On 8-20-07 Employee Reported to mine supervisor, that on 8-8-07, he was stretching to pick-up a 4x6x1/2 sheet of plate steel with a co-worker, in doing so, he strained his back. on 8-20-07 employee was seen by DR. at the Louisa Medical Clinic, in Louisa Kentucky,and was givin a prescription for pain. Employee did not fail to report to work.

Employee opened inspection door on mixer unit to examine a plugged condition. The inspection door was not secured in place and fell, striking the employee's right hand, causing swelling and pain. Employee was taken for X-rays, no fractures were identified, however; the employee was ordered off work for his next shift and given a Prescription for pain.

While mounting a railcar, employee felt a pop in his left shoulder. Employee was given full duty release from local doctor on 5/14, but went back to the doctor on 5/17. The doctor on 5/17 put the employee off work, for further treatment, until June.

Employee was shoveling belt and bent over to pick up a rock and felt a pain in his abdomen area.

The employee was lifting a CLA for a joy miner, with the help of another employee when he felt a sharp pain in lower back.

EE was loading miner bits on flat car to take into the mine. He twisted around and felt pain in right arm, shoulder & neck.

Was lifting a rub-rail on a joy miner, felt pain in lower back.

Was pulling miner cable out of roadway strain lower back.

Was moving a section of track 30' long when moved it slipped rail fell back striking employee on top of right foot.

Employee had changed batteries and put old batteries on charger. While raising lid on battery, slipped from his hand and struck left hand between lid and frame.

Employee was moving the portable stairway so he could grease the top jacks on a 988 F model loader. While rolling the stairway into position, the employee strained his lower back, resulting in a lost time accident.

EE was on his knee's in front of scoop unloading mega block from scoop bucket. EE twisted around to pick up block from scoop bucket and felt some- thing pop his knee.

Employee was moving miner cable out of the way, strained lower back.

EE was pulling a torgue shaft out of Conv. motor on Joy miner when pulled outer cover hot oil had built up behind the cap & expelled on right hand

While lifting rub rail to put back on Joy miner, he felt pain in left knee. First day out 3/7/06.

Employee picked up wash down hose to clean coal out of crusher. He was using his right hand to twist the nozzle to the on position. When the sudden burst of water pressure came on, the employee did not have a secure hold on the nozzle and the hose. This resulted in the employee's wrist being put in an awkward position, therefore, spraining his right wrist.

Employee was coupling an air hose to adjoin two railcars. When finished, the employee rose from the squatting position and felt a pop in his lower back along with pain. Doctor's report showed that the employee had chronic back pain and spasms. The employee had just returned to work on 10/17/05 from being off 3 1/2 months due to a non-work related back injury.

Employee attempted to re-attach the bucket onto the Skid Steer. At this time employee placed himself between the bucket and the Skid Steer. The bucket was raised approximately 12 inches off the ground. The bucket fell off the boom striking the employee in the back of both heels. This resulted in bruises to his heels and the Dr. prescribing Vicodin for pain.

Employee picked up drive shaft, the shaft slipped catching left ring finger under drive shaft.

Employee was loading glue on roof bolter, felt pain in lower back

Employee was helping with general cleanup duties while his co-workers were eating lunch. The employee was using a high pressure water hose to wash down the pugmill feed belt. The high pressure water hose he was using slipped out of his hand and the hose nozzle struck him in the chin resulting in a laceration which required 6 stitches.

Employee was striking a pick breaker conveyor chain on a feeder and a piece of metal came loose striking him in the right eye lid.

Accident not reported until 8-10-05. Employee was lifting bags of rock dust when he pulled something in his shoulder. Right

Employee was replacing a man-door and a concrete block fell striking him on the left thumb.

Employee was putting scoop on charge and twisted left knee.

Employee was climbing ladder on rock truck and twisted his right knee resulting in a fracture.

Employee was replacing the grating over the Dump 1 tail roller. The grating slipped and hit the ground. The employee tried to replace the grating by himself and strained a muscle in his back.

Employee was lifting lid on miner and felt pain in lower back. Started missing 5/19/2005.

Ldr oper. was helping change out training roller. Employee tried to pick up roller by himself from the dollie without employee assistance. This resulted in an abdominal strain. Employee felt that he was alright, but he went to the doctor on 5/10, 1 of his 4 days off per his schedule. The Foreman didn't find out that the doctor wanted the employee off for 1 week, until 5/14.

Employee was putting up a roof bolt and cut his right wrist with a roof bolt plate. Required 4 stitches.

Employee was pulling miner cable and felt pain in lower back. Started missing work on 3/22/2005.

Employee had just replaced a return roller on the Riverside Crusher Feed Belt. While replacing the guard by the tail roller his left pinkie finger was caught between a hole in the expanded metal guard and another guard resulting in a fracture and laceration of this finger.

Employee was lifting a piece of metal and felt a pain in his lower back. Started missing work 3-8-2005.

Employee was pulling on a piece of belt and felt a sharp pain in his lower back.

Employee was lifting a wearplate. The wearplate slipped catching his middle finger of right hand.

DID NOT MISS WORK UNTIL 2/3/05. RETURN 2/8/05. WAS LIFTING FEEDER FLIGHT SHARP PAIN RIGHT SHOULDER.

Employee was pulling a cathead across scoop batteries and felt pain in lower back.

Employee was pulling rock off of miner and got his ring finger on right hand caught between rock and roof of mine. (Required 7 stitches)

Employee was reaching across roof bolter to get a box of glue and felt pain in lower back.

Employee was pulling cable and felt leg give away under his weight. (Started missing work 10/11/2004).

Employee was lifting roof bolts off back of pinner and twisted his lower back.

Employee was putting lid on miner and the lid slipped catching his right index finger.

Employee was moving belt structure out of roadway and felt sharp pain in lower back. (Started missing work on 8/19/2004.

Adjusting pin on trolley needed to be removed to provide a more secure fit to the structure beam. During the adjustment the trolley arm came loose from one end striking the employee in the left side of the cheek. The employee received nine stiches as a result of this accident. No lost time.

Employee was changing top roller on feed belt and the roller turn over causing a jagged edge to cut forearm. (6 stitches)

Employee was pulling miner cable and felt pain in lower back. Started missing work 6/14/2004.

A piece of metal broke off of a keeper pin on a metal flight striking the employee on the ring finger of the right hand. (required 3 stitches)

Employee was pulling miner cable and felt a sharp pain in his lower back.

Employee was pulling belt and felt pain in groin area.

Helping scoop operator change batteries, batteries rocked forward and caught finger between battery frame and scoop.
