Employee was struck in the left leg by a screen that was dropped from an elevation. The screen contacted the handrail of the stairway, slid down the handrail, and struck the employee in the left leg as they were attempting to access the stairway. This resulted in an avulsion fracture to their left fibular head.

Mostrando los 12Employee was stepping from a paddle boat being used as a temporary access to get onto the dredge. The boat shifted in the water as they were stepping onto the dredge. EE pulled the boat back with right leg, felt their knee pop, lost balance, and fell onto their knee. The dredge was inoperable and staged close to the bank. The boat was located between the bank and the dredge.
IE was using a 6ft step ladder to unclog a hose under the sand hopper. When the hose disconnected from the sand hopper, IE fell off the fourth step of the ladder landing on back.
A SLC truck came onto the scales overweight so driver proceeded to dump off at the dump off area. Customer returned to the scales underweight and the driver said to the scale clerk to print the ticket. Driver exited the truck to retrieve the ticket and told the clerk had just cut thumb off. I've reached out to SLC for more information or details on this accident without success.
EE was beginning to make repairs to a failed spider plate inside the log washer. EE placed left foot on the log shaft, left foot slipped, which caused left shin to make contact with the edge of the log shoe.
The IE repositioned self on top of a weir which caused the employee/operator to lose visual of one another. As the 326F bucket was lowered, it brushed EE's outside right leg which caused EE's inside right leg to scrape alongside the flange of the weir. IE's inside right leg received a laceration from below the knee to the bottom of EE's calf.
EMPLOYEE JUMPED OUT OF BED OF TRUCK
EMPLOYEE AND CO WORKER WERE MOVING FLAT PLATES OFF A TRAILER. EMPLOYEE WAS PULLING THE PLATE WALKING BACKWARDS WHEN HE PULLED THE PLATE AND IT DROPPED ON HIS RIGHT FOOT. EMPLOYEE WAS DIAGNOSED WITH A CONTUSION
Employee was wearing all his required PPE, glasses, gloves, hard hat, and steel toed boots. The employee struck a steel pin with a steel hammer and a piece of the pin broke off.
An accident occurred during routine maintenance on C10X10-30 Dredge. The pump shaft assembly was not properly rigged to prevent hazardous motion. The assembly slipped when lifted, striking and knocking the employee to the deck. The assembly landed on the employees leg resulting in two broken bones.
Driver was not employed by Southern Aggregates. He was a customer driver. Driver lost consciousness while standing outside his truck near the scale house.
Employee was in route to adjust feed gate on hopper. Upon climbing a two rung step, the employee's right boot slipped off top rung with his leg sliding down the rung and edge of catwalk. The edge of catwalk came in contact with inside right leg between knee and ankle lacerating the skin. Employee was treated and released from the local clinic with no restrictions.
