IE was bent over the drill head of roof bolter attempting to take suction hose off to get it unplugged. His knee bumped the joy stick causing the drill head assembly to move pinching his left hand (little and ring fingers) in a pinch point on the boom causing injuries to his fingers.

IE was lifting rub rail on Joy 14CM-15 when he felt a severe pain in his lower back. IE was transported by ambulance to Indiana Hospital and was released.

Roof fall found by mine examiner on return run at #2 Entry Return Run at Station 2045. Size of roof fall 70' Long X 18' Wide X 6' High. Ventilation was not disrupted. No equipment involved.

Employee is experiencing pain in his left knee from repetitive kneeling and crawling. Employee has been a miner for 21 years. This is not a result of any one single accident. Employee has worked full duty up to and including 10/28/2014. Last day worked was 10/28/2014.

Roof started to work #1 Entry 11204, then started to work #2, #3 Entry Outby at 4:35 AM. Fall occurred 11173 #3 Entry for 140' on the 7200 high voltage. Area is cribbed and abandoned.

IE was putting structure in scoop bucket when he pinched his left ring finger between structure and scoop bucket.

When doing preshift roof started to work and fall occurred at inby dump point from #6(ss2053)to & including #3(ss2051). Size of fall 245'x19'x10'. Ventilation not disrupted. Roof bolter covered and miner, haulage and section scoop are inby the cave.

Injured states he was carrying a bag of B-Bond and a jug of water, he slipped in the mud and strained his groin area. The injury was diagnosed as a hernia on 10-10-14 that will require surgery to repair. The employee is currently working his regular job. The accident will become reportable as a NFDL (LTA) once he has the surgery.

Employee stated that as he was attempting to remove a rock (2' x 3' x 6-8") from the wishbone on the ripper of the D11T CAT Dozer when the rock shifted pinching his thumb between the machine frame and the rock causing a laceration requiring 5 sutures. He is continues working his regular job.

Injured stated that as he hooked up the bulk dust bag to the IT Loader Fork, he turned around and his right foot fell into a hole in the pallet causing his right knee to twist. He felt a sharp pain. Employee continued working his regular job, but was later diagnosed with a tear of the medial and lateral meniscus. He had surgery on 9/29/14.

IE was cutting plastic from J-Hook when knife came off end of hook cutting the palm of his left hand.

After roof bolting machine was pulled up to install next row of bolts & after ATRS was set, IE swung his head out & placed drill steel into head in preparation to drill outside hole, when a piece of roof rock(18"x12"x1"-2" thick) fell striking canopy then striking his left hand causing laceration.

IE was working with guard on feeder to replace shear pin, guard shifted and pinched IE's right index finger between guard and frame of feeder. IE was wearing metacarpal gloves at time of incident.

Contract employee making pre-shift on section when he began having chest pains. A Mine EMT administered O2 and transported to surface. Although he felt better on the surface, mine management insisted transport to ER by ambulance. He received a cardiac catheterization consisting of Angioplasty and stent implantation for 85% coronary artery blockage. NOT A MINE ACCIDENT OR ILLNESS!

During the reaming of a bolt hole to allow a cable bolt to fit, the drill steel fell from the roof striking IE left middle finger.

IE was bending a roof bolt when he twisted his right knee. IE has worked full duty up until surgery on 07/31/14.

IE stated that while walking to catch up with a co-worker, who was carrying a hammer, he stepped into an irregularity in the bottom causing him to stumble forward hitting into the hammer being carried by his co-worker. As he fell, the hammer struck his lip causing a laceration requiring 3 sutures.

Employee developed soreness in his right knee from repetitive crawling over an extended period of time. Diagnosed by MRI as degenerative changes consistent with cartilage loss as well as degenerative meniscus tearing. Employee continued working his regular job until he had surgical repair of the knee on 10/31/2014.

IE was hanging power cable for belt drive when he felt a pinching sensation in his lower left abdomen. IE worked full duty up until 06/04/14, date of surgery.

Employee stated that while assisting co-workers to belt up the section, he lifted a piece of belt structure straining his left shoulder. He continued to work his regular job until he had surgery to repair a torn rotator cuff on 12/2/2014.

IE was replacing top structure on #6 belt when he strained his left shoulder. IE continued working his regular job until 08/21/14 when he had surgery to repair left rotator cuff.

Roof Fall happened at 6 East #4 Panel (Intersections & Crosscut #3 to #4 Entry). Size of Roof Fall: 100' Long x 19' Wide x 6' High. Ventilation was not disrupted. Belt structure and High Voltage cable involved.

IE was making splice, machine slid off belt rails pinching right hand between rails and belt splice machine. IE was wearing metacarpal gloves.

Employee injured his right bicep on Monday morning at the start of the shift (5 AM) when he was lifting a 4-inch pump flexible discharge line and attempting to take a tight bend out of the line. He felt a pain in his right elbow. He had an MRI that diagnosed a right distal bicep rupture. EE had surgery to repair the rupture and has been working his regular job.

After drilling hole for roof bolt & putting up channel, realized hole needed enlarged to accept bolt. As he started larger drill steel thru channel into initial hole, had his hand on channel to assist getting steel thru hole in channel, when drill steel contacted channel causing his finger to be pinched between channel & roof causing laceration & fracture to right index finger.

Employee is a belt man and has been for 11 years. ee has been experiencing swelling & pain in left knee from repetitive crawling and kneeling. EE has worked full duty up until 03/12/14, when doctor put on modified duty. No modified duty available. Last day worked full duty 03/11/14. Occupational Illness.

Moving man made scaffolding to the next work site when one section of scaffolding pivoted and came down on his head. Hard hat was on at the time, when scaffolding came down tipped his hard hat off and struck him in the head.

While using sledge hammer in an attempt to switch over a stuck flop gate, employee caught his little finger between the hammer handle and the lever for the flog gate resulting in a laceration.

While trimming 36" section belt prior to making a splice, injured worker using a hawk bill knife was cutting through the belt when the knife caught the board. Injured repositioned himself to get leverage. While applying pressure the knife skipped out of the board and across the belt contacting his right knee causing a laceration requiring 5 stitches.

Two employees building cribs in 4-East area of mine, as one employee tightened his crib, the sledge hammer handle hit the crib, bounced out of the employee's hand causing the sledge to fly, striking an employee on the left side of his face causing a 3/4-inch laceration on the left side of his face between his lips and nose requiring sutures to close the wound.

A Roof Bolter Operator Trainee, while being trained, got his fingers caught between the drill steels as he was putting the drill steels together to drill a hole. Employee was wearing the proper metacarpal gloves at the time of the accident. Laceration to his finger required stitches.

IE was uncoupling skid from rear of scoop, when pin was pulled out of top of pin area, scoop rolled forward making pin go at angle catching fingers between pin and rear frame.

Injured stated that when pulling a piece of rock from the top of his continuous miner, he felt a pain in his abdomen. He continued to work his regular job and refused to go to a doctor. On 05/13/2014, he was sent to a doctor who diagnosed a left inguinal hernia.

Employee was walking along the RHS when his foot slipped, stuck in the mud and he felt pain in his right knee. He continued to work his regular job until and MRI revealed a torn miniscus.

Employee was lying on the catwalk reaching out to paint steel angles when he lost his balance and slipped falling through the belt structure landing on uneven ground striking a rock and fracturing his pelvis.

Employee was operating a scoop tractor using a 1/2-inch grade 80 steel chain to tow a disabled MBM when the chain broke he was struck by a link causing a laceration requiring sutures.

WHILE STEADYING A BEARING SO ANOTHER EMPLOYEE COULD USE A HAMMER TO KNOCK IT LOOSE, A SLIVER OF METAL FLEW OFF THE BEARING AND LODGED IN OUR EMPLOYEE'S HAND.

Employee lacing belt through belt tail piece when he felt a pop in his right shoulder. Possible shoulder dislocation (He has history of having dislocated this shoulder at least two other times).

IE was tramming miner, he was walking in a right to left direction. The miner was stopped and IE stepped into some uneven bottom and felt a popping sensation in his lower back.

Employee tightening down take-up on the E-14 Belt Drive. As the take-up was reengaged, the dolly came back and pinched his hand left hand ring finger between the guide roller on the dolly and the track frame causing multiple lacerations requiring sutures.

Crew has just mined 15' out of #5 Entry off 20x10 x-cuts when roof fell in cut, then intersection started working. They backed the miner up and a few minutes later the intersection fell in.

Roof fall occurred in an intersection in the R-3 entry (Intake) of the 12-Butt off 2-Right Continuous Haulage Section approximately 310-feet outby the working face. The area had been supported using Jenmar 6-foot "Suprtwist" roof bolts with 8-inch x 8" plates on 4-foot x 4-foot centers. No roof defects noted.

IE was checking torque on bolter, he had gauge wrench in chuck and put pressure on it, it checked ok, when he went to release pressure, he pulled lever too far and spun wrench into his right wrist, hand and hip area pushing him against the bolter.

IE was installing outside bolt in #4 Entry of 1 Butt, when IE went to straighten the bolt, the bolt spun around hitting his left hand on the end of T-5 Channel resulting in a laceration of his left index finger.

IE was driving toward section belt on Johnson Man-Trip. Riding w/right hand resting on warning bell & driving w/left hand on solid steering wheel. Man-trip hit uneven bottom causing operator to grip bell w/right hand. The bell bowl spun pinching his finger between the bell and the clapper causing a FX. Continuing to work full duty. Surgery is scheduled for 07/09/13.

Roof Bolter Operator - While attempting to remove a damaged strap by loosening the nut on a super twist roof bolt, the strap rotated suddenly around lacerating the left side of his face requiring three sutures.

Please see attached 06/05/2013 letter. At no time was an incident/accident reported by the employee to AMFIRE's Mine Management.

Employee states reeling oil hose at the oil trailer stepped into a hole catching his toe on a rock causing him to fall, spraining his left hand. No medical attention until 6/27, X-ray of the hand revealed a fracture to the metacarpal bones in his hand.

Roof fall was found in a section of the mine that was idle and has no equipment. Fall was found on a pre-shift examination. No ventilation affected and no escape ways.

IE was driving mantrip out at end of shift, did not notice uneven bottom, front wheel dropped in, steering wheel kicked and jammed right hand causing injury to right middle finger.
