Employee was hanging curtain while a co-worker was striking pin steel against the bolter. Co-workers steel flew out of his hand striking employee in the face.

Employee was looking in the motor compartment when the hood was not secured correctly falling on his hand. Employee immediately went to hospital sustaining a fractured hand.

Employee was reaching for materials in which were behind the curtain. While moving the curtain employee didn't realize there was a damaged pie pan on the machine in which cut his arm. Employee went to hospital sustaining a laceration to his arm that required sutures. Employee returned to work the following workday.

Employee was installing a roof bolt when he heard his elbow crack. Employee went to the doctor sustaining a strain/sprain to his arm and may need surgery and will remain off work for an uncertain period of time.

Carrying a prop-setter with co-worker, co-worker dropped his end and EE said that it pulled a muscle in his back. He did not want treatment but that he wanted to go home for the day. He called after the holiday and said that his back still hurt so we sent him to the doctor.

Injured was servicing a Dozer, when the grease hose he was using developed two pin holes. The pressure on the hose through the holes pentrated the skin on injured right hand, allowing grease to be injected under the skin. Injured was taken to local hospital and had the grease surgically removed.

Injured was packing I-beams for undercast, cutting his arm on the corner of the beam. The injured received stitches for the laceration making this a reportable incident.

Employee was lifting and handling supplies (Cable Bolts) to the bolters when he pulled his back. He went to Gibson General, then went to Dr. in which ordered him to stay off work for the rest of the week.

Employee was installing rub rail on the miner; rub rail fell off onto employees left foot. Employee was taken to Vincennes ER where he was ordered to remain off work for an uncertain period of time.

Injured had just returned to work on 01/09/2012 shift, from being off, was helping build a stopping and said he re-aggravated his back. The cause of injury is unknown still to this date, but injured has missed work after reporting that an injury had occurred making this a lost time accident.

Injured was building a stopping at the 107-XC of the Main South 2, as she was building the stopping she placed a concrete block next to a mandoor and smashed her right Pinky finger. Injured received 4 stitches, making this a reportable incident.

Employee was lifting/handling the lid on a piece of equipment when his hand slipped pinching his finger. Employee immediately came out and went to Good Samaritan Hospital, sustaining a fracture and laceration. Employee reported back to work next scheduled day of work.
