Injured was moving the fork of the telehandler and pinched EE's finger between the fork and the headache bar of the machine. Injured suffered a laceration as a result of this and received stitches for this injury.

An unintentional roof fall has occurred in the #9 Entry at the 6XC of the S1C. The fall was approximately 30 feet in length and 18 feet wide by 14 feet high.

The encoder shaft that has electronic transmitters to send back to the PLC to let the hoist no its location inside the shaft was damaged and would not allow the transmitters to relay information back to the PLC for the hoist to operate.

Injured fell when EE tripped over the power cable and fell on EE's back and landed on EE's SCSR. Injured was placed on restrictions for a lower lumbar strain.

Injured had a foreign body in eye which caused a scratch to eye. Injured was given a prescription for the injury.

Injured was making a splice on a cable, when the knife slipped and cut left hand. Injured was taken to hospital and received 6 stitches for injury.

Injured was a passenger in a man-trip which struck a roof jack which was installed in concrete along the travel-way. The corner of the bumper caught the jack causing the injured to strike EE's knee against the dash of the mantrip.

Injured was stepping out of EE's D10 Dozer at the end of EE's scheduled shift. As EE stepped out EE lost EE's balance, EE's foot slid between the track and dozer frame, EE fell backwards fracturing EE's left leg below the knee.

Injured was walking and twisted their knee after stepping awkwardly, continued to have pain. On 2/26/18 had an MRI and discovered has a medial meniscal radial tear in the left knee. Was placed on restrictions on this date.

On the morning of 1/12/2018 the area had a winter storm with ice and snow and the facility power kept flickering back and forth causing the PLC on the hoist unit to scramble its brain so to speak. The utility company supplying the power checked their lines and they discovered a line that had been hitting together.

The elevator for the south portal was inoperable due to a piece of ice on a governor rope which caused the car brake to not release. The elevator was fixed after approximately 45 minutes.

Injured was reclaiming belt structure and smashed the tip of finger causing a laceration and a fracture.

Injured got a foreign body in right eye and had to have it surgically removed. Injured also had prescription prescribed for the injury.

An unintentional roof fall has occurred on the S4D travelway. The fall was approximately 35' by 10' wide and approximately 7' high. An action plan to rehabilitate the area was submitted and implemented for the area.

Injured was walking and felt a pop in left knee and on 11/27/17 injured went to doctor and had fluid drained off of that knee. Injured was diagnosed with Patellar Tendinosis.

Injured got a foreign body in EE's eye and had to go to eye doctor to have eye cleaned out. At the doctor it was discovered EE had a laceration from the foreign object and received a prescription for EE's eye.

Injured was swinging a sledge hammer and strained a muscle in lower back, on 11/9/2017 injured was seen by doctor and prescribed a muscles relaxer for back.

Injured got a foreign body in left eye, went for treatment for injury to eye doctor and was given a prescription for injury.

Injured was building a stopping and stood up and stepped back and twisted left knee. Injured was taken to Hospital and had an MRI and it was discovered injured had a bucket tear of Meniscus of left knee.

Injured was using a grinder with a cut off wheel while working on a tail gate when the grinder kicked and struck the injured in the knee. This caused a laceration which required stitches.

Injured bent over to pull a piece of steel out of a scoop bucket. As EE lifted the steel EE felt pain in right knee/leg. EE followed up with the Doctor on 9/20/2017 and it was determined EE strained lateral hamstring. EE was placed on restricted duty and will have 4-6 weeks of physical therapy.

Injured reported on 9/7/2017 that the injured had strained back either on 9/5 or 9/6 while hanging a curtain. Injured was sent to Doctor and was prescribed a muscle relaxer and steroid making this a reportable incident

Injured broke the tip of 4th finger on right hand, after hitting it with a miner bit. While trying to clean out the clip area for holding the miner bits in the insert, the injured hit finger with a bit causing a small fracture on the tip of finger.

Injured strained a muscle in their back while performing routine maintenance on the roof bolter. The injured was seen on 8//28/2017 by doctor and prescribed a script for a muscle relaxer for the muscle.

Injured strained back on 8/4/2017 and continued to work on 9/28/2017 injured had an MRI performed and it was discovered injured had a herniated disc in back.

injured was hanging a curtain and felt a pain in shoulder area, but continued to work. Pain continued to progress and injured had an MRI scheduled and on 8/16/2017 injured was diagnosed with a tear in the muscle in the shoulder and placed on restrictions.

Injured was struck by the battery end of a coal hauler, as employee was working on miner cable along the rib-line. The injured suffered bruising along their side with no fractures or other injuries.

Injured was bringing up the pot to connect to the split steel when the drill steel caught the chuck and buckled, and struck the injured on left thumb. Injured was taken to clinic for x-rays where it was discovered injured had dislocated thumb.

Smoke was observed coming from the raw coal pile at approx. 6pm on 6/7/17. A track-hoe was dispatched to the spot observed & began digging our the spot where the smoke was observed. At approx. 8:45am MSHA inspector arrived on site and questioned if we had reported the incident. Part 50 is a fire not extinguished within 30 min not smoke! We were issued a citation.

Injured was checking curtains at the start of shift when a piece of draw rock slid off the side of the fence and struck the injured on injureds lower back. Injured was taken to hospital where it was discovered injured had a transverse process fracture in injureds lower back.

Injured was installing a thumb on a 345 excavator. As injured was moving a weldment a piece of steel pivoted and cut their arm. 8 Sutures were required to close the wound.

Injured was helping construct a structure at an outby location, and fell and hit the ground with left side and lower back (tail bone area). Injured complained of pain in back and was taken to hospital for x-rays. It was discovered with the x-ray that the employee had cracked one of employee's ribs.

The injured's left leg became wedged between a battery coal Hauler and the coal rib on the #1 unit after the miner backed into the hauler and pushed it back into the rib. After transporting employee to the hospital it was discovered employee suffered a fractured leg.

Injured was installing supplemental support into the roof and before ee began ee was scaling some loose roof down. As ee was scaling a piece of draw rock slid down and struck finger on left hand. This caused a fracture of finger.

Injured had a hole in the canopy jack hose and attempted to lower the canopy with their hand wrapped around the hose, so the oil would not spray them. This caused a pin hole laceration on a finger and allowed the oil to be injected into the finger. Injured have to have the finger medically cleaned out.

Injured was cleaning rocks of the top of the miner and as employee was pushing the rocks off the employee lacerated the right hand and received 5 stitches.

A 51 year old miner operator with over 30 years of experience was struck by a battery hauler, while making second cut in the #3 entry to the right. The injured was transported to local hospital for treatment of injuries.

An unintentional roof fall has occurred on the P3-1 at the 16th XC in the #9 entry. The fall was approximately 20'X 20'X approximately 13'plus high

Injured was doing repair work on piece of machinery, as employee was walking around the equipment injured stood on a crib block and the crib block shifted. This caused the injured to strain a knee.

Injured twisted knee while exiting the man-trip. On 12-28-2016 MRI was performed and injured had suffered a torn meniscus.

Injured was cutting the 2L on Unit 1 when employee was struck by a piece of carbide from a miner bit. The injured received 3 stitches from the laceration.

Injured was lifting a cable and strained shoulder. Continued to work but had discomfort on 12-23-2016 injured had MRI performed and on 12-28-2016 was informed that injured had a torn rotator cuff in the shoulder.

The injured was in the process of setting jacks when the bands on a new bundle of jacks were cut off the load of the jacks shifted and the injured attempted to stop the load when EE's finger was caught between two jacks causing a laceration. The injured received stitches to EE's finger.

The injured was in the process of removing a gear box from a bin roller when the gear box cleared the shaft it made an upward motion pinching index finger between the motor pedal and the motor causing a laceration. The injured was seen by a doctor and received stitches.

The injured was in the process of changing miner bits when a bit was struck with a hammer and a piece broke off hitting the injured in the hand. The injured had to have a small piece of metal surgically removed from the hand.

The injured was installing a bottom roller at the M7 drive. Upon lifting the roller the injured felt a pop in stomach just above belly button. Upon being seen by a doctor, was diagnosed with a hernia.

The injured was assisting another employee with moving a belt bar. The Injured smashed EE's right index finger between the belt bar and a pallet of block causing a fracture to EE's index finger.

The injured was dragging miner cable over when ee tripped and fell landing on left wrist. The injured was placed on restrictions by a doctor on 11/9/2016.

The injured was in the process of roof bolting when the drill steel became stuck in the hog panel being bolted to the mine roof. While the injured was trying to free the drill steel, it became free and struck left hand. On 11/9/16 injured was placed on restrictions and could not perform regular job duties.
