The employee stopped to fill the water truck. The employee got out of the truck and was standing on the ground. The employee started to step back upon the running board of the truck. As the employee pulled self upon the running board, EE hit the left side of head on the bottom of the metal bracket supporting the mirror, causing laceration to EE's head requiring staples.
Employee turning a crosscut in No. 3 entry. When EE bent over to move the continuous miner cable, EE felt a sharp pain in EE's lower back.
Employee was squatting down and hanging a shuttle car cable when EE felt pain in EE's right hip and leg. The employee stated that EE's hip locked, and EE could not move.
EE was throwing trash from a scoop bucket to a loader bucket to be hauled to the dumpster. When doing so, EE grabbed a metal oil can lid with jagged edges. The lid cut EE's 5th digit on right hand.
The employee was lowering the pinner cable from the roof of the mine when EE felt a sharp pain in EE's left shoulder.
The injured employee and co-workers had installed a new pump in a outby area of mine. The employees were hanging the 3" discharge line to the mine roof when the discharge line came off the pump and sprayed water and mud into the face and eyes of the employee after the safety glasses were knocked off. This resulted in a small tear to the retina of the left eye.
Employee was lifting a timber and turned and felt pain in mid to lower back.
Employee was taking a chain out of a belt hanger when the belt rail fell striking EE's left hand.
The employee was shoveling around the section tailpiece and felt a sharp pain in lower back.
Employee was removing a guard from the transfer jack off of a ram car, and when they lifted the guard they strained their lower back. Employee finished the shift and went to a local clinic for medical treatment.
Employee was replacing the guard over the tail shaft on a feeder. The guard slipped and caught the employee's left hand resulting in a simple fracture on EE's index finger. The employee did not go to the hospital until 01/22/2021 and was released the same day.
EMPLOYEE WAS DROPPING WORK BASKET IN THE LOCKER ROOM WHEN IT SLIPPED CUTTING RIGHT PINKY FINGER REQUIRING 4 STITCHES.

The employee had placed gravel in a pail. The employee was lifting the pail above EE's head to place on a upper level when EE's right shoulder and neck began to hurt. The employee continued to work until surgery was performed on the right shoulder on 8/06/2020. The employee is currently off work.
