Was riffling a coal sample and was turning his body while carrying a the sample, weighing approximately 70 pounds, and his left knee gave out on him. There was swelling and he could not put a lot of pressure on his leg.
Employee was lifting a miner cable when he strained his back. The employee continued to work until May 23, 2011 at which time he was removed from work by his physician. This accident has been under investigation and has now been determined (June 17, 2011)to be work related and reportable.

Employee was loading belt structure in to a transport vehicle when he slipped allowing the piece of structure to strike his right hand. This resulted in a fracture and a laceration requiring stitches to his index finger on his right hand.
Employee was loading rock dust into a duster. He was bending over and lifting the rock dust bags from the mine floor. He experienced pain in his lower back. He continued to work until May 19th. At this time he started to miss work.
EE was changing button belt rollers. EE used a come a along to pull up the belt, but had to install the button roller by lifting them and pushing them into place. EE was working in a stopped ot bending position.
The EE was cutting off the sideboard of a dozer blade. After making the cut, he struck the metal with a hammer to knock off the blade. The metal did not fall. He grabbed metal with hand and metal fell striking him on top of right foot behind the hard toe cap.
Employee was assisting in replacing a hydraulic jack on the continuous miner. The jack slipped, catching the employee's left little finger between the jack and the miner frame. This resulted in laceration to the left little finger.
Employees was changing tires on the RD 800 Mack. EE was putting the inside tire back on truck when his middle finger on his left hand was pinched between the tire & spoke of the truck. He was taken to Norton ER stitched the end back on going to give a week to heal may have to take off first knuckel.
Employee was driving a coal truck & the tailgate of the truck popped open. He was trying to close it back, his ring finger on his right hand was smashed & cut. He was taken to the E.R. and had 20 stitches.
WE WERE USING A CRIB BLOCK TO BLOCK OPEN THE TAILGATE OF A MACK MUD TRUCK SO WE COULD CLEAN MUD FROM AROUND TAILGATE, AS WE WERE PLACING CRIB BLOCK AGAINST TAILGATE, CRIB BLOCK SLIPPED & STRUCK THE RIGHT WRIST OF INJURED EMPLOYEE. HE DID NOT WANT TO SEE A DR. AT THAT TIME BUT LATER ON THE 24TH HE WENT TO A DR.
Employee was carrying an outrigger pad around to set up on the boom truck. While carrying the pad, he stepped in a hole in the ground and felt a sharp pain in the groin area.
Employee was trying to pick up on a steel beam to work it into position for installation, when he felt a sharp pain in his groin area. Not reported to supervisor until 7/15/05.
EE REPORTED BACK PAIN ON 01/04/05 AND CONTINUED TO WORK UNTIL 02/14/05. EE SAID WAS BENT OVER WHEN EE EXPERIENCED PAIN IN BACK. EE WENT TO DR ON 02/14/05. DR TOOK EE OFF WORK PENDING THE RESULTS OF AN MRI. RESULTS WERE RELEASED TODAY SHOWING NO HERNIATION WITH DEGENERATIVE DISK DISEASE (MODERATE)

EMPLOYEE WAS REMOVING BOLTS FROM A TWELVE INCH WATER LINE HANGING FROM THE ROOF. ONCE BOLTS WEREREMOVED THE WATER LINE SLIPPED FROM HIS HANDS AND LANDED ON HIS LEFT KNEE.
EMPLOYEE WAS BENDING A 5 FOOT ROOF BOLT WHEN HIS HANDS SLIPPED OFF THE BOLT. THE BOLT REBOUNDED & STRUCK HIS LOWER LEFT JAW. HE WAS WEARING LEATHER GLOVES. FRACTURE TO JAW WITH 3 TEETH DAMAGED
ATTEMPTS WERE BEING MADE TO TURN OVER THE GEARS OF A REAR END FOR REPAIRS WHEN THE TIP OF A MIDDLE FINGER WAS SEVERED FROM A LEFT HAND.
HE WAS WORKING ON A D-10 DOZER. HE HAD BOTH PUSH ARMS BLOCKED AND WAS WORKING ON THE OUTSIDE OF THE ROLLER FRAME. THE LEFT SIDE PUSH ARM SLIPPED, ALLOWING THE ROLLER FRAME TO DROP APPROXIMATEL Y 4". A TRACK CLEAT STRUCK THE EMPLOYEE'S LOWER RIGHT LEG, CAUSING A CONTUSION. HE STARTED MISSING WORK ON 4/29/03.
EE WAS REMOVING MINER CABLE FROM CABLE HANGING HOOKS WHEN THE DROPPED UNEXPECTEDLY. CABLE HANGER CAUGHT HIS RIGHT INDEX FINGER CAUSING A LACERATION AND FRACTURE AT THE SECOND NUCKLE.
EMPLOYEE STATED HE WAS PLACING ROOF BOLTS IN THE ROOF BOLT TRAY WHEN HE FELT A PAIN IN HIS BACK. DOCTOR STATED HE HAD REINJURED AN OLD INJURY.
HE WAS LOADING 4 INCH THICK CONCRETE BLOCKS INTO A SCOOP. ONE BLOCK SLIPPED AND CAUGHT HIS LEFT HAND AGAINST THE BUCKET SIDE BOARD.
EE WAS INSTALLING THE ENGINE COMPARTMENT SIDE DOOR ON A DOZER. DOOR WEIGHT ~30LB. HE WAS HOLDINGIT IN PLACE WITH ONE HAND & ONE KNEE. DOOR SLIPPED & FELL, CATCHING HIS LEFT LITTLE FINGER BETWE EN THE DOZER TRACK CLEAT & THE DOOR. THIS RESULTED IN A FRACTURE WITH LACERATION AT THE FIRST JOINT OF THE FINGER.
EMPLOYEE WAS LOADING OIL FROM 5 GALLON CANS INTOA HYDRAULIC RESERVOIR WHEN HE EXPERIENCED PAIN IN HIS BACK. POSSIBLE LOWER BACK STRAIN.
EMPLOYEE WAS LIFTING SUPPLIES FROM A SCOOP TO LOAD ON THE ROOF BOLTER WHEN HE EXPERIENCED PAIN IN HIS GROIN. HE WAS TAKEN TO THE DOCTOR WHO FOUND NOTHING AND GAVE HIS A RETURN TO WORK SLIP. EMPLOYEE WORKED UNTIL 9/26 WHEN HE WAS DIAGNOSEDWITH A HERNIA. HE STARTED MISSING WORK ON 9/27.
EMPLOYEE WAS USING AN ANCHOR PIN TO BEAT ROCK THAT HAD FALLEN INTO THE FEEDER ON THE 001 SECTION. AS THE ROCK BROKE, HIS LEFT HAND CAME INTO CONTACT WITH A PIECE OF SHARP ROCK. THIS CAUSED A L ACERATION TO HIS LEFT HAND WHICH REQUIRED 6 STITCHES.
EMPLOYEE WAS WORKING IN LOW COAL TRYING TO MOVE A VENTILATION CURTAIN WHEN HE FELT A PAIN IN HIS LOWER BACK.
HELPING TO LIFT A TRACK PAD FOR A D-11 DOZER WHEN HE EXPERIENCED PAIN IN HIS LEFT SHOULDER.
TWO EMPLOYEES WERE MOVING TWO FIVE GALLON BUCKETS OF OIL FROM THE TRACK OF A D-10 DOZER. ONE EMPLOYEE HANDED A BUCKET FROM THE TRACK TO THE EMPLOYEE ON THE GROUND. EMPLOYEE ON GROUND WAS SITTI NG THE BUCKET DOWN WHEN HE EXPERIENCED SHARP PAIN IN HIS LEFT SHOULDER-DISLOCATION.
WHILE HOLDING UP THE DRIVELINE SO THE TRANSMISSION COULD BE SLID IN, THE DRIVELINE SLIPPED AND FELL ON FINGER CUTTING THE FINGER ABOVE THE JOINT.
