No 5 Strip Coal

Employee stated that he was driving to the job parking lot. He hit a hole in the road causing the seat in his mechanic's truck to bottom out. He stated that this caused immediate pain. He mentioned pain to his back/leg area a few times afterwards. He continued to work and never went to doctor. Left work on 1/12/16, missed starting 1/13/16, and now wants to see doctor.

Employee stated that he was helping set up a 6 inch water pump. He was pulling the discharge hose while walking backwards and felt a sharp pain in his lower back. Did not go to doctor until 03/20/13 and was not reportable until he missed his first shift of work on 4/04/13, thus reportable at that time.

Employee stated that he was on the ground outside the drill. He stepped on a rock and twisted his left ankle. The pain got worse as day progressed. He could not put weight on his left ankle the next day. Went to doctor on 6/14/12 and missed work.

Employee was hauling coal from surface pit to tipple in a Mack DM886SX tandem coal truck. As he was descending a grade on haul road, the truck overturned onto the driver's side entrapping the driver. The driver had a medical condition which led to the accident. The heart attack led to the driver's death.

Employee stated that he was preparing to put a tooth on the loader bucket. He stepped on some ice on the ground, slipped, fell and caught himself against the bumper on his mechanic's truck. He twisted his back resulting in low back pain.

Employee stated that he was in the process of welding a step back onto the hard nose of a dozer. When he placed the welding ground line onto the step, the step fell off and struck employee on the top front of his head causing a laceration. Reportable due to requiring sutures.

Employee stated that he was helping mechanic to change the teeth on a loader bucket. Mechanic cut teeth off with cutting torch and employee was knocking teeth off with a hammer. A piece of metal slag flew off, struck and embedded in his upper left arm.

Employee stated that he was operating a dozer scaling the highwall. He was prying on a rock with the dozer blade, when the blade slipped off and hit the highwall. This stopped the dozer and caused a whiplash effect. Went to doctor on 5/29/09 and on 6/9/09 missing two shifts work, thus reportable.

He was coming up the hill in a truck and assumed that something in the transmission broke and turned the truck up onto its side.

Employee stated that he was standing on his dozer cutting a fallen tree off of another dozer. When the tree was cut, it pushed him off of the dozer causing him to fall onto the ground resulting in some abrasions, contusions, and some back pain.

Employee stated that he was placing a piece of drill steel into the rack on the drill. He sat down on the cab of drill to push end of steel into place with left foot. Drill steel kicked back, forcing leg backward, resulting in pain. Back strain; possible vertebra problem or pinched nerve. X-ray negative. Working restricted duties; attending physical therapy.

Stepping out of the truck, felt pain go down left side of back & down left leg.

Employee stated that he was walking down the steps on a loader. His right foot slipped on next to last step causing him to lose his balance. He was holding onto the handrail and as he started to fall, he gripped the rail tighter causing him to sprain his right wrist.

EMPLOYEE STATED THAT HE WAS BACK DRAGGING THE ROAD WITH THE DOZER. HE BACKED OVER A ROCK WITH THE DOZER TRACK. AS HE DID, THE DOZER JARRED THE EMPLOYEE WITH PAIN OCCURRING IN LOWER BACK AREA RADIATING INTO GROIN AREA. WENT TO DOCTOR IN ER THAT NIGHT AND MUSSED THE NEXT SCHEDULED WORK SHIFT.

EE STATED THAT HE WAS ON A LADDER CUTTING THE TIRE GUARD OFF OF THE HAULER BED. THE GUARD BROKE OFF SUDDENLY. IT PARTIALLY STRUCK HIS FACE AREA CAUSING HIM TO LOSE HIS BALANCE. HE TURNED AND JUMPED ONTO THE GROUND. HE LANDED ON HIS LEFT LEG AND THE IMPACT CAUSED A FRACTURE TO HIS LEFT LEG WITH LACERATIONS TO HIS FACE.

EMPLOYEE STATED THAT HE WAS OPERATING A DOZER PUSHING SHOT ROCK TO THE LOADER. HE BACKED UP TO AVOID A ROCK WHICH WAS GOING TO FALL FROM THE MATERIAL BEING PUSHED. WHEN HE DID, HE BACKED THE D OZER UP AND INTO A LARGE BOULDER WHICH CAUSED THE DOZER TO JAR THE EMPLOYEE. WENT TO DOCTOR ON 1-15-04 AND INCURRED LOST TIME ON THAT DATE.

EMPLOYEE WAS USING A LOADER TO MOVE A LARGE ROCK OUT OF THE WORK AREA. AS HE WAS CARRYING THE ROCK UP THE ROAD, THE LOADER BECAME UNBALANCED AS IT TOPPED THE CREST OF THE ROAD. THE LOADER BUCK ET HIT THE GROUND CAUSING THE LOADER TO JAR THE EMPLOYEE. HE HIT HIS HEAD AGAINST THE FRONT WINDSHIELD CAUSING A LACERATION REQUIRING SUTURES.

EE NOTIFIED EMPLOYER ON 7/23/02 THAT ACCORDING TO HIS DR., HIS PREVIOUS NECK INJURY WAS AGGRAVATED BY AN INCIDENT OCCURRING ON 5/24/02. ACCORDING TO THE EE, HE WAS OPERATING A HAULER & HE WAS DUMPING THE LOAD ON A STEEP GRADE. THE LOAD DUMPED OUT QUICKLY CAUSING THE HAULER BED TO MOVE UP AND THEN BACK RATHER QUICKLY. THIS CAUSED THE HAULER TO JAR THE EE'S NECK.

EE STATED THAT HE WAS WORKING ON A HYDRAULIC PROBLEM WITH THE DRILL. HE STEPPED ONTO THE COVER OVER THE DRIVE LINE AND DOWN ONTO THE OTHER SIDE. AS HE DID, HE STEPPED ONTO SOME HYDRAULIC HOSES ON THE DRILL CAUSING HIM TO TWIST HIS LEFT ANKLE. NO LOST TIME INCURRED BUT IS ON RESTRICTED WORK ACTIVITY.

EE STATED THAT HE WAS STANDING ON THE BUMPER OF THE MECHANICS TRUCK WITH ONE FOOT ON THE PUSH BEAM ON THE DOZER. AS HE WAS HEANDING AN OIL CAN TO THE MECHANIC ON THE DOZER, HIS FOOT SLIPPED CA USING HIM TO FALL AND HIT HIS LEFT HIP AREA TAILBONE ON THE PUSH BEAM. DID NOT GO TO DR UNTIL 3-21-02 AND WAS ONLY REPORTABLE WHEN FIRST LOST WORK DAY OCCURRED 4-5-02.

EE STATED THAT HE WAS INTHE PROCESS OF PUTTING THE SIDE DOORS BACK ONTO THE DOZER ENGINE COMPARTMANT. AS HE WAS SETTING THE DOORS ONTO THE PUSH BEAM ONT HE DOZER, THE DOORS FOLDED TOGETHER CAT HCING HIS OFF LITTLE FINGER IN THE DOORS CAUSING A LACERATION TO HIS FINGER NO LOST TIME BUT REPORTABLE DUE TO REQURING SUTURES.

EMPLOYER RECEIVED ON OR ABOUT 5-1-01 A NOTICE DATED 4-27-01 WHERE THE EMPLOYEE HAS FILED A CLAIM FOR BLACK LUNG BENEFITS. THE EMPLOYEE ALLEGES COAL WORKERS PNEUMOCONIOSIS. NO LOST TIME HA S BEENINCURRED.

EE STATED THAT HE WAS SCALING THE HIGHWALL WITH A DOZER WHEN A LARGE ROCK BROKE LOOSE. THE ROCK FELL AND HIT THE CORNER OF THE DOZER BLADE AS HE WAS BACKING UP AND OUT OF THE WAY. THE CAUSED T HE DOZER TO SHAKE/JAR THE EE RESULTING IN A MILD CERVICAL STRAIN. REPORTABLE DUE TO LOST WORK SHIFT ON 3-9-01.

EMPLOYEE WAS REMOVING A PULL DOWN CYLINDER FROM A HIGHWALL DRILL. THE CYLINDER BECAME STUCK. HE PRYED ON CYLINDER AND WHEN IT WAS PRYED LOOSE ITSWUNG AROUND ON THE ATTACHED SAFETY CHAIN. THE END OF THE CYLINDER HIT THE TOP OF THE EMPLOYEE'S RIGHT FOOT BEHIND THE STEEL SAFETY TOE.

THE EE STATED THAT HE WAS OPERATING A HAULER IN THE STRIP PIT. THE HAULER WAS BEING LOADED WITH SHOT ROCK BY A FRONT END LOADER. A LARGE ROCK FELL OUT OF THE LOADER BUCKET AND ONTO THE CAB SHI ELD OF THE HAULER. THIS CAUSED THE HAULER TO ROCK AND JAR THE EMPLOYEE.
