.

The injured was assisting another employee with moving a belt bar. The Injured smashed EE's right index finger between the belt bar and a pallet of block causing a fracture to EE's index finger.

Injured was installing Ziggy rails and jacks on the S4D belt line and lacerated pinky finger on right hand. The injury required 5 stitches.

Injured hyper extended left knee while moving miner cable on the #2 unit. The injured was placed on restricted duties which prohibited EE from doing normal job duties.

The injured was lowering a 40 foot extension ladder down to the floor with another employee to move the ladder to the needed location when the second employee lost their grip on the foot end of the ladder. This resulted in the weight on the ladder coming down on the left shoulder of the injured.

The injured was pulling down a curtain when EE twisted right knee. The injured went to the doctor and it was discovered that EE's ACL and Meniscus was torn in EE's right knee.

The injured was installing a saddle in a beltline, when EE was pulling the saddle under the belt EE felt a sharp pain in lower back. The injured was placed on restrictions by a doctor.

The injured stated that EE had a sharp pain in EE's right shoulder after loading supplies on a roof bolter. The injured was seen by a doctor and placed on restrictions.

The injured was lifting a piece of 8 inch aluminum waterline when EE felt a pain in their lower back.

The injured was hanging power cable with tie wire when the tie wire can in contact with EE's left. This resulted in a abrasion to the corneal of EE's left eye.

The employee was in the process of setting a jack and took hand off the jack to do another task. This resulted in the jack falling over striking the injured in the right foot.

The injured was in process of building a crib. When a crib block was handed to EE, EE lost grip of the crib block dropping it. When EE attempted to catch the crib block EE's pinky finger was smashed against another crib block resulting in a fractured finger.

The injured was unloading C-Channel from a Dapco trailer and lost grip of the metal while moving it. The C-Channel landed on the top of right foot causing a fracture.

The injured was washing the company bobcat in the wash bay of the surface shop, the safety lock holding the cab up appeared as if it was hit and dropped the cab down on the injured landing on his back.

The injured was removing a crib block from underneath the operator side of a roof bolter. While picking up the crib block the injured felt a pain in his groin area.

The injured was hanging bolter cable when he injured his left shoulder. After going to the doctor it was identified as a torn labral muscle in his shoulder.

The injured was lifting an 18 foot ziggy rail when he felt a pain in his right shoulder. Continued to have pain and upon visiting the doctor an MRI was conducted and it was discovered that he had a torn Labrum in his shoulder. Surgery Scheduled for 12-21-2015.

The injured was in the process of hooking up a tail piece during a belt move when the waterline tree came off allowing the water in the waterline to flow out. The pressure of the water coming from the waterline struck the injured in the leg pushing his leg into a stationary scoop bucket.

The injured was bolting when he attempted to wipe mud off his drill steel to find the drill depth mark on the steel. When the injured put his thumb on the steel it became caught in the drill steel.

Injured was closing the battery lid and smashed his left index finger between the frame of the battery and the lid.

Injured was operating a twin head roof bolter. His drill extension was stuck in the chuck of the drill boom. As he pulled it out it hit him in the mouth causing a laceration and chipping a tooth.

Injured was changing out a basket in the prep-plant. As he was cleaning the old basket a piece of dirt got in his eye. He went to the Dr on 4-13-15 and his eye was flushed out with prescription medication.

Injured smashed his left ring finger with a Ziggy Rail while unloading them.

Injured smashed his right middle finger while changing bushings on a bucket for a scoop on 3/2/2015 and did not report incident until 3/9/2015. Injured was seen by doctor and had a fractured finger.

Injured was loading a machine door in a scoop. As he was lifting the door it slid down the bucket of the scoop and struck his leg.

Injured was exiting cab of crane. Injured lost his footing and all of his weight shifted to his right arm while holding onto the grab handle. Right shoulder became dislocated. This is a aggravation of a previous injury. Should was put back into place at Good Samaritan Hospital Emergency room.

Injured was stacking roof matting against a coal rib. The roof matting fell over and hit his knee causing a strain.

Injured was throwing continuous miner cable against the coal rib. As he picked up the cable he strained his left shoulder and was put on restricted duty.
