While walking towards the jaw feeder, the employee grabbed the frame to brace self and cut EE's finger on a sharp edge

Cut left palm from changing cutting edge bolts

While freeing up a brake system, piece of rust got into employees eye.

While lowering a light on a electrical box, the light came down and hit their jaw, fracturing a molar tooth.

Employee was adding a fellow employee who had fallen to their feet and injured their head/neck and lower back.

Piece of metal got into employees' eye sometime during shift, unknown how it happened.

Employee pinched EE's finger between two metal stands while moving the stands.

Operator rubbed EE's eye and got a small piece of metal in EE's eye. Next day eye was swollen and doctor had to grind a piece of metal out of the operators eye.

EE was installing a tire on equipment when their middle finger got caught between the snap wire and the rim flange causing an injury and cut on the right middle finger.

Employee was lifting a heavy rock, dropped the rock and it rolled into EE's right ankle.

Operator had EE's hand pinched between a head pully and a bracket, resulting in a cut that required stitches to close the wound.

Employee had a small piece of metal blow into their eye and become embedded.

Carrying ANFO to drill holes from pallet & stemming holes. Bags of ANFO weighed 50 lbs each. Tenderness felt @ 11:00 AM (described as muscle ache). Incident did not get reported until 10/12/22 (when medical evaluation was sought).

Stood up while working under wash plant and suffered a neck contusion when head struck wash plant.

Employee was assisting to remove cutting edge bolts from a loader bucket that was properly blocked & locked out. Employee failed to use PPE. Since injury we have retrained employee as well as gave additional task training. Employee got EE's finger cut which required five stitches. No long term impairment.

Metal guard fell and struck operator in the head, causing a laceration needing staples to close.

Employee was trying to move a tail pulley section that was on the ground when EE felt an abnormal feeling in EE's right bicep.

While removing support stand, finger was pinched by falling shaft top, resulting in a cut needing stiches to close.

While puling on ladder that was frozen in the ground, operator pulled hard, ladder came free striking operator in the knee causing a contusion to the knee.

Lifting drill steel and felt sharp pain in lower back/hip area

While removing discharge chute from conveyor, a piece of angle iron struck operator in the mouth resulting in a laceration that required 10 stitches to close and a temporary cap on a tooth that was cracked.

Operator injured lower back while pulling core from drill casing.

Operator was loading wooden blocks into loader bucket and hurt their lower back

Operator cut their finger when they lost grip while placing a large bolt in the jaw crusher. Bolt landed on the right index finger causing a small laceration to finger tip requiring 3 stiches to close.

Throwing wooden blocks into loader bucket, felt sharp pain in lower back

Operator was lifting up railing on cone crusher and strained lower back.

Moving large wooden block, finger was pinched between wooden block and steel frame. Impact to finger cause closed fractures to tip of finger.

Twisting pieces of pipe together resulting in wrist tendonitis

While lifting a flex mat screen, employee felt pain on the right-side of his neck.

Middle finger got caught/smashed while replacing hydraulic cylinder, resulting in the need for stiches.

Pushing air to air line back onto pipe, bracing against battery box of loader with side of body. Pushed hard enough against battery box to crack a rib.

Employee was pulling a rope attached to a conveyor belt by a clamp, clamp came free and hit employee on the right hand.

Lifting/throwing stone, felt tinge in lower back

Ee was helping to lift a wire screen cloth over a railing when he pinched his right ring finger between the screen and railing.

rock fell between bucket cylinders, went to remove rock and it smashed his finger.

EE was handling a large water hose that twisted her left wrist and pulled her left pointer finger backward.

EE apparently got something in her left eye while working in scale house weighing trucks.

EE lifting wood blocking while helping to setup a crushing plant.

EE helping clean large loose rock off primary conveyor belt with fellow EE, rock rolled down conveyor belt along frame and hit EE's left hand.

While moving rock around metal culvert pipe, rock rolled back towards pipe pinching finger between the rock and the pipe.

While climbing ladder, ladder shifted, pinching employees finger between ladder and metal beam.

While lifting dredge pipe employee felt pain in lower abdomen and reported it as report only. He sought treatment on 10-28-10.

Treading on new drill bit, bit would not turn, when EE unscrewed the bit it fell and pinched finger between bit and breakout table.

While opening the door on the tool trailer, the door stuck and EE felt pain in lower back.

Climbing down steps of Volvo haul truck, EE felt pain in his lower back. Possibly pre-existing condition.

While lifting a box employee felt twinge in back.
