Miner was entering the cab of a loader and shut EE's own hand in the door. EE received medical treatment for a DISTAL PHALANX FRACTURE of EE's right middle finger, was given a splint, and placed on modified duty.

Miner pulled himself up by grabbing onto a pipe and felt a pop in their hand.

Opened the side panel on the loader and was punctured by the edge of the panel or the handle, resulting in a 2.5 cm cut on EE's right hand in between the thumb and first finger. EE needed 3 stitches.

While lifting up on a piece of conveyor belt, miner strained their left arm. It was later determined (11/22) that their left bicep was torn, and they would need surgery to repair it.

EE was rigging something with a bridle chain, threw one leg over the load and it bounced back and the hook caught EE on EE's cheek. 5 stitches to close cut.

While removing the shaft from a head pulley, the shaft bumped miner's arm and broke the bone.

Roller shaft slipped during maintenance activity and smashed miner's thumb

While removing a troughing roller on the belt line employee pinched a finger.

Strained back while changing screens on crusher.

While descending caged ladder, SRL lanyard hook on fall protection harness became caught on ladder cage. As lanyard extended from SRL it knocked off miner's hardhat then lanyard hook dislodged falling and lacerating miner's forehead. Miner was at ground level when struck.

Employee was exerting pressure to place a block and strained their right shoulder. EE was put on modified duty and ordered to ice and rest it.

Miner was preparing to work on a crusher. Upon inspecting the crusher, EE hit head on the overhead conveyor (de-energized), which knocked off EE's hard hat. EE lurched forward to grab the hard hat and hit head on a support for the conveyor wiper which cut EE's head. EE received 5 staples from the ER. EE returned to doctor the next day and was released to full duty.

Concrete finisher was working in the area of a concrete block wall (eco blocks) when a landslide occurred causing the wall to fail. The wall fell and made contact with the workers lower leg. Worker initially declined any treatment and said was ok.

Climbing down vertical ladder off of the filter press. Employee got a sharp pain in wrist and thumb area of left hand.

Re-installing hydraulic motor on power screen with the assistance of another employee. Miner felt some pain in shoulder and sought medical attention. Miner was given 2 days off before returning to work.

Miner was collecting fire extinguishers in preparation for the annual inspection. The miner looked into the now abandoned wash plant control room by boosting himself up to look inside. When he did so he rested on his stomach and chest. When he did so he heard a pop. The miner finished out his day. The following Monday the miner informed us he would seek medical treatment.

He told his supervisor at around 11:00am that he was experiencing back pain that had been getting progressively worse for the past several weeks. He stated that on March 26th he reached to retrieve some food from his lunch box and felt something pop in his lower back. At 13:45 he informed his supervisor he might not return to work on Monday and he was going to see his chiropractor.

Miner was doing his regular work of shoveling, cleaning, picking up parts and materials to insure a safe workplace. Worker felt pain in lower back.

Miner was unhooking lifting hook on boom truck from storage position. Miner placed right hand near shrive and when block move it caught right little finger between shrive and wire rope.

Working as a Utility man cleaning and conducting plant maintenance, the worker was removing the bottom section of a return roller guard that is hinged on one side and bolted on the opposite side. As he removed the nut securing the guard, the guard swung down and the corner struck the back of his right hand. This resulted in a laceration, requiring five stitches.

Two miners were installing a control center cover. One miner accidentally allowed the cover to slip and that pinched the other miners left index finger between the cover and the machine. This caused a laceration which required stitches.

Miner was fabricating a support for a water tank overflow pipe. The miner removed his hard hat temporarily to put on his goggles and was shaking the goggles to remove rain water, when he accidentaly dislodged the support angle iron which struck him in the forehead.

While lifting a steel grate to position it into its concrete framework the miner dropped the steel grate without first removing his fingers smashing the middle and ring finger of his right hand.

Worker was picking up a 42" troughing roller when he felt a pain in his left middle back.

While loading old equipment onto a trailer the miner was struck by shifting equipment.

Three people were trying to lift a large electrical panel and it slipped and fell hitting his left knee.

WHILE UNLOADING A CONVEYOR OFF A FLATBED, DURING LIFTING THE CONVEYOR, A 4 FOOT SECTION PIVOTED DOWN HITTING EMPLOYEE IN THE BACK.

SPRAINED LEFT KNEE WHILE HELPING ANOTHER EE POURA SLAB OF CONCRETE & WHILE LIFTING ONE END OF THE BOARD HIS KNEE POPPED & HE FELL DOWN

EMPLOYEE WAS CLEANING OUT GRIZZLY, FINGER BECAME CAUGHT BETWEEN PRY BAR AND HAND RAILING.

BROKE HIS FINGER DOING MAINTENANCE ON CRUSHER.

HIT ELBOW ON A PIECE OF IRON.

WHILE MOVING EQUIPMENT INTO A STORAGE SHED, BACKED INTO SOMETHING & LOST FOOTING. EQUIPMENT CARRIED THEN FELL ON TOP OF RIGHT LEG, BRUISING THE CALF.

EE CUT SUPPORT FROM HOPPER. HOPPER ROLLED ON EE S ARM.

EE WAS LAYING DOWN WORKING OVERHEAD ON CRUSHER SCREEN. WHEN HE TURNED AT AN AWKWARD ANGLE, HE TWISTED HIS BACK.

WHILE REPAIRING CONVEYOR BELT HAND HUNG UP ON BROKEN BELT CLIP.
