EE was pulling cable from a spool at the cable winder. A chain meant to keep the spool centered came loose, allowing it to shift and create a gap near the stops. The EE reached for the knuckle and, while pulling it down, placed their hand into the gap, where it became caught in a pinch point.

Mostrando los 26Miner was holding a timber while setting a breaker row. Another miner swung the axe to hit the cap piece and the axe glanced off and struck the victim on top of the hard hat.
While using a track jack, the jack bar slipped out of the employee's hand and struck them in the chin causing a laceration.
EE was removing the CM pot and while trying to pry the pot free the bar slipped and pinched EE's left hand ring finger between the bar and the CLA tip.
On September 23rd at approximately 9:00pm, EE was monitoring the LW belt, a 20' flapper was noticed traveling into the take-up. EE shut the belt down and locked out the drives and winch. EE then removed the guarding and began cutting out the flapper. While pressing up on the belt, the knife slipped and punctured their left wrist.
EE was trying to hook the knuckle to the pan. The draw bar needed to be coma longed into place. There was no D ring on the draw bar so ee hooked the coma long around the draw bar and then hooked the body of the coma long to the canopy. EE instructed another EE to raise the shield as EE was watching the draw bar the chain broke and struck EE in the mouth/nose.
EE was hanging line brattice in xc20 between #4 and #5 entry with a insulated hook and when EE lowered EE's hand the end of the hook had caught EE's glove tearing into the back of EE's left hand, between EE's index finger and thumb
EE was working on removing the wipe and hanger brackets on 5 West belt. While removing the come along from the wipe it became jammed and wouldn't release. EE took a screwdriver and while prying on the hook of the come along, the screwdriver handle broke causing the wipe to shift and pinched the EE's hand in between the end of the screwdriver and the top of the belt.
Employee was tightening a bolt with a wrench and it fell off and hit EE's hand causing injury.
EE was trying to split a small wedge with an axe. EE held it flat against a set can and struck the wedge. It did not break so they lifted it up with left hand and with the axe in right hand. When they struck the wedge against the can a thumb was under the blade of the axe. EE received 3 stitches.
EE was shoveling at the tail of a surface belt. When throwing a shovel full of material onto the belt, slipped backwards and hit elbow on the cement floor. XRay to elbow was negative. EE returned to work. Elbow has healed but still has pain in shoulder. EE had an MRI done on 8/21/2020 and has began missing work 8/26/2020 due to pain in shoulder.
While using a razor knife, miner cut index finger. Miner was not wearing the required cut resistant gloves.
Crew was unloading a cable spool out of truck bed, EE was using a bar to pry spool over bed rail. The spool shifted and pinched EE's hand between bar and truck fender causing laceration to right hand ring finger.
While breaking up rock with sledge hammer at the Longwall Tailgate drive, a rock fell off the gob plate and landed on employees left foot causing a fracture on top of foot.
EE was hammering in a new bit sleeve on miner drum, a piece of metal came off sleeve and hit EE in left cheek, causing laceration.
Two EEs were tightening bolts on shield trailer with ratchet. EE moved ratchet from bottom-side bolts to top-side bolt changing direction of pull and when other EE pulled on ratchet in same direction as previous bolts, EEs finger was caught in pinch point between ram cylinder and ratchet handle.
Employee was using a pointed bar to operate a track jack. Employee stepped on the point with right foot to push down. The point went through EE's boot and broke the skin.
While working on shuttle car conveyor, employee was struck on the hand by a hammer
Employee was assisting with a belt splice and got hand hit with a hammer causing injury.
Injured miner was assisting with belt repair when finger was struck by a single jack hammer resulting in a fracture to the tip of left index finger.
EE was replacing foot shaft for continuous miner. EE was holding a punch with a punch holder while another miner struck the punch with a hammer. The hammer rebounded off the punch striking EE on the right side of cheek fracturing teeth.
EE was arranging cover plates on stage loader conveyor using a come-a-long when hook came out of anchor and struck EE on chin creating a small laceration and breaking a front tooth.
Employee was trying loosening the locking nuts on a 13" length of all-thread. When the nuts broke loose the employee fell forward striking his hand on the bed of the boom truck. This caused a laceration to the palm side of his right ring finger.
Making a mechanical belt splice. Victim was holding a belt splice tool and another employee was striking the tool with a hammer. Victim moved tool and his partner swung the hammer, striking victim on back of right hand.
On July 15, 2014, the employee saw a Dr. and was not allowed to return to work due to an injury that occurred on July 5, 2014. On July 5, 2014 the employee was shoveling coal from a shuttle car with a broken conveyor chain. While shoveling, the employee struck a conveyor flight with the shovel and strained his left shoulder.
Employee was cutting a cardboard box with a knife when the knife slipped and cut his leg.
