EE was in the service area working on a fuel pump. He was getting a tool out of the bed of the service truck and banged his knee on the bumper as he exited the truck. The next day he asked to be taken to urgent care due to knee pain. He was diagnosed with a knee strain. He was given a brace and crutch,restricted duty. Follow up is 9-25-15.

IE was standing to side of a forklift when the operator of the forklift accidentally pushed on the gas instead of brake while IE was assisting in loading a part onto the forklift. Part of the forklift(Not the forks) struck the IE in the side, likely bruising him. Has not returned to work because of other findings by Dr.
IE's tip of left index finger was smashed during a repair on a hammer drill. After taken to the emergency room, at approx 3PM IE's finger was removed back to just below the finger nail. IE returned to work on next scheduled day 3/10/13.
Individual was changing a truck tire and tried kicking it loose to get if off of the hub. The kick resulted in lower back pain right side and right leg pain. The employee went to Doctor a week later and he gave him prescription(s) and he returned to work with restrictions. Is following up. Employee was instructed to use a hammer next time.
Working on K-Tec scraper, loosened off hydraulic hose, pan dropped to ground, caused metal cover to fall on left foot
While replacing blades on scraper, lifted one and felt pain in lower back. Continued working until 11 days later when felt the injury again while in office.
Employee was unloading grease can from back of truck. while lowering to ground lid of grease can cut employees hand. wound was cleaned and wrapped on site. wound was then cleaned, closed and bandaged at ER.
Replacing gizzly bars in primary feeder. Grizzly bars were leaned up on the side of the feeder when it slid dowm and broke the big toe on the right foot of the employee.
Hopper got plugged up and employee was using a 3 inch pipe to clear the hole. While pulling the pipe back up by hand he felt pain in the right side of his lower back.
Employee was assisting with restaking some bags that fell of a pallet during the night, onto a new pallet. He was not using proper body alignment and twisted his upper body causing him to pull his lower back muscle.
