The employee was stepping out of EE's pickup and twisted EE's left knee.
Mechanic was climbing the stairs on a CAT 777 haul. Three steps up the stairs they felt their right knee pop.
EE was sweeping in truck shop. The broom handle broke and pinched finger resulting in 2 stitches
While doing pre-op grader right rear side, notice the engine compartment door was open, crawled up to close the door, the latch handle was broken, went back to cab to get pliers to close the latch, after closing the latch, crawling back down slipped on the small lower foot step. Reached to grab the hand rail to keep from falling off the grader and somehow cut left hand.
Reported neck was hurting from loader hitting or rock dropped in bed of truck. Went to Urgent Care for treatment on 10/6, did not notify employer until next day. Employer requested 2nd opinion from Reliance Medical Center. Released to full duty 10/7 with follow up appointment on 10/14, follow up referred chiropractic treatment. Received chiropractic treatment on 10/20.
Was at pond 11 checking water pump fuel level. While walking slid off the berm into the mud. Right foot stuck in the mud about 5" deep and I twisted ankle, pain also felt in the lower knee area.
Employee lost control of water truck due to wet conditions and rolled water truck onto side
While dismounting a D11 Dozer, the employee's foot slipped on a track grouser, causing a loss of balance and resulting in the employee striking EE's head on an iron grab handle.

An employee was using the spanner wrench to change a disk on a handheld grinder. When the nut came loose, employee felt pain in right hand. Employee reported it at the beginning of the next shift and was taken to the clinic. EE was not released to full duty.
Ee was unhooking the trail cable from dragline. EE was following to make sure it didn't get snagged on anything. EE's eyes were on the cable while passing behind the shoe, EE walked into the shoelace counterweight. EE was evaluated on-site and sent to the clinic for precautionary reasons. Upon further evaluation the employee was not released to return to work.
An employee was installing seals on a 777 duo cone. EE's hand slipped and contacted the sharp edge of another seal, causing a cut on EE's left hand, middle finger. The cut required sutures, and EE was released to full duty.
An employee felt pain in EE's low back as EE climbed the boarding ladder of a front-end loader. EE's back muscles spasmed as EE climbed up, no slip or fall was involved. EE was taken to the clinic for precautionary reasons and was released to restricted duty.
An employee was seen by an orthopedic specialist and was not released to return to work. EE began missing time on 10/17/24. EE's chief complaint was hand/wrist pain which EE attributes to years of equipment operation.

A welder was grinding in the welding shop. They felt something fall into their eye. Was taken to the clinic, where a piece of metal was removed from their eye. Welder was given prescription eye drops, making this reportable. Welder was wearing safety glasses and a face shield at the time of the incident.
While being loaded in an End Dump with dirt, the operator experienced bouncing in the cab when the excavator dropped a bucket load into the truck. The operator reported experiencing soreness in the neck and shoulders. After being evaluated at a local clinic, the operator was given work restrictions that prevent them from returning to work.
While removing the bolts from the belly pan of a dozer, dirt fell in behind the employee's safety glasses and got into EE's eye. EE was taken to the clinic and the debris was successfully removed. The employee was given prescription eye drops, making this incident reportable.
Contracted to install belting onto the overland conveyor. The belt tech was cutting the splicing material on the flatbed of the work truck when the blade slipped from the straight edge and skid across the employee's middle finger, pointer finger, and thumb on EE's left hand. Employee received a total of 17 stitches. Cut through skin only, no tendons. No PT required.
An employee was unloading a box of filters. The box was directly under a light switch. When EE reached into the box, EE's finger contacted the corner of the metal faceplate cover. The corner was sharp and cut EE's finger. EE received stitches, making this incident reportable.
During the start of shift crew stretching an operator felt a slight pulled back muscle. While heading out to the field the operator felt pain and got worse in route to the work area. After doing their equipment pre-op inspection the operator called the supervisor to inform them of the pain.
An employee complained of right wrist pain while operating a haul truck. The employee states that this is an occupational illness as there was no injury event. The employee was sent home and was later diagnosed with carpal tunnel syndrome and has not returned to work.
An employee was dismounting a dragline. EE mis-stepped as descended one of the last stairs and fell to the ground. EE was later diagnosed with a torn tendon, which will require surgery to repair.

EE received a crushing type injury to EE's hand. EE was taken to the clinic for treatment and was not released to return to work.

EE was taken to the clinic and checked out. EE continued to work and later went to get rechecked. EE was not released to duty and the employer was notified of this on 1/10/24.
Filling transmission fluid on dozer assisting Mechanic. Mechanic started unit to fill filters and check oil level. Filter housing cracked causing oil to spray on oiler on the right side of face and right eye.

The employee was lifting an air pump out of a grease barrel and felt pain in EE's back. EE was taken to the clinic for evaluation and received a full release. EE was taken back to the clinic on 8/24/23 due to continued pain and was not released to full duty.
The injured employee was stepping from the lower deck of the excavator to the upper deck. EE missed the step and hit shin on the step. EE received a cut that required stitches to close, making this incident reportable.

EE was getting loaded in a Cat 777 The bucket load of mud caused the truck to shift and at end of shift EE reported a sore back. EE went to medical and was not released to next scheduled shift
The employee exited a vehicle and began walking across a parking area. EE slipped on ice and fell on EE's right leg/ankle. EE was taken to the clinic for X-Rays and was diagnosed with a fracture. The employee was not able to return to work.
while operating Cat D11 dozer cleaning highwall for dragline rocks fell from on dozer. Operator stated EE was ok at the time. EE worked 4 shifts and then went to Dr reporting sore back. Operator did not report to work on 2-16-23 making this a reportable incident
Employee tripped in mud, landing on elbow. EE requested medical evaluation. EE was not released to work pending further tests. (MRI)
While installing rebuilt brake for drag rope on Dragline EE pinched left middle finger resulting in fracture to end of finger.
Employee dismounting boarding ladder on last step knee gave out and fell. Employee did not want medical evaluation. On 11/15/2022 employee, requested medical evaluation. Seen at medical facility, employee was released to full duty. On 1/09/2023 went in to medical medical facility to have knee checked. Employee was given a Do Not Return To work notice and had surgery scheduled.
Disassembling crushers Thursday 11/10/22 at end of shift, employee lower back was hurting. 11/14/23 low back pain was still noticeable hurting. Employee released full duty 11/14/23. Had follow up appt. Did not notify Safety Dept of getting more treatments. Safety Dept notified on 7/13/23 by WC Insurance Co.
The injured employee was removing a jack stand from under a D6 dozer. EE pulled the pin to collapse the extension and it did not collapse. When it did collapse, EE's finger was pinched, causing an avulsion to the little finger on EE's left hand. The employee did not return to work for EE's next shift making the incident reportable.
An employee reported to work. Before the start of shift, they reported to their supervisor that they had been bitten by an insect. It is not known when or where the bite occurred or what type of insect it was. The employee experienced a severe reaction to the bite.
