IE was lifting rub rail on Joy 14CM-15 when he felt a severe pain in his lower back. IE was transported by ambulance to Indiana Hospital and was released.

IE was putting structure in scoop bucket when he pinched his left ring finger between structure and scoop bucket.

Employee stated that as he was attempting to remove a rock (2' x 3' x 6-8") from the wishbone on the ripper of the D11T CAT Dozer when the rock shifted pinching his thumb between the machine frame and the rock causing a laceration requiring 5 sutures. He is continues working his regular job.

IE was working with guard on feeder to replace shear pin, guard shifted and pinched IE's right index finger between guard and frame of feeder. IE was wearing metacarpal gloves at time of incident.

IE was hanging power cable for belt drive when he felt a pinching sensation in his lower left abdomen. IE worked full duty up until 06/04/14, date of surgery.

Employee stated that while assisting co-workers to belt up the section, he lifted a piece of belt structure straining his left shoulder. He continued to work his regular job until he had surgery to repair a torn rotator cuff on 12/2/2014.

IE was replacing top structure on #6 belt when he strained his left shoulder. IE continued working his regular job until 08/21/14 when he had surgery to repair left rotator cuff.

Employee injured his right bicep on Monday morning at the start of the shift (5 AM) when he was lifting a 4-inch pump flexible discharge line and attempting to take a tight bend out of the line. He felt a pain in his right elbow. He had an MRI that diagnosed a right distal bicep rupture. EE had surgery to repair the rupture and has been working his regular job.

Moving man made scaffolding to the next work site when one section of scaffolding pivoted and came down on his head. Hard hat was on at the time, when scaffolding came down tipped his hard hat off and struck him in the head.

Injured stated that when pulling a piece of rock from the top of his continuous miner, he felt a pain in his abdomen. He continued to work his regular job and refused to go to a doctor. On 05/13/2014, he was sent to a doctor who diagnosed a left inguinal hernia.

Employee lacing belt through belt tail piece when he felt a pop in his right shoulder. Possible shoulder dislocation (He has history of having dislocated this shoulder at least two other times).

Employee was pulling hitch over to hook to ride, he pulled hard on tongue and smashed left index finger against receiver plate. Employee was wearing Metacarpal gloves.

Employee was throwing bags of rock dust onto scoop when he felt pain in lower left back. Employee released to full duty with attention not to aggravate the injury, with stipulation of no underground work.

Employee was building a wall with 6" block. The block (wet) slipped out of hand and landed on left hand catching small little finger.

Employee was helping move MRS, he was 40' back watching the cable and the MRS operator was 25' away from MRS when he pressurized it against the mine roof. The bolt end broke off and became like a projectile striking employee on the lenses of his left eye glasses breaking the left lenses and striking the left eye.

Unloading airlock doors off of a skid when the doors slid pinching his finger causing a fracture that required surgery to repair. Employee was wearing metacarpal gloves.

Injured and co-worker removing RFM Tail cover/guard in order to re-thread the belt through the tail when a bolt that secures the guard broke allowing the cover to slide down on his right hand and index finger causing crushing injuries and an open wound requiring sutures.

Injured was removing side cover off miner when he pinched his left small finger between the cover and a bit block causing a laceration requiring sutures.

Employee stated that he reached across the RFM with his left arm to pull himself over to the MBC to put oil in the machine when he felt a pain in his left shoulder. The employee continued to work his regular job until 10/01/2012 when he had surgery to repair his rotator cuff.

Employee was moving a large rock (4'square 4" thick) when rock came down on right pinky finger, smashing end of finger and removed fingernail and split end of finger. He was wearing gloves.

Injured claims that, while setting cribs, he strained his lower back while moving a rock lying on the bottom.

While installing a 6-foot resin bolt and spon straightening out the second bend in the bolt, employee states that he felt pain and heard a pop in his right arm. Employee worked his regular job until 12/14/2011 when he went to the doctor for a follow-up visit and was restricted from working. First day missed was 12/14/2011.

Employee was assisting moving CM cable when he felt pain in his lower right abdomen.

Employee was shoveling around belt tailpiece when a cable tender chain broke and struck him on the right side of his body in the rib area.

Employee stated that while shoveling coal along the beltline, he felt a "pop" in his left shoulder.

While removing a front rub rail off the side of the Continuous Miner, the back rub rail fell off striking the employee on the top of his left foot causing a fracture.

Employee was placing timber on the forks of the front-end loader, when a nail in the timber caught his finger, causing a laceration that required sutures to close.

Employee was opening a 4" shut-off valve, when valve came off it struck employee above his right eye causing laceration.

Employee was assisting on a belt move, when a jack set to hold the RFM came loose and allowed a crib to fall from on top the jack and smashed his left thumb between the crib and RFM, causing an open fracture.

Employee was pulling shuttle car cables and got peeble lime on his left wrist and right hand causing a chemical burn.

Injured and co-workers were installing a guard on the #1 CBM when the guard slipped pinching his thumb causing a simple fracture.

Employee & co-workers replacing belt drive motor. The motor was blocked and lifted by come-a-long onto the bed plate. Employee using a rock bar to assist in positioning the motor onto the bed plate. During positioning motor, supporting eye-bolt stripped out of top motor. Motor fell 10" striking employees lower left leg causing a simple fracture.

Employee was helping pull a broken conveyor belt together with a pulley and wire rope. As they were pulling, the pulley came loose and struck a post, dislodging the post which struck the employee.

Employee states that as he was placing a 5 gallon can of oil on his jeep he accidently pinched the tip of his left index finger between the can of oil and the frame of the jeep causing a simple fracture to the tip of the finger. While the accident occurred on 01/13/2010, the employee did not go to the doctor until 01/22/2010, and did not inform mine management until 01/25/2010.

Employee was working with another employee installing water line using a crib to assist. His hand slipped and his left thumb was caught between the crib and the water line.

Employee was working with a coworker unloading blocks for a stopping. As he reached to pick up a block, the coworker threw a block and caused his right thumb to get caught between the two blocks.

Employee and co-worker were carrying a drill head for a Fletcher Twin-boom Roof Bolter. The co-worker slipped losing control of the drill head, which caused the injured employee to pinch his left middle finger between the drill head and the coal rib, causing a fracture and laceration.

Employee was washing off Joy miner when he got foreign object in his left eye.

Employee states that he was moving rocks in preparation to bolt a cut when he felt a pain in his right groin. He worked his regular job until the scheduled miner's vacation (June 29 to July 5). Employee was disabled by Dr. Bucci effective 7/6/09 for a right inguinal hernia.

On 06/05/09 employee was lifting & loading belt structure into scoop bucket when he felt pull in lower left abdomen. Employee continued working full duty until 07/28/09 when physican instructed him to work "light duty". Employee is scheduled for hernia surgery 08/04/09.

Employee was changing batteries on Scoop when jumper cable slipped. He reached to grab cable and caught his left hand between battery and frame.

Employee states that he was pulling miner cable when he felt a pain in his right groin. Initially thought to be a muscle strain was later diagnosed as a hernia requiring surgery on January 5, 2009.

Employee was changing final drive on miner when he felt pain in groin. He has continued working until 02/06/09 (date of surgery).

Employee was pulling on a 2" hard plastic water line. The line was being pulled with a 4 Wheel Johnson and pull rope broke causing 2" inch hose clamp to pull through employee's right hand causing laceration on right thumb.

Employee was lifting a roll of messenger line and felt pain in his right shoulder. Employee has continued to work full duty until 10/10/08, which is date he had surgery.

Employee was watering batteries on the Long Airdox Scoop. The lid was not latched and fell on employees' left forearm.

Employee was lifting and throwing bags of rock dust onto a scoop tractor, when he felt a pain in his lower back.

Employee was lowering battery lid on scoop and felt pain in his lower back. Employee worked light duty March 6 and March 7, he attended Annual Re-training on March 8th. The first day of lost time is March 10th.

Employee stated that he was pulling cable when he felt a pain in his right shoulder. Employee continued to work his regular job until 09/15/2008 when he had surgery on the shoulder to repair a torn rotator cuff.
