The ee was attempting to remove a rock from the chute of the #1 belt dump over point that had gobbed off. As the ee moved the rock - a piece of metal band that could not be seen by the ee cut through his gloved hand resulting in a laceration to his ring finger on his right hand. The ee received 5 sutures to close the laceration.

The contract employee entered the mine at 9:00 a.m. He stated to two co-workers that he had injured his back over the weekend. At 1:05 p.m. he called to the mine surface and alleged to the mine superintendent that he had injured his back at the #5 belt drive. Abby Contractors is controverting the workers compensation claim.

The ee was injured when he tried to put the rub rail back on the miner. The ee stated that he felt his knee "pop".

The ee alleges an injury to his back while dismounting a personnel carrier. On August 18th the ee denied any event that caused injury. The ee left the mine and mine property on his own without medical assistance.

Two employees were installing leaky feeder cable. After the cable was installed the two employees were attempting to remove the cable spool from the cart when the reel moved and caught the ee's middle finger on her right hand between the reel and the cart frame resulting in a laceration to the tip of her finger.

The employee was helping the continuous miner operator move the machine. After pulling the miner cable pull rope and hooking the rope to the miner, he began having lower back pains. The employee finished his shift, but went to the doctor the following day due to the pain becoming worse. The ee was placed on restricted work duty.

The steel cable for the B-2 belt drive take-up unit had traveled outside of the wheel guides. The injured employee was attempting to return the cable into the guides when the trapped cable became free and his left hand was caught between the steel cable and the wheel. This resulted in a laceration to the employee's index finger on his left hand.

Employee was closing a swinging air lock door when the ventilation current pulled the door from his control resulting in the employee twisting his knee.

The section crew was in process of pulling back the section power center and the employee was helping configure the high voltage cable as the cable was being pulled by a scoop. The employee's left leg was caught under the cable, resulting in the employee's left ankle being injured.

During the process of coupling a supply car to a scoop, the employee's ring finger on his right hand was caught between the coupling pin and the bumper of the scoop, resulting in a laceration.

Employee was uncoupling hitch from supply car. When the coupling pin was removed the hitch caught the employee's finger between the hitch and the supply car-resulting in a laceration to his index finger on his right hand.

The employee was working as a belt drive person performing routine clean-up around the #6 drive area. The employee moved a metal plate (24" X 36") from the drive area and was in the process of positioning the plate against the coal rib when his little finger on his right hand was caught-between the metal plate and the rib.

Employee had drilled a 10' hole to install a cable bolt. As he was bringing the drill steel out of the hole; the pusher steel and the first extension stuck together. As he was pushing & pulling the steel to pull it apart; the steel came apart and one piece hit him in the mouth chipping two teeth.

After replacing belt in a belt line, the ee was using a chain hoist to pull the belt together to splice. The crew had loosened a top roller and the ee placed his arm at that roller while operating the chain hoist handle. The chain caught the roller and raised it up. The forearm of the ee was caught between the rigid belt structure & the roller.

The employee was loading rock dust onto a rock dusting machine in a scoop bucket and strained or sprained his lower back.

The employee was helping another employe put a compartment cover back on the roof bolting machine. The cover had been removed to wash the compartment out. The employees lifted the cover back on to the roof bolter and were sliding the cover in place when the employee caught his finger between the cover and the frame of the machine.

The employee was working on the triple deck to replace a bad screen in the discharger end of the bottom deck. The EE was cutting out the huck bolts in the back plate to remove the screen. The EE stated he cut all the bolts out except two. These 2 bolts were left in until a means of securing the plate was in place. 1 bolt did not have a nut & the plate fell on his foot.

Employee was assisting with the loading of a 1000 ft. piece of conveyor belt. The roll was on a stand/ the steel band holding the belt had been cut. Employee was pulling the steel band from the belt & the end of the steel band came out from under the belt & sprang back striking the employee on the neck causing a 2 inch laceration which required 7 stitches to close.

While performing a belt conveyor move the employee was attempting to route a flexible water line under the conveyor when his rescuer became hung on the belt structure of the idled belt conveyor. While attempting to free the rescuer the employee felt a pain in his back.

The EE was bolting the No. 2 left crosscut and had drilled the last roof bolt hole in the place. The coal height was too low in the area to put the 42" resin roof bolt in the hole. As he was bending the roof bolt slightly to get the bolt in the hole, he felt a pain in the right side of his lower back, near his rib cage.

The employee was getting onto a battery personnel carrier. He had put his right leg onto the carrier and when he was pushing his body onto the carrier with his left leg he felt something pop in his knee and it locked up.

EE WAS LAYING PVC PIPE (WATER LINE) OUTSIDE ON THE MINE SURFACE AND HURT HIS BACK. (EE DID NOT REPORT INJURY IMMEDIATELY - CONTINUED TO WORK UNTIL 5/25/05.)

Shoveling #5 belt and allegedly strained his back.

THE SECTION IS PILLARING AND THE EE HAD CUT SOME TIMBERS FOR A BREAKER LINE. AS HE WAS THROWING THE TIMBERS OUT OF THE SCOOP BUCKET ONTO THE RIB IN THE NO. 1 HEADING, HE PULLED A MUSCLE IN HIS LOWER BACK. THE EE STATED HE TWISTED HIS BODY WHEN HE THREW THE TIMBER. THE TIMBER MEASURED APPROX. 56" LONG X 4" SQUARE.

HE HURT HIS LOWER BACK PULLING BELT STAND OUT OF MUD.

EE WAS LOADING BELT STRUCTURE INTO SCOOP DIPPER. EE THREW PIECE OF STRUCTURE INTO SCOOP BUCKET AND TWISTED HIS BACK, PULLING A MUSCLE.

AS THE CONTINUOUS MINER ON THE SECTION WAS CHANGING PLACES, THE EE WAS HELPING MOVE THE MINER CABLE OUT OF THE ROADWAY. AS EE WAS PULLING CABLE, HE STRAIGHTENED UP & FELT A POP IN HIS BACK. EE CONTINUED TO WORK @ REGULAR JOB TILL 4/1/02 WHEN HE WENT TO A SPECIALIST DUE TO PERSISTENT & INCREASING PAIN. HAD BACK SURGERY 4/2/02. MINE WAS IDLE 4/1/02 & EE BEGAN MISSING WORK 4/2/02.

AT THE START OF THE SHIFT, THE PREP PLANT WAS STARTED & AN ATTEMPT WAS MADE TO START THE REFUSE BELTS. THE NO.1 REFUSE BELT SHOWED A HIGH AMPERAGE READING WHEN ATTEMPTING TO START IT, SO THE F OREMAN, PLANT ELECTRICIAN & A MECHANIC WENT TO THE NO.1 TAILPIECE & FOUND THAT SOME BOLTS HAD BROKEN OUT OF THE FEEDER FRO THE BELT AND ALLOWED REFUSE FINES TO ACCUMULATE AROUND THE BELT TAIL

THE EMPLOYEE GOT OUT OF THE MINER DECK TO HANG THE LINE CURTAIN ACROSS THE ENTRY PRIOR TO TAKINGA CUT OUT OF THE NO. 3 HEADING ON THE 002-0 SECTION WHEN HE FELT A PAIN IN HIS LOWER BACK. HE CO NTINUED TO WORK THROUGH HIS REGULAR SHIFTS THRU 11/13/01 AND DUE TO INCREASING PAIN WENT TO THE DOCTOR ON THAT DATE. HE WAS PLACED ON RESTRICTEDWORK DUTY BEGINNING 11/14/01.

THE INJURED AND ANOTHER EE WERE CHANGING A TRAM MOTOR ON A SHUTTLE CAR. THE NEW TRAM MOTOR WAS BROUGHT TO THE SHUTTLE CAR IN THE BUCKET OF A SCOOP AND THE EE WAS PUSHING ON THE TRAM MOTOR TO T URN IT AROUND WHEN HE FELT A SHARP PAIN IN HIS BACK. THE EE CONTINUED TO WORK HIS REGULAR JOB THRU 8-24-01 AND WENT TO A DR AFTER HIS REGULAR SHIFT DUE TO TO WORK HIS REGULAR JOB THRU 8/24/01

AS SCOOP WAS PASSING UNDER CABLE EMPLOYEE RAISE UP TO HANG CABLE AND WAS CAUGHT BY CABLE BENDING HIM BACKWARD AND DISLOCATING KNEE JOINT.

THE SHAFT ON THE STATIONARY ROLLER FOR THE NO. 5 BELT DRIVE BROKE AND THE INJURED ADN THE OTHER EMPLOYEES THE END OF THE ROLLER AS IT WAS BEING PUSHED INTO PLACE. THE INJURED STATED HE HAD LI FTED THE END OF THE ROLLER AND SET IT DOWN ON THE BOTTOM AND WAS REPOSITIONING HIMSELF TO AGAIN LLIFT ON THE ROLLER. AS HE REPOSITIONED HIMSELF HE PLACED HIS HAND DOWN ON THE FAME OF THE TAKE

AT THE END OF THE REGULAR SHIFT, EMPLOYEE TOOK THE BATTERY POWERED PERSONNEL CARRIER OFF CHARGE TO BRING TO THE SURFACE. AS HE TURNED TO GET INTO THE OPERATOR'S COMPARTMENT, HE FELT HIS RIGHT KNEE POP AND BEGAN TO STING.

THE EE WAS IN THE PROCESS OF REPAIRING A VIBRATOR ON THE NO.1 CLEAN COAL BIN. AS HE WAS POSITIONING A PORTABLE WORK PLATFORM INTO PLACE TO BEGIN WORK, A SLEDGE HAMMER THAT HAD BEEN LEFT ON THE WORK PLATFORM FELL AND STRUCK HIM ON THE RIGHT SIDE OF HIS FOREHEAD. THE EE RETURNED TO WORK ON HIS NEXT SCHEDULED WORK SHIFT.

CRIB BLOCKS WERE BEING STACKED -SKIN-TO-SKIN IN AN EFFORT TO PREVENT COAL SPILLAGE ON THE OFFSIDE OF THE FEEDER. ONE EE WAS ON THE OFFSIDE WEDGING THE CRIB BLOCKS WHEN THE TOP BLOCK WAS DISLOD GED. THE CRIB BLOCK FELL 30 TO 36" CATCHING THE EE'S HAD AGAINS THE TOP OF THE TAILPIECE. EE WENT TO THE DR WHERE HE RECEIVED SUTURES FOR LACERATIONS TO THE 3&4TH FINGERS OF THE RT HAND FRACT

SHEET METAL WAS STACKED AGAINST OVERCAST IN AN UPRIGHT POSITION EE TRIED TO REMOVE ON SHEET AND ALL 5 PIECES ON OVER TRAPPING EE'S LEG AND BREAKING THE ANKEL.

WHILE HOOKING UP RAIL TO BE PULLED HIS LEFT HAND BETWEEN TWO PIECES OF RAIL. SUTURES WERE PLACEDIN WOUND IN LEFT HAND.

HIS TOOL BOX LID FELL ON HIS HAND. HE DID NOT GO TO DR. UNTIL MARCH 9, 1998. X-RAYS WERE TAKEN ON 3-9-98, A BROKEN BONE WAS FOUND ON HIS LEFT HAND. SIMPLE FRACTURE-NOT REPORTABLE UNTIL 3-9-98 .

HE WAS TAKING DOWN MONORAIL WHEN IT FELL AND STRUCK HIS LEFT HAND. SUTURES WERE PLACED IN WOUNDS ON HIS FINGERS. NO LOST DAYS.

EE WAS PICKING A BAG UP THAT HAD COAL IN IT. HE STARTED HAVING BACK PAIN.

THE EMPLOYEE WAS REMOVING A 3/8" HIGH PRESSURE HOSE UNDER THE PANLINE. THE STAPLE ON THE PRESSURE SIDE WAS PULLED CAUSING THE LOSE TO STRIKE HIM. MEDICAL TREATMENT WAS PROVIDED.

EE WAS PULLING SOME BELT THROUGH THE STAGELOADERWHEN HE NOTICED BACK PAIN.

EE WAS MOVING A CABLE OVER FOR CLEARANCE WHEN THE BACK PAIN WORSENED. THIS IS NOT THE FIRST PROBLEM EE HAS HAD WITH HIS BACK. TWO MONTHS AGO HE WAS WALKING TOWARD THE SECTION, WHEN HE EXPERIEN CED SOME BACK PAIN.

EMP GOT HIS HAND CAUGHT BETWEEN TWO LEG JACKS THAT THEY WERE DOING REPAIR WORK ON. THIS INJURY REQUIRED STITCHES.

EMP WAS HAULING, LOADING AND UNLOADING SUPPLIED WHEN HIS BACK PAIN WORSENED. EMP HAS HAD PROBLEMS WITH BACK AT DIFFERENT TIMES. REPEATED MOTION IS THE CONTRIBUTING FACTOR TO EMPS BACK PAIN.

EMPLOYEE HAS HAD PROBLEMS WITH KNEE PAIN OVER THE PAST COUPLE OF YEARS. THE DATE STATED IN ITEM 6 IS THE DATE EE STARTED MISSING WORK FOR TREATMENT.

EMPLOYEE WAS SWING OUT DRYER (#1 REFUSE), WHEN HE MASHED HHIS TWO MIDDLE FINGERS ON HIS RIGHT HAND. THIS INJURY REQUIRED STITCHES.

EMMPLOYEE WAS MOVING RAIL, WHEN HE FELT PAIN IN HIS BACK.

EMP WAS HOOKING A CHAIN TO A SHIELD, WHEN THE CHAIN TIGHTENED IT HIT EMP ON RT KNEE.

EE WAS REMOVING A PIECE OF DRAWROCK OFF THE MINER, WHEN THE ROCK ROLLED CUTTING HIS FINGER. THIS INJURY REQUIRED STITCHES.
