Employee stated that they were climbing up onto the dozer to work on the engine. As they did, they felt a pop in left knee. They experienced extreme pain in the knee as they attempted to walk on it. Went and got checked. Missed work on 06/24 and returned on regular duty on 06/25. Went to ortho on 06/27. Returned to work on regular duty on 06/30. Lost 2 days work.

Mostrando los 27Employee was lifting a 3 gallon bucket of oil onto loader bumper. As they were lifting, their feet got stuck in mud causing them to lift & turn their back simultaneously. Minor pain rest of day. Next day they said they could hardly move. Went to ER and off work since that day.
Employee stated was picking up a metal cover to put back onto the side of a hydraulic tank on a hauler. When employee did, right arm started tingling. Pain developed in right shoulder area. Went to doctor to have checked. First lost day of work occurred on 5/15/2018.
Employee stated that EE was climbing up the ladder on the hauler to check out a steering leak. The lower step swung underneath the bumper causing EE to pull self up the ladder. EE felt pain in the arm, neck, shoulder and back area. Went to doctor to get checked and missed work starting 2/21/17.
Employee stated was lying on right side on the ground installing a roller cap and 2 bolts on dozer. During this process, felt a pop in right shoulder. Continued work and went to doctor on 12/14/16 to have checked due to still being sore. Was placed on restrictions starting 12/15/16, thus reportable.
Employee stated that EE used a torch to heat up a hand rail on dozer so as to bend it back into place. As EE was getting down, EE accidentally put hand on the hot metal hand rail causing burn to right hand. Got checked on 7/21/16 and had some skin debrided and bandaged. Was put on restricted duty on 7/26/16 and returned on regular duty on 8/2/16.
Removing a D11R axle from the final drive. Hand slipped down axle pinching left pinky between axle and roller frame causing a laceration.
Employee stated that he handed another mechanic a step ladder to put into his truck. As the mechanic was putting the ladder into the truck, it hit the tailgate causing the end of the ladder to strike the injured employee's nose resulting in a laceration to the nose. Reportable due to requiring two sutures.
Employee stated that he was getting up into the hauler. The first step was a little high off of the ground. He grabbed the handrail to pull himself up the ladder. When he did, he felt a sharp pain in his right shoulder. Went to ER that night to get checked. Kept off of work for the next two days.
Employee stated that he was standing beside parts truck while another employee was dislodging a stuck piece of steel plate in the parts truck bed with a sledge hammer. When the employee was swinging the hammer against the steel plate, he missed and the hammer slipped out of his hands striking the injured employee in the mouth area causing a laceration requiring sutures.
Employee was re-installing a metal plate on a Komatsu WA900 when it slipped catching his left pinky finger between the plate and a hydraulic tank
Loading wet hole bags off a trailer onto the support truck for the shot, hurt elbow.
Employee stated that they had chained up the push beam on a dozer so as to move it out of the shot. The employee was pulling on the push beam while another employee was removing the chain which was stuck. He said he felt a pop in his right shoulder. Went to doctor, missed work shift on 1/20/14 and on restrictions until follow-up appointment.
Employee stated that he was going to the loader when he stepped onto uneven ground. As he was climbing onto the loader, he felt a pop in his right knee. His knee started hurting and swelling later. Continued to work but went to ER to get it checked later that evening. Missed the next work shift.
Employee stated that he was helping the mechanic put the bottom foot plate on the left leveling jack on the drill. When he raised up, he hit the back of his head on the leveling jack causing a head injury. Has not missed any work shifts but is reportable due to being on light duty.
Employee stated that he was helping set up a 6 inch water pump. He was pulling the discharge hose while walking backwards and felt a sharp pain in his lower back. Did not go to doctor until 03/20/13 and was not reportable until he missed his first shift of work on 4/04/13, thus reportable at that time.
Pulled a piece of metal out of clamshell, when it cleared one side, it sprang out and struck him on the chin & chest.
Employee stated that he was servicing a dozer. He was climbing on and off the dozer repeatedly. He stated that he hurt his right foot during this process. He went to doctor on 11/19/12 and missed work starting 11/20/12.
Employee stated that he was hanging a temporary curtain in rear window of loader so as to block glare from the light plant lights. His hand hit the overhead liner on the cab causing dust/dirt particle to fall into left eye.
Employee stated that he was in the process of welding a step back onto the hard nose of a dozer. When he placed the welding ground line onto the step, the step fell off and struck employee on the top front of his head causing a laceration. Reportable due to requiring sutures.
Employee stated that he was placing a piece of drill steel into the rack on the drill. He sat down on the cab of drill to push end of steel into place with left foot. Drill steel kicked back, forcing leg backward, resulting in pain. Back strain; possible vertebra problem or pinched nerve. X-ray negative. Working restricted duties; attending physical therapy.
EE STATES HE WAS WORKING ON DRILL BENCH AND IT WAS RAINY AND MUDDY. HE GOT OUT OF THE TRUCK AND WALKED TOWARD THE TRUCK CONTROLS ON THE BACK OF THE TRUCK. HE TURNED TO REACH FOR THE CTONROLS A ND FELT HIS BACK PULL. HE THOUGHT THE PAIN WOULDGO AWAY HE WENT TO THE DR THE NEXT DAY BACK SRAIN.
THE EE WAS MOVING AN ENGINE VIBRATION DAMPER TO ALLOW ACCESS TO THE FRONT CRANKSHAFT SEAL WHEN HE FELT SOMETHING POP IN THE LOWER MIDDLE PORTION OF HIS LOWER BACK.
EE STATED THAT HE WAS GETTING INTO THE LOADER CAB. HE HAD OPENED THE DOOR AND WAS REACHING IN TO GET A BROOM SO AS TO SWEEPOUT THE CAB. AS HE WAS STRETCHING/TWISTING, HE FELT A PULLING SENSATI ON IN HIS LOWER BACK/LEFT SIDE AREA. REPORTABLE DUE TO MISSING ONE WORK SHIFT ON 8-9-03.
EE RAISED THE ENGINE HOOD ON THE HAULER TO CONNECT AN ELECTRICAL WIRE ONTO THE AC COMPRESSOR. HE ACCIDENTLY HIT AN ANTIFREEZE LINE WITH HIS FOOT CAUSING IT TO RUPTURE AND SPRAY HOT ANTIFREEZE ONTO HIS RIGHT SIDE/HIP AREA. HE WENT TO DOCTOR DUE TO BURNS AND SOME BLISTERING ON SIDE/HIP. NO LOST TIME INCURRED.
EE STATED THAT HE WAS INTHE PROCESS OF PUTTING THE SIDE DOORS BACK ONTO THE DOZER ENGINE COMPARTMANT. AS HE WAS SETTING THE DOORS ONTO THE PUSH BEAM ONT HE DOZER, THE DOORS FOLDED TOGETHER CAT HCING HIS OFF LITTLE FINGER IN THE DOORS CAUSING A LACERATION TO HIS FINGER NO LOST TIME BUT REPORTABLE DUE TO REQURING SUTURES.
EE STATES: WHILE HOOKING UP DISCHARGE HOSE TO THE TRUCK IT CAME OUT AND HIT MY LEFT KNEE.
