Injured was in the process of changing a bolter tire and could not get it off. As he was trying to beat the wheel off with a hammer it bounced back and caused him to strain his wrist.

injured was using a pair of come-a-longs to pull open the northgate of the sump hopper. Injured felt a pain in his right shoulder.

INJURED WAS USING A UTILITY KNIFE TO CUT OFF A TORN PIECE OF CONVEYOR BELT FROM THE SECTION BELT. THE KNIFE SLIPPED AND CUT HIS RIGHT LEG REQUIRING 4 STITCHES.

injured was using a ratchet, socket and bolt tap to rethread bolt hole. Injured removed gloves to start tap in the hole. His left hand was on the ratchet head holding it on the tap and right hand pushing down on ratchet handle. The tap shank broke causing injured's left hand to move suddenly and strike his left forefnger on the broken tap resulting in a cut requiring stitches.

Injured was knocking a roll pin out of a splice link and removed his glasses to wipe the sweat off of his face. When he started striking the splice link with the hammer again he had forgot to put his glasses back on. A piece of metal struck him in the eye.

injured and co-worker were using a pipe wrench to free a pump sheave wheel. the sheave wheel suddenly free up causing injured to pinch his right ring finger between the sheaved wheel and the pipe wrench.

Injures was changing an oil filter when the wrench he was using slipped causing him to bump his right elbow.

EE was stripping wire with a razor knife, knife slipped and cut into upper thigh on right leg. Treatment consisted of 24 stitches.

The injured and co-workers were in the processing of removing conveyor belt from the belt takup. The injured was using a knife to cut the excess belt. The knife sliped sutting his left small finger. The cut required 4 stitches. The injured person was wearing gloves at the time of the accident.

Injured was using a 16# sledge hammer to break rock at a rock fall. injured was swinging sledge hammer and felt pain in his arm. (Injured did not request medical treatment until 04/28/06)

Injured was cutting a piece of belt with a utility knife. The kniffe passed through the belt faster than what he anticipated and entered his right leg just above the knee. Eight stitches were required.

Injured was cutting a piece of cable out of a belt line. The cable was under tension and when he cut it the cable wrapped around his left index finger and left little finger. This resulted in a cut to his left index finger which required stitches and a fracture to his left little finger.

Injured was sawing a 3x3 timber when the saw jumped out of the groove cut by the blade and cut his left index finger. Laceration was 2 inches long.

INJURED AND CO-WORKER WERE TAKING A CANOPY OFF A 630 SCOOP. THE INJURED EMPLOYEE WAS UNDER THE SUSPENDED CANOPY. WHEN THE SCOOP WAS MOVED, THE CANOPY BECAME CAUGHT ON THE SCOOP TIRE FENDER. CO-WORKER USED A BAR TO LOOSEN THE CANOPY CAUSING IT TO SHIFT AND STRIKE INJURED IN THE HEAD.

INJURED WAS RETRIEVING A SHOVEL FROM UNDER A PILE OF ROCK. THE SHOVEL HANDLE BROKE AND STRUCK INJURED ON HIS LEFT FOREARM RESULTING IN LACERATION.

INJURED WAS USING A PRY BAR TO SUPPORT A SPROCKET. THE PRY BAR SLIPPED CAUSING INJURED TO CATCH HIS RIGHT THUMB BETWEEN A STAGELOADER CHAIN LINK AND THE PRY BAR. THIS RESULTED IN A LACERATION REQUIREING 5 STITCHES. INJURED WAS WEARING GLOVES AT THE TIME OF THE ACCIDENT.

The injured was positioned at the mouth of the 8K fault heading approximately 3 feet inby the outby rib corner. A catamaran jack was position at 56 crosscut in the #1 entry 1/2 crosscut outby the injured's locaton. The catamaran jack chain was tensioned and a chain link broke and ricochet into the break striking the injured.

Injured was cutting a piece of plastic on his cap light prior to the start of his shift. As he pulled the knife, the bladed passed across his finger. The laceration required three stitches.

Working on dozer (D-10). Lifting on lift jack and hurt back.

Employee claimed back pain next day after using chain falls on 10-28-04.

HAD RAIL CAR WRECKED IN BOOSTER DRIVE AREA AT #21 BREAK. HAD RAIL CAR JACKED UP AND WAS GOING TO PULL CAR OVER WITH 3/4 TON COME ALONGS. HOOKED UP TO A SHACKLE ON A RIB BOLT. WHEN PRESSURE FROM THE COME ALONGS LOOSENED PLATE, A PIECE OF RIB ROLLED OUT & STRUCK EE ON THE OUTSIDE OF THE RT. LEG.

Injured was using a 3/4 ton come-a-long to couple carrier to a locomotive. The come-a-long spun around catching his finger between the handle and the chain.

injured was using a come-a-long to remove the door from the longwall head drive. the door fell on injured leg.

EMPLOYEE WAS USING CHANNELOCKS TO LOOSEN A NUT ON A BRACKET THAT ANCHORED THE END OF THE TROLLEY WIRE. AS HE PULLED, THE CHANNELOCKS SLIPPED CAUSING HIM TO LOSE HIS BALANCE AND TWIST HIS RT. KNEE.

The injured was using a knife to cut a hole in a piece of belt. The knife cut through the belt and into injured left wrist. This resulted in a cut requiring approximately 16 stiches.

THE INJURED EMPLOYEE WAS USING A SLEDGE HAMMER TO KNOCK OUT CONCRETE BRATTICE BLOCKS. WHEN HE SWUNG THE HAMMER, THE HEAD GLANCED OFF A BLOCK AND CAUSED HIM TO CATCH HIS LEFT INDEX FINGER BETWE EN THE HAMMER HANDLE AND THE MANDOOR FRAME RESULTING IN A TIP FRACTURE.
