Employee stated that EE was checking and repairing the screens in the shaker. EE was in a seated position and proceeded to stand up when EE slipped. EE reached for the belt with EE's right hand to catch self. The belt moved and EE's finger contacted the pulley causing a laceration to EE's right 5th digit. The plant was not in operation at the time of the incident.

Mostrando los 4Employee was struck in the left leg by a screen that was dropped from an elevation. The screen contacted the handrail of the stairway, slid down the handrail, and struck the employee in the left leg as they were attempting to access the stairway. This resulted in an avulsion fracture to their left fibular head.
A SLC truck came onto the scales overweight so driver proceeded to dump off at the dump off area. Customer returned to the scales underweight and the driver said to the scale clerk to print the ticket. Driver exited the truck to retrieve the ticket and told the clerk had just cut thumb off. I've reached out to SLC for more information or details on this accident without success.
EMPLOYEE AND CO WORKER WERE MOVING FLAT PLATES OFF A TRAILER. EMPLOYEE WAS PULLING THE PLATE WALKING BACKWARDS WHEN HE PULLED THE PLATE AND IT DROPPED ON HIS RIGHT FOOT. EMPLOYEE WAS DIAGNOSED WITH A CONTUSION
