The employee was simply walking through the Maintenance Shop holding a box cutter they had just got out of the vending machine. Upon inspection of the box cutter, they noticed a foreign object (possibly glue) on the blade. They proceeded to wipe the blade off on their left sleeve, when they cut/punctured their wrist area.

Mostrando los 12Employee was clearing the opening of a vertical discharge pipe for the Cement Kiln Dust (CKD). The discharge of previous ckd had prevented the flow of discharge and had created a back-up. Employee used a rake to remove blockage at which point the material quickly flowed from pipe causing ckd to go airborne dusting the employee. The employee inhaled the material.

Loosening bolts on the 2nd deck of the wash plant using a crescent wrench when wrench slipped causing left hand to slam into sharp metal corner puncturing EE's glove, leaving a deep puncture wound in left hand.
Working on the durability cell cutting silicone liners with a straight razor. While holding it down with EE's left hand and cutting with EE's right EE cut through EE's glove and into EE's left thumb.
EE was asked by operator to re-install a ripper tip retaining pin. The pin had walked out of bore in ripper shank. EE struck pin with hammer to realign. When hammer struck pin, the impact caused a piece of metal to break off and was projected into EE's chest.
Employee received 2 lacerations on the right hand from a hand tool while troubleshooting a motor sensor.
Pulling with pry bar, bar slipped and smashed finger
EE right hand became caught between 2"x6" lumber and a rebar bundle.
