Dust hose to the dust collector pot became full of mud. EE removed the hose at the lower point to to clear it. The hose fell off. employee continued to unclog the hose by elevating it the middle. EE ended up moving hose off to the side to allow it time to dry out. The next day EE returned to installed it. The employee said EE did not need medical attention till 4-22-26

Mostrando los 17Employee was underneath an articulated haul truck setting jacks and cribbing. The job site was very muddy and while twisting and grabbing equipment they felt their left knee pop. Called nurse triage, they advised visiting urgent care. Doctor orders from urgent care stated employee needed to be off of the appendage and work until an MRI can be taken. MRI scheduled through work comp.
Employee was in the Secondary tower running the plant when employee noticed that the feed stopped due to a plug. Employee left the tower and Locked Out the #1 belt and the feeder pan in the tunnel. Employee went to inspect the plug. Employee put on gloves and attempted to remove the plug. While removing the rock, employee's finger was smashed between 2 rocks.
Employee reached out to install a ground clamp for the welder, as EE brought EE's right hand back to EE's body, EE's hand struck the damaged metal guard EE was about to repair causing a laceration to EE's right palm.
Employee attempted to throw metal pipe into roll off bin pipe struck bin and came back and struck employee in the left ear causing laceration that required stiches.
Employee received a contusion from hammer bit that struck ee.
On 7/15/2015 EE's right index became caught between the timber head and head cable of the ramp barge while taking the cable over the timber head. Tip of finger was surgically amputated the same day. EE was released to restricted duty following finger surgery. 7000-1 will be updated when EE is removed from restricted duty.
Miner suffered broken bone in left hand while prepping conveyor for transport. Hand was caught between portable 5th wheel plate arm and conveyor frame as miner was moving the plate into transport mode. Surgery was conducted the same day, hand placed in cast and miner was released to light duty following surgery.
The belly pan of a Cat 730 haul truck fell on an employee's wrist casuing a break to his right wrist.
Employee was repairing an axle trunnion and while setting it on the ground, left hand middle finger became trapped between the part and a bolt causing fracture and laceration to finger.
Picked up a cribbing block and put it in the back of a truck. The block rolled back smashing his finger between the block and the side of the truck.
While doing maintanence on a locked out, tagged out not running vibrating feeder pan, employee inadvertently rotated a pulley and pinched right thumb.
Employee claims he was moving a fuel hose when he aggravated his left shoulder. This employee had previously injured this shoulder in a NON-WORK related accident. This was not recordable until he went to the doctor on May 4, 2007 and received a cortisone shot in his shoulder.
Ee was tarping a load and the tarp fell into the back of the truck. He went into the truck bed to retreive the tarp and as he picked it up he noticed a catch in his shoulder. He refused medical treatment until 9/15/06, and at this point it became MSHA reportable.
EE OPENED SIGHT DOOR OF OIL BURNING STOVE TO LOOK IN AND SOOT BLEW INTO HIS EYES CAUSING ABRASIONS.
EE STRUCK HIS LEFT KNEE ON A PIECE OF ANGLE IRON, WHILE CLIMBING A SCREEN LADDER, CAUSING A LACERATION.
PULLED SHOULDER WHILE CLIMBING DOWN LADDER.
