The employee was shutting off wash down hydrant with a T handle. The threaded flange connecting the casing split in half, shooting all 6 feet of the stem upward into the employee's face splitting EE's lip open requiring sutures.

An employee was installing seals on a 777 duo cone. EE's hand slipped and contacted the sharp edge of another seal, causing a cut on EE's left hand, middle finger. The cut required sutures, and EE was released to full duty.

An employee felt pain in EE's low back as EE climbed the boarding ladder of a front-end loader. EE's back muscles spasmed as EE climbed up, no slip or fall was involved. EE was taken to the clinic for precautionary reasons and was released to restricted duty.

An employee was removing the cable clamps from the drag drum on a dragline to perform a drag rope change out. EE removed the first cable clamp and it fell, striking EE's right foot on the safety toe of EE's boot. EE was evaluated at the clinic and X-Ray revealed a fracture of EE's little toe.

An employee was unloading a box of filters. The box was directly under a light switch. When EE reached into the box, EE's finger contacted the corner of the metal faceplate cover. The corner was sharp and cut EE's finger. EE received stitches, making this incident reportable.

The employee was trying to close the door on the front of the dragline cab. EE's left hand/thumb was in the frame of the door as it closed. EE's thumb was pinched in the closing door. EE received a crushing type injury to EE's hand. EE was taken to the clinic for treatment and was not released to return to work.

Filling transmission fluid on dozer assisting Mechanic. Mechanic started unit to fill filters and check oil level. Filter housing cracked causing oil to spray on oiler on the right side of face and right eye.

The employee was lifting an air pump out of a grease barrel and felt pain in EE's back. EE was taken to the clinic for evaluation and received a full release. EE was taken back to the clinic on 8/24/23 due to continued pain and was not released to full duty.

The employee was leaving work at the end of shift. Employee stopped at the crew shack to wash face and head. A piece of dirt or coal fell into EE's left eye when EE washed EE's hair. The particle caused a scratch on the cornea and the doctor administered prescription eye salve, making this incident reportable.

While installing rebuilt brake for drag rope on Dragline EE pinched left middle finger resulting in fracture to end of finger.

Disassembling crushers Thursday 11/10/22 at end of shift, employee lower back was hurting. 11/14/23 low back pain was still noticeable hurting. Employee released full duty 11/14/23. Had follow up appt. Did not notify Safety Dept of getting more treatments. Safety Dept notified on 7/13/23 by WC Insurance Co.

The injured employee was removing a jack stand from under a D6 dozer. EE pulled the pin to collapse the extension and it did not collapse. When it did collapse, EE's finger was pinched, causing an avulsion to the little finger on EE's left hand. The employee did not return to work for EE's next shift making the incident reportable.

The injured employee was rolling up old conveyor belt. The employee was changing spools and received a laceration to the left hand from a sharp bur on the end of the metal spool. The employee was wearing leather gloves at the time. The wound was sutured, making this incident reportable.

Employee was working on replacing a broken fitting on the shear slough plate. When the fitting going to the cylinder was removed, the sloughing plate dropped to the stops on top of the shear and pinned the employee between the shear body and the sloughing plate. Employee quickly got chains connected to sloughing plate to raise slough plate and remove the employee.

Employee was taking a wooden crib block off a pallet, turned around to walk away when the first row of cribs toppled over and hit EE's lower left leg. Employee later went to urgent care on June 4th and was prescribed antibiotics for an infection, on June 6th EE's personal doctor wrote a note to be off work till June 13th.

A mechanic was clearing tools from an area where EE had just completed work on a Letourneau loader. When the mechanic picked up a pump assembly EE felt something protrude through EE's cut resistant gloves, upon removing EE's glove EE saw the cut on EE's left index finger. The wound was cleaned and the employee received stitches.

Employee was packing a turbo drill and felt pain in their shoulder. Note: Employee DID NOT start missing work because of this incident until April 29, 2022.

Employee was working on a dragline, while crawling through a bulk head looking for a grease leak they felt a pop in their left shoulder. Occurred on 4/3/22, employee was released with no restrictions & missed no work until 4/18/22 when a second doctor took them off of work.

Employee felt pain in right knee while adjusting an external speaker. Was seen by the medical provider, released to full duty. Had 2 more follow-up visits then referred to an orthopedic doctor where a tear in the meniscus was discovered. Employee had surgery May 16th.

On 12/19/2021 an EE, pushed a crib block onto a crane with closed fist and felt pain in Rt Middle finger. The EE later went to ortho and had out patient surgery 2/11/2022. The EE has missed no scheduled work shifts.

EE lifted a 5 Gal pail of ATF and felt a pop in neck. Employee was seen at medical and was not released for next shift.

Monday - Employee lifted a pin for a hydraulic cylinder from the ground, carried it to the truck, hauled it to the Weld Shop and handled it 2 more times. Tuesday EE started feeling discomfort. Wednesday EE reported the incident and was taken for medical evaluation, where EE received chiropractic care.

Mechanics were loading a swing cylinder for a motor grader onto a pickup truck. One mechanic stumbled while moving it to the truck, causing the load to shift suddenly and the other end of the cylinder dropped, pinching the other mechanic's finger between the end of the cylinder and the truck bed.

Employee was out cleaning the windows on the left side of the cab of CAT D11T Dozer. As they grabbed the door to swing it open, their hand was on the safety latch on the top of the door resulting in smashing their right thumb. Employee was taken to Colstrip Medical Center where x-rays revealed a fracture on the tip of the right thumb. Employee was released with no restrictions.

A safety chain had been installed as an anti-topple device for the walking gob-side walking shield. As the shield was being moved, the chain broke and a piece of it struck the EE in the face, causing a laceration.

Was helping attach Huesker mesh to the shield tips, in preparation for LW recovery. As EE stepped across the panline they felt a pinch in lower back. The following day EE was stiff and sore. EE was examined at Medical and received Chirpractic treatment from their own chiropractor.

Employee injured a shoulder while replacing a component on a LeTourneau loader. A MRI was conducted later on identified a torn rotator cuff. Surgical repair completed on March 10, resulting in this being reportable and lost time began. Info entered in MSHA database on 3/11/20. A review of that system indicates the form did not save properly. Re-entered.

An employee was moving a 55 gal drum off of a pallet, when the employee moved the drum to the edge of the pallet the weight shifted pinching the employee's left index finger between the door frame and the drum. The employee received 6 stitches in their left index finger.

On 9/02/19 the employee felt a pop in EE's shoulder while changing a tire on the small water truck. On 9/03/2019 the employee was seen at the medical provider and placed on restricted duty.

Day shift employee while getting tools ready for work picked up the tool bag and felt a pinch in back with a sharp pain. Reported it to supervisor, an EMT checked employee out, and employee was brought outside for further evaluation. Employee was taken to a medical provider where employee received chiropractic treatment and was released to full duty.

Employee injured shoulder while lifting the cover off of the 8MVA PC in LW-404. The employee was examined and released to duty that day. An MRI was conducted later on and indicated surgery would be necessary. That surgery occurred on September 16, 2019. That medical treatment and subsequent lost time made this injury recordable as of that date.

Teammate is unable to remember what took place. After investigating our opinion is EE was installing tire liquid into the tire improperly when the tire slipped out of the tire hand pinning EE's arm between the tire and the rim.

An employee came to work on April 8th, stating they had hurt their back on April 2 or 3rd. They had not reported anything to anyone until that time. The employee was seen at Reliance Medical on April 10th. They received chiropractic care and was placed on Modified Duty beginning that day and triggering MSHA reportability.
