EE was pulling drill steel out of top and fractured his right little finger between drill steel and drill pot.

Employee was changing crawler take-up on Joy 14CM15 miner. The gathering head pan was chained up. He placed himself under the gathering head pan to push take-up in place with his feet. The chain broke causing him to be caught between the gathering pan and mine floor. He had fracture of nasal bone & 3 ribs, sprain of sacroiliac & pelvis, pneumothorax trauma & liver laceration.

EE was hanging a ventilation curtain and cut his left middle finger, requiring stitches, on a CAB brattice hanger.

EE was cutting metal band on a pallet of Heintzmann jacks. When the band was cut, ajack rolled off the pile and struck his left foot.

EE was pushing/installing a 16 foot cable roof bolt when he felt pain in his right shoulder.

EE was moving a set of cable crossovers. He felt pain in his lower back and the left side of his middle and upper back

EE alleged experiencing stomach muscle strain after lifting on roof bolter T-Bar. Did not start missing work until 8/18/11.

Employee was repairing the high voltage cable after a copper theft at the surface substation. He dropped the PLM coupler and mashed both thumbs.

EE picked up a bag of rock dust, twisted to put on Boss Hoss. Alleged pain in his lower back. E-Doc #14116925 that was submitted 5/10/11 for alleged back injury on 4/28/11 was determined by Workers Compensation (BrickStreet) to have sufficient inconsistencies and contradictions to concluded that he was not injured in the course of and result of employment.

EE bent over to pick up his miner cable and felt pain in his lower back.

EE was drilling mine roof, a piece of rock hit drill steel and caused steel to come out of drill chuck. The drill steel struck his left forearm. He continued working his regular scheduled shift thru 5/3/11.

Employee "ALLEGES" that while hanging a miner cable some foreign material (rock dust) fell into his right eye. Subsequent investigation revealed the employee actually had rust in his eye from an off the job accident. The employee's workers compensation claim was denied based on this fact.

EE felt pain in his lower right groin area when he stepped off the roof bolter he was operating. He continued working until 4:00 pm, end of shift and worked the next day, his last day worked 1/6/11.

EE was loading pizza pans on the roof bolter, felt pain in groin area on Saturday, went to Doctor on Monday and returned to work on Tuesday.

EE was lifting a rock that was hung between tailpiece and feeder and felt pain in the groin area. His first day missing work was 9/28/10.

Employee "ALLEDGELY" strained his back while lifting roof support pizza pans.

EE was installing belt on #9 Belt Conveyor. Both hands got caught under cable, cutting and mashing fingers on both hands.

EE was moving a feeder reducer from one side to another, pulled muscle on right side of back with right leg pain.

While restoring old mine works for advance mining, EE was lifting on a rail to remove a chain and pulled a back muscle.

Putting BOP on wellhead. BOP was started and was not threaded on far enough when it fell to the ground striking employees foot.

EE was assisting in making a belt move when the tailpiece kicked sideways. The side board knocked him into the rib, cutting his head and mashing his left thumb.

Employee was lifting an Omega block to the top row of a stopping and felt pain in his left shouder, but continued woking. He called the mine office on 8/18/09 and informed them that he had gone to the doctor and rotator cuff surgery had been scheduled and that he would be off work from that date.

Employee was unloading belt structure, structure hung up and jerked his left shoulder. His first day missed was 9/10/09 for surgery to left shoulder.

Employee was pulling 3" PVC pipe in No. 3 belt entry, pipe hung up and struck top of his right foot.

Employee ALLEGED to have felt pain in his lower back and both legs while unloading three (3) Heintzmann jacks off a battery powered ride. Note: Employee worked 3-23-09 & then began missing.

Employee lacerated his left indx finger in a hole on the ATRS extension while installing the ATRS on the roof bolter.

Employee injured his left shoulder while putting up rib boards. His first day missed due to the accident was 3/20/09.

Employee was putting scoop on charger and dropped batery lid on his hand.

Employee was helping make a belt move, picked up a piece of structure and twisted lower back.

EE was helping make a belt move, he stepped on a crossbar, it flew up striking his right arm and left cheek of his face.

Employee was lifting locker in change room; locker slid thru hands cutting palm of left hand.

Employee was lifting shields off of miner, Injured/strained lower back. First day missed due to injury was 8/12/08.

EE pulled something in his lower back while shoveling mud from under section tail piece onto belt.

Injury "ALLEDGED" to have injured his back when he hit his head on the mine roof while loading 8 foot roof bolts onto the roof bolting machine. He continued to work until his first day missed on 6/14/08.

Employee was pulling on conveyor belt when he injured his back.

Employee was shoveling under the belt conveyor when he heard a pop in his right arm. He continued working till his first day missed on 5/12/08.

EE strained his back when lifting on the miner cable during a power move. He continued working his regular job until 3/18/08, his first day missed.

At #9 belt head on #2 section strained right knee while changing reducer. Note: EE didn't miss any work while at Mine #25. He transferred to Mine #18 on March 6, 2008.

Employee was stacking structure into a supply car and a piece fell and cut his finger.

Employee was changing a belt scraper; scraper fell catching his left hand between scraper and belt frame.

Employee was helping change shield on continuous miner, caught right hand under shield and caused a laceration.

Employee was leveling belt structure. When he lifted bottom stand he felt a burning sensation in his back. First day missed due to injury was 9/26/07.

Employee "ALLEGEDLY" injured his neck and shoulder when belt was being changed out. Belt structure came apart and fell on him while sitting in scoop. He finished the shift.

Employee was stacking belt structure when a piece slipped out of hand and struck his left knee.

Employee "ALLEDGEDLY" twisted his lower back while loading belt structure in scoop bucket. He did not miss work until 4/25/07.

Employee was getting a crib block out of the belt head, cut right ring finger and strained right arm.

Employee was moving the feeded cable, a timber fell striking his lower back.

Employee was closing air-lock door on slope belt, slipped in mud and door closed on right hand.

Employee was changing a hydraulic pump on #2 Shuttle Car and dropped pump on finger.

Employee was taking lid off shuttle car, lid slipped out of helper's hand and all the wieght of panel went to employee, injuring his back. Employee quit 2/20/07.
