A contract employee stated that EE dropped a single belt rail on EE's right foot. EE's foot was X-rayed and was negative for any broken bones.

On September 10th, 2025, a non-fatal accident occurred during the process of retreat mining on the 005-0 MMU. A rock struck a mobile roof support operator, which resulted in injuries to their lower leg areas.

The employee was taken off work by employee's physician on 10/28/2024. Employee was terminated per company policy on 01/13/2025.The company was presented a positive Chest Radiograph on 04/07/2025.

Employee was bolting mine roof and was attempting to guide steel into drill chuck when the drill steel slipped and cut EE's left middle finger. Required stitches to close wound.

Employee was pulling an MRS while another employee was pushing with a scoop. While pulling the MRS the scoop jack knifed and struck the rib causing employee to hit shoulder on the inside of the deck of the scoop. Original accident date was 09/30/2024 but employee continued to work until 01/29/2025.

Continuous Miner operator on the 006 MMU was installing a ventilation curtain when a piece of rock from the rib fell and struck them, causing lacerations to left arm and hand.

Employee picked up a bucket of plaster and twisted and felt a sharp pain in the lower back. Pain got worse throughout the shift.

Employee stated that EE arrived on the section on the mantrip and that EE was packing a bucket of bolter bits and dust filters from the mantrip towards EE's bolt machine and stated EE fell causing EE to strike EE's wrist on the bucket of bits.

Employee was in the parking lot of the mine office and was standing observing a machine part being loaded into a scoop bucket. The employee started turning in circles and fell face down into the gravel and started seizing. The fall caused cuts to the face and head.

Bolt machine operator stated that had lost suction on the roof bolter. EE assumed something was clogging the pre-dump area and attempted to clear any obstructions. In the process of cleaning out the obstruction, two of EE's fingers were smashed causing a laceration to the fingers.

Employee had hands placed on belt rail, observing belt and a piece of rock fell from the top and lacerated right pinky finger.

A diesel lo- trac that was located at Cross Cut #7 on the 6 left panel of the active 006 MMU caught fire. There was nobody operating or around the machine when it caught fire. There was no injuries associated with the incident and the fire was extinguished safely.

Employee was inserting a cable bolt into the mine roof and the bolt plate slid down and the corner of the plate cut employee's forearm.

Employee was operating a roof bolter and the coupling that joins the drill steel broke, causing the drill steel to strike EE in the nose and left cheek.

Employee and co-worker carrying belt roller. Injured tripped on anchor pin sticking up from the mine floor. Employee fell injuring Left wrist. Employee continued to work and went to medical facility on 8/22/2023. This accident is under investigation to determine if injury occurred on mine property.

Employee was removing belt structure and slipped and injured right ankle.

A non-injury roof fall occurred on the 2A belt line between cross cut #37 and 38. It was determined through investigation that the area had deteriorated over time due to the age of the mine.

Employee was next to the roof bolting machine while machine was being moved and the machine ran over EE's foot.

A Non-Injury Accident occurred on the MMU. No. 005-0 Section during retreat mining. A coal bump or outburst occurred on the 2 Left off South Submain. The coal bump or outburst was on the right rib of the No. 3 entry. All employees were withdrawn from the mine. MSHA was notified of this event. Upon MSHA's arrival to the mine site an investigation of the coal bump or outburst began.

A Non-Injury roof fall accident occurred in the No. 2 entry at crosscut # 10 on the MMU. No.005-0 Section during retreat mining, which stopped mining operations due to the remote control units would not operate the mobile roof supports to tram the units back away from the pillar line.

Employee was pulling a joint of waterline and slipped and fell striking their chin on a piece of mobile equipment. The employee received 3 stitches in their chin.

A non-injury accident occurred on 6/17/22. Elevated amounts of carbon monoxide (co) and methane gas detected in 3-LT Bleeder panel off 6 - South Mains. Though the elevated gas is due to geological lineament features crossing near this location.

Roof fall encountered around 12:05 pm on 06/16/2022. Roof fall located #2 entry 2 crosscuts inby left main surface exhausting fan on South Mains Panther side of mine. Fall is approximately 7' thick, 30' length, 19' wide. Also extends 10' in adjacent crosscut.

Employee was working on their knees helping to set timbers. Employee started to feel pain in left knee. As the shift progressed, the pain started getting worse. One medical report stated a fracture in the kneecap. A separate medical opinion stated that it was only gout.

Employee was carrying roof bolts to load onto tray. The employee slipped on a bolt wrench that was laying in the walkway.

The employee was lowering the pinner cable from the roof of the mine when EE felt a sharp pain in EE's left shoulder.

Employee was in the area where a bolt machine axle was being beaten out of the machine with a hammer. A small shard of metal broke from axle hub and struck the employee in the right forehead.

Employee was removing a grease hose at a head drive. A small piece of rock about 3 inches round fell from the top striking the employee on the left hand.

Employee laid off on March 27, 2020 due to Covid-19 issues and the mine closing. The employee was diagnosed with (CWP) Coal Workers Pneumoconiosis. The information was made available to the company on 12/28/2021.

Employee was leaning against rub rail of Joy Miner while troubleshooting was going on. As power was supplied to the machine, the employee felt an electrical shock.

Employee was moving the shuttle car cable anchor when EE received an electrical shock. A shard of metal was sticking out of the cable. This was found during the investigation.

Employee was lifting a timber and turned and felt pain in mid to lower back.

The employee was riding in on a man-trip and hit a pot hole. EE was bounced up and when EE came back down EE struck EE's middle back on the seat. EE then continued to work until the pain got worse. EE was then transported to the hospital.

The roof fall was encountered around 6:30 pm on 08/16/2021. The roof fall area is located at crosscut #79 on 1 East Mains, #3 Belt in the intersection. The roof fall is approximately 20' in length in the intersection on the beltline and extends approximately 35 feet into the 4 left crosscut and ranges from 2' to 6' thick.

Employee was taking a chain out of a belt hanger when the belt rail fell striking EE's left hand.

Roof fall occurred on the D-3 mains in the #3 entry, #3 crosscut, and measured approximately 20' in length. 20' in width, and 8' thick. This area is not an escapeway and is only traveled by a weekly examiner. No ventilation or travel was disrupted by the roof fall.

employee was bolting in the #2 heading when a piece of rock fell off of the canopy of the bolt machine hit left arm,requiring 4 stitches

Employee was installing an angle bracket in the #6th row of bolts in the #2 heading when a rock fell out of the rib striking EE in the lower back and hip.

employee was in process of pulling a stuck MRS out of the 6th entry when the pin for shackle broke. It hit the ground and ricocheted over in the x cut between #6 where IE was at, striking IE's left elbow and forearm. IE required surgery to remove a piece of metal lodged in IE's arm.

EE was bolting top when a piece of rib rolled out striking EE's ankle and foot. EE went to Whitesburg ARH. X-rays showed a broken bone in EE's foot and the small bone in EE's lower leg.

Employee was bolting the first cut out of the 4R crosscut on 005 when a slab of rib rock rolled off striking employee in the back and side. It appears there is a compression fracture to the vertebrae. Employee was taking off work and referred to ortho.

EE was on a shuttle car and pulled into a crosscut not fully getting out the way and was hit by another shuttle car. This has now became a loss time claim

This is an older area of the mine that was intake for 10+ years and is now return. The intersection that fell is at the bottom of a dip, and appears to have been a large localized horseback. There should be no effect to ventilation either short or long term, as there are multiple additional return entries. No one was injured in the accident.

The roof strata in the immediate area of the fall appears to be on localized stack rock that has deteriorated over time. THIS IS A NON-INJURY ROOF FALL

Employee was pulling feeder cable, felt sharp pain and burning sensation in right shoulder

Employee was changing hydraulic pump on section feeder when pump fell on EE's finger.

Employee was bleeding air from 1.5" water line, line jerked causing valve too fully open and the fittings slammed into employee's right forearm causing fracture.

Employee was operating shuttle car when employee ran over a rock, hurting back. Employee was placed on a back board and brought to surface then transported to ER by ambulance. CT was negative. Employee was taken off work 2/15/2019 pending further treatment.

Employee was drilling hole/bringing out steel, stopped halfway, dropped pot - when attempting to get steel out of hole, a rock fell striking employee finger against the drill pot.

Employee bolting SR punch through, a section of rock rib above the coal rolled off striking employee. Rock was 12' L, 2' W and 10" thick. Legs were trapped under one end of rock. Employee complained of leg pain and then back pain as employee was being removed from mine. Employee has fractures to right ankle, both sides of pelvis, spine and ribs.

While employee was attempting to crib up the pan on continuous miner, the chain broke catching employees' left thumb between the pan and the crib block.

While tramming a diesel scoop, ran over a rock in the roadway, hitting head on the top.

After helping change a set of scoop batteries, employee laid a hand on the scoop bucket. The operator inadvertently bumped the ram lever, catching employee's hand.

Employee was roof bolting on knees when the rib rolled off behind ee, striking lower back and rolling down onto legs. EE went to the ER, was treated and released. EE went back to the dr. a week later and was taken off work.

EE was moving miner cable & bent down to move cable. Coal rib rolled out on EE striking EE on the back of the head & neck. EE was transported to Holston Valley Hospital for treatment.

EE was driving a Mac 3D ride when EE felt something pull in EE's right elbow, EE finished EE's shift, then went to the ER & is to follow up with EE's doctor. EE's doctor has taken EE off work until EE see orthopedic specialist. Has loss time starting on 08/07/2017.

EE was spotting a corner bolt LOCC #5 entry inby right corner, when a piece of draw rock measuring 39"x10"x10" at widest point fell and cut left forearm. EE received stitches at the ER.EE has no loss time and return to work next day.

Installing roof bolt when EE spent the bolt next to the top & small piece of material went into EE's eye, EE had safety glasses on at time of accident. EE went to eye doctor & a piece of metal was removed from eye. There is no lost time.

Employee was filling in on a roof bolter when a piece of draw rock fell & cut right shoulder. EE went to ER received 4 stitches. No loss time, full duty return to work.

Employee was hooking up a manifold and got hit in the mouth by the hose causing tooth to come out.

Employee was removing steel from hole after drilling hole, a piece of draw rock fell & hit employee's hand cutting pinky finger on left hand. Employee crushed the tip of little finger & had tip amputated. Employee returned to work on 04/21/2017.

Employee was moving a miner cable and strained back.

Employee walked up to the miner operator to relay a message. The miner was cutting out for overcast and a piece of a roof bolt flew back from the cutter head striking ee in the neck, under chin.

Employee was putting hub back on a Mac 3 and felt pain in lower left back. Doctor took him off work starting 4/21/2015.

Employee states he was driving down roadway and hit a bump in roadway causing him to bounce off seat and strike his hard hat and head on top. Resulted in a stiff neck and tingling into both arms and burning in his feet.

Bates Contractor was trying to move a piece of draw rock out of road and strained his left shoulder and right upper arm. He continued to work up until a doctor took him out of work on 2/10/15.

A roof fall was discovered on the 3 East mains, 1 xcut out by SS#3011 in the #2 entry.

Employee got off the ride and was walking toward the power center when stepped in a hole and his right knee gave out. Employee tried to catch himself on the power center hurting his right shoulder.

Employee has filed a cumulative trauma claim with the Kentucky Labor Cabinet Department of Workers' Claims. As per Dr. report, employee has 22% whole person impairment. **THERE IS NO ACCIDENT ASSOCIATED WITH THIS CLAIM.**

Employee pulled a piece of draw rock. When he started to clear the rock from the roadway he twisted, then stumbled and fell. He experienced a sharp pain in his back and down his left leg and knee.

Per notice received from MSHA, Employee has been diagnosed with a sufficient degree of pneumoconiosis. Therefore, this report is being submitted. We have not received any notifications of his fillings per worker's compensation carriers(s) & neither have received notice either. This claim is not associated with an accident. Date of accident is date of notification.

Employee was bending strap resulting in a strain to his left shoulder. He has not missed any work.

Loading supplies onto a scoop and caught right forearm on a sharp edge of a strap. Resulting in 9 stitches and tetanus shot.

On 9/21/12 a bump/coal outburst occurred during retreat mining or pillaring on the MMU No. 002-0 section. The bump/coal outburst occurred to the coal pillar between the No.4 and No.5 entries at crosscut No.32 on the 1-left panel off the 4-east mains. The coal pillar where the bump/coal outburst occurred is approximately 15'feet inby survey station No.6609.

The employee was operating a shuttle car when a coal outburst occurred. The flying debris from the outburst struck the employee in the face and head which resulted in a fractured skull with fractured facial bones.

Retreat mining or pillaring was being done on the MMU No. 2 section when a roof fall occurred trapping the Fletcher mobile roof support unit. There were no injuries associated with the roof fall.

EE was using slate bar to lift a scoop tire to get a chain under the tire. This resulted in pain to his left shoulder. EE continued to work until his surgery on November 26,2012.

The employee stated that he was pulling the continuous miner cable when he stepped into a hole causing him to fall and twist his right knee

Hurt back while rising up from a kneeling position.

While loading roof bolt plates onto drill in walkway, EE twisted to pick up plates and felt a pop in his back.

Employee was building brattice when he was wedging top. He drew back hammer and felt pain in his shoulder.

Employee was repairing a cable and when he picked up the cable to tape it up the cable blew up in his hand causing some burns to the left hand but there was no electrical shock.

Employee was walking around scoop, stepped on something and twisted his right knee.

While operating the continuous miner, a piece of rock flew back striking the employee on right side of neck.

On 4/9/12, EE was gathering materials to work on belt drive. He stopped by the tool hole to get tools. When he lifted the tool box to place on ride, he felt a sharp pain in his lower back. EE continued to work but pain got worse.

On 4/9/12, EE was hanging curtain when rock fell. The rock struck EE on arm causing a laceration. Laceration required four stitches to close wound.

EE was using a hand held Hilti Drill. The drill hung up and it swung around hitting him in the face.

An unintentional roof fall was found at Bk 65 in the #6 entry. Roof fall is 1 Bk inby survey spad #4909. The fall is approximately 20'L x 18'W x 5'High. No interruption in ventilation occurred. No injuries were reported. No equipment was involves. Area was dangered off. Fall was found at 7:30am. MSHA hot line was notified. MSHA inspector issued a K order at 7:45am.

12/9/11 EE was helping to hang bolting machine cable. The cable reel hung up and jerked cable down catching EE from behind. The EE cont'd. to work and did not miss any work until 3/22/12. He had O.P. surgery on his knee on 3/22/12. This was his first day away from work due to this accident.

Employee had started to drill in the top when a piece of dry rock fell striking his left index finger, resulting in a laceration. Employee was wearing metacarpal gloves. The small piece of rock sliced between seams. The cut required 4 stitches to close.

Employee was trying to move large pieces of draw rock off the continuous miner with a slate bar, while attempting to walk one piece he lost his grip on the bar, which then swung up striking him under the shin, resulting in a laceration which requited 3 stitches.

The miner was laying out roof bolts to bolt the last row of the 003 4-right crosscut. There were straps located in his tray, and as he brought his right arm back, he struck his right elbow on the straps. This resulted in a laceration to the miners right elbow requiring 12 stitches.

Employee was loading supplies to take to the roof bolter. A metal band had already been cut on a bundle of roof mats (straps) and as he turned to pick up more mats the sharp edge of the cut band struck his right forearm resulting in a laceration which required 10 stitches to close.

On 8/24/11 the individual filed a claim for Coal Workers Pneumoconiosis with the Kentucky Labor Cabinet.

Employee was bolting in 5 right crosscut. As he was putting his steel in the tray the rib popped and a piece of it went into his right eye. He was wearing safety glasses. he went to the doctor and had it checked. He was released to come back to work with no restrictions.

EE was drilling a hole for a roof bolt when the drill steel stopped up. There was pressure on the drill head which caused the drill steel to bow outwards. When the EE released the pressure the drill steel kicked back striking his left hand. This resulted in a hairline fracture.

Employee was operating a DBT scoop helping to push an MRS outside. His scoop bucket climbed the back of the MRS causing his scoop to go up. He hit his head on the top causing a laceration to his ear. This did result in 8 stitches

Employee was rolling up a curtain in the #5 heading in preparation to scoop it. A small section of the rib rolled off and struck his right arm resulting in a laceration. He was on the 002 section at the time of the accident.

Roof Bolter operator was going to spot a bolt on the shear of the 3-Rt crosscut. As the employee was swinging out the head of the roof bolter, the rib rolled off striking the employee on his back and the back of his right arm resulting in a cut to the back of his arm which required 6(six) stitches to close the wound.

Employee was installing a rib bracket with roof bolter when the coal rib popped sending particles into his right eye. Employee was wearing safety glasses. Employee continued working the rest of his shift. He stopped by and had his eye washed out and checked at Whitesburg A.R.C. Hospital on his way home.

EE was installing roof bolts in #6 herading, as he was pushing bolt up into hole a piece of drawrock fell striking him on the right arm. This required 4 stitches.

Contractor employee was removing the metal bands from a bundle of roof straps. The contract employee was pulling on the metal band when it came loose hitting him on the left forearm. The metal band sliced his left forearm requiring 3 stitches to close the cut.

Employee was standing at the side of the coal feeder when a 4 inch water line coupling got loaded into the feeder. As the water coupling came into contact with pick breaker in the feeder, the coupling was thrown striking the employee on the upper right jaw and cheek bones resulting in fractures to the cheek and jaw.

The EE got down on his knees while operating the CM. When the EE started to stand up, he stated that he had pain on the backside of his knee. The EE stated that he did not know what happened that his knee just started hurting. The EE cont. working until 11/09/10. Note: The employee did not start workers compensation until 11/09/2010.

The EE was assisting in belting back when he accidently stepped on a wooden crib block twisting his right knee.

Employee had just pulled bolter into #4 heading, as he swung boom out to put up first bolt, a section of the rib rolled out striking his right side with a glancing blow.

Employee was preparing to bolt # 5 Heading. He was on his knees cleaning out his dust box when a large section of the rib broke off and fell striking the employee on the back of his legs and feet.

EE injured back on 5/20/10 while setting timbers. EE refused to fill out accident form at this time. He continued working his normal shifts, on 5/27/10 he went to the doctor on his own.

The employee was operating a shuttle car. He dumped his load of coal at the feeder. He started to turn around in the operators compartment of the shuttle car, when the car started to travel from the feeder towards the face. The employee got his right foot that was hanging out on the operators deck caught between the rib and machine resulting in fracture to right ankle.

The employee was helping the section repairman pick up tools and put into bucket. The employee lifted the bucket of tools to put them on a ride when he felt a sharp pain in his lower back.

Contractor working at the mine, rock dusting the off side of the belt conveyor with a rock dust machine and hose. The contractor was walking backwards when he tripped over a piece of belt structure falling and hurting thumb. Contractor went to get medical treatment on 2/25/10.

EE was using a spray machine to coat the ribs, roof & brattice with a non-combustible material or coating. The tip of the spray machine stopped up while spraying. The 2nd EE was trying to unclog the tip of the machine. When the tip uncloged, the pressure forced the material out knocking the ee's safety glasses off permitting the material to splatter into his left eye.

Employee was using a spray machine to spray brattice/ribs for a battery charging station. While spraying the nozzle stopped up. The employee was trying to clear the material away from the tip, when the object in the tip was cleared the pressure forced the material out knocking his safety glasses off and spattering in his eyes.

Employee went to get his tool bag out of his tool box. When he picked it up, he pulled something in his right shoulder.

The employee was operating a scoop hauling supplies underground. A diesel mantrip came upon the scoop and was traveling in the same direction. The scoop operator attempted to pull out of the travelway into a crosscut to permit the mantrip to pass. The scoop operator ran over a wooden crib block and bounced the employee into the roof.

EE was operating bolt machine while putting the last roll in the #4 heading when a piece of rock fell from the t-bar mashing his right hand on the drill pot. The rock measured 3' long x 10" wide and 8" thick.

Employee was cutting a piece of metal. A small fragment hit him it the right eye under his safety glasses.

A roof fall was found in the old #2 entry near the (A) set of seals. The roof fall was in the intersection at the 4th crosscut from the old portals. Fall is approximately 6' wide x 25' long x 4 1/2' thick. Fall was found during a weekly exam. Roof fall shown on map as fall #1. Roof fall will not be cleaned up at this time.

A roof fall was found in the old #3 entry neat the (A) set of seals. The roof fall was in the intersection starting at the 4th crosscut and proceeding toward the 5th crosscut from the old portals. The roof fall is approximately 60' long x 12' wide x 4 1/2' thick. Fall was found during the weekly exam. Fall shown on map as fall #2. Fall will not be cleaned up at this time.

A roof fall was found in the #3 entry near the (A) set of seals. The roof fall is in the intersection at the #2 crosscut inside from the old #2 portal. The roof fall is approximately 0' to 6' long x 6' wide x 4 1/2' thick. Fall was found during the weekly exam. Fall shown on map as fall #3. Fall will not be cleaned up at this time.

EE was hanging miner cable with section boss when he stepped in a low spot in the bottom causing him to twist his left knee.

Employee was working on continuous miner on mmu n0/002 section. He and helper had to remove large lid on miner to gain access to trouble shoot and repair. Large lid fell while re-installing pinning the employee s hand between the lid and machine. This resulting in the 1st joint of the 3nd finger on the employees right hand being cut off.

The employee was moving a shuttle car out of his way. He placed his right hand on the canopy as the car moved. The canopy caught his fingers against the roof mashing them.

EE was making a splice in #8 belt. He was cutting belt against a belt square, when he accidentally cut his right index finger.

Employee was opening a set of airlock ventilation doors on roadway and strained his back.

The section foreman on 003 section was making an exam of the n 0/5 face area when saw a crack in the face. He checked face area and detected a small amount of cha or methane and low oxygen. He examined the n0/6 face area. found small crack in the face. detected low oxygen and low contrentration of cha. withdraw employee. notified msha.

The injured employee was lifting one end of a metal H beam. When the employee lifted the end of the beam, he felt a sharp pain in his groin area. The employee continued to work his regular shifts without missing any work until 3/17/09. Surgery scheduled to correct condition.

Employee was crossing a run-through tailpiece. As he stepped down he stepped on a turnbuckle which caused him to fall. His boots were muddy.

Employee was moving bags of rock dust into scoop bucket. While twisting with rock dust, he felt a sudden sharp pain in lower back.

EE was at miner talking to miner operator when a rock, 6" X 12" X 60", rolled out of rib. Rock hit EE on ankle.

on 1/28/09, a roof fall was found by weekly examiner. A roof fall is on old 4-wouth mains at matt of 2 left panel in intersection. This fall is in old works and is not blocking airflow or travelways. No one travels this area. Area was dangered off. Fall was approx. 5' high x 30' long x 19' wide. MSHA inspector

Employee was taking lid off of the reel on a shuttle car when the lid slipped and hit his index finger on right hand.

Injured was trying to pry fender back on S/Car, when fender fell. He grabbed the fender to keep it off of him, as it fell, resulting in twisting his lower back and right leg.

Employee was drilling hole in roof for a roof bolt and went to change out a piece of steel, when it fell on his right hand.

While backing up in a Mac 20 EE hit his neck on a water line, causing pain in his neck and left arm.

EE was pulling on the airlock door and felt a pain in his left shoulder. Came outside and went home/doctor.

While working on a minitrak employee was leaning over the equipment and was pulling on a ratchet when he felt something pop.

EE was driving a pin in a cat pod on #3 MRS. He was laying on his side, when a piece of metal came off and struck him under his right eye. He was wearing safety glasses, but because of his position, the metal went under his glasses.

The rib rolled off pushing employee into other ee knocking both men to the ground.

EE was welding and did not wear a welding hood. He received flash burns to his eyes.

Employee was lifting structure and tripped over a rail injuring his lower back.

Employee picked up a nail board from the mantrip, when the board turned, twisting employee's right hand/wrist. On work restrictions for 1 week.

Employee was riding mantrip looking for a water leak on belt, when he hit his head on the mine roof.

Employee was picking up miner cable when he felt a muscle pull in his left upper arm.

While roof bolting employee caught his right hand between drill steel and drill head.

Employee was working on shuttle car cable when he went he went to power center to hook cathead back up & try breaker. When he put breaker in, cathead & receptacle started firing up. He received the burns while the arcing was taking place. He stated he pulled the cathead from the box while this was going on.

After installing the last roll of roof bolts, the EE was filling his glue box up and a piece of rib rolled off and hit his lower legs.

EE was working on a water pump when his foot got stuck in mud. He reached up & grabbed a waterline hanging from the top, the waterline came loose from the top & fell striking employee on the back of the neck.

While roof bolting, a piece of rock rolled off rib measuring 6' X 20" X 6" thick, striking employee on right knee.

While roof bolting, employee took drill steel out of hole and the steel flipped hitting him on the chin.

Rock fell between roof bolt and rib striking employee on shoulder and lower back.

Employee was using a sledge hammer to bust rock off miner when he smashed his little finger on his left hand-received 10 stitches.

Employee was traveling to the outside when chips of concrete fell into his right eye. EE was wearing glasses.

Employee was removing a rock from roadway when he felt pain in his lower back.

Employee was injured by a piece of falling draw rock (bruised back)

Employee was beating a bit insert back in the miner when the handle came out of the hammer causing him to hit his left hand on the bit lug.

Employee got his finger caught by the auger of the rock duster which resulted in a laceration to the right hand tip of middle finger.

Injured and co-worker were bolting 3 rt open x-cut. Injured went to the back of the roof bolter to hang his cable. When he picked up the roof bolter cable he received an electrical shock.

Injured & co-worker were bolting #3 entry on 001 section. They had put up the 1st row and were starting on 2nd row. Injured was drilling his 1st hole when a rib rolled off without warning knocking him to the ground, pinning him against the pinner. The rock was approximately 6' x 2' x 7 1/2'.

While tramming scoop, employee got too close to rib & bumped rib with canopy of scoop. A few small rocks fell on his neck & shoulder. He stated that he thought larger pieces were about to fall. He jerked forward to avoid anything else striking him. He stated that he felt pain in his neck & shoulder after that.

Employee was moving crib blocks out of the roadway when he strained his back.

Employee was watching the miner cut coal when metal chip off of the miner bit flew back hitting him in the left eye.

Employee was taking out belt structure when he raised up catching neck and left shoulder on a belt hanger. This caused severe pain in neck and partial numbness in left shoulder and arm.

EE was lifting a piece of belt structure when he felt pain in his lower back.

Employee was roof bolting when the drill steel bent and mashed his right hand.

Employee was bent over attaching a welding cart to the mantrip when his back popped and he felt pain down his right leg. *Started missing work due to this injury 8/7/06.*

EE was standing in scoop bucket hanging a curtain when his feet slipped and he fell injuring his right side. had x ray he has 2 cracked ribs.

Employee was walking from the telephone to the feeder when he slipped & fell in an area that was on a slope going downhill.

Employee was stepping down off of #4 tailpiece when his right foot went into a hole in the bottom and his right knee twisted sideways. *Emoloyee did not start missing work until 7/10/06.

Injured was making a belt splice when hammer slipped and struck his left index finger. He recv'd approx. 20 stitches.

EE WAS SPLICING A CABLE WHEN THE KNIFE SLIPPED AND CUT HIS RIGHT LEG. HE REC'D 5 STITCHES.

Employee was working around the beltdrive when he smashed his left thumb between the guard and the beltdrive. Went to Dr and had thumbnail taken off and injury was cleaned and dressed.

Employee picked up timber and turned with the timber to set it and strained his lower back.

EMPLOYEE WAS SITTING ON MINE FLOOR SERVICING A MANTRIP & WHEN HE STARTED TO GET UP HE FELT PAIN IN HIS RIGHT KNEE.

EMPLOYEE HOPPED OFF SCOOP AND TWISTED RIGHT KNEE.

Employee took planetary & planetary drive reduction unit loose and rolled it out on the ground. When doing so, it rolled out striking employee on top of his right foot.

Employee was lapping up setup belt when he heard something pop in lower left side of his back.

Employee was carrying two cans of oil to his shuttle car when he felt pain in his lower back.

Employee was throwing roof bolts up to the front of the roof bolter when he felt a sharp pain in his right shoulder. He began missing work due to this injury on 11/17/05. RTW 11/18/05 on light duty.

Employee was holding down on welding cart to unhook from diesel when cart came unhitched. The front end raised putting his elbow into the top while his hand was still on the tongue.

EE WAS HANGING A MINER CABLE WHEN HE STRAINED HIS BACK & SHOULDER

Employee was bent over throwing a miner cable out of roadway when he strained his lower back.

EMPLOYEE WAS OPERATING CONTINUOUS MINER WHEN A PIECE OF COAL FLEW INTO HIS EYE.

Employee was moving miner out of #2 heading when he slipped on wet bottom and twisted his right knee.
