Mostrando 50 de 300EE was operating a drill press in the welding bay to drill holes into a bracket, EE secured it in a vice. While standing to the right, EE held the vice with EE's left hand and operated the press with EE's right. The bit bound, causing the bracket to spin out of the vice which caused EE's left thumb to be caught between the vice and bracket.
EE was pulling cable from a spool at the cable winder. A chain meant to keep the spool centered came loose, allowing it to shift and create a gap near the stops. The EE reached for the knuckle and, while pulling it down, placed their hand into the gap, where it became caught in a pinch point.
Miner was holding a timber while setting a breaker row. Another miner swung the axe to hit the cap piece and the axe glanced off and struck the victim on top of the hard hat.
While using a track jack, the jack bar slipped out of the employee's hand and struck them in the chin causing a laceration.
Drilling with a jack leg drill. EE removed drill pressure and the drill dropped but the drill steel stuck in the roof. EE grabbing the top of the drill to keep it from falling when the drill steel fell from the hole, striking EE's finger. EE was taken to the ER and diagnosed to have sustained a laceration and fracture to the right index finger. It required stitches and a splint.
Miner was stepping down a ladder and received a Jones Fracture to EE's left foot as EE transferred EE's weight.
While cleaning out dust hopper EE removed the guard and was working on unplugging the hopper when EE dropped EE's knife into the hopper. EE had thought that EE had turned off the hydraulic power and hydraulic lever on the agitator. When reaching into the hopper to get EE's knife the agitator came on and caught EE's right arm between the agitator and dust hopper.
Removed a tee on a 2" aqua line. Line sprung back at the injured breaking / chipping a front tooth.
Bolter operator drilling roof. When adding pusher steel, stumbled / slipped and put uncontrolled feed pressure up while right hand was still around the steels EE was marrying. This caused the steels to bend toward the mast, pinching EE's right ring finger between the steels and the feed cylinder. This caused a skin avulsion roughly 3/4" x 3/4" at the tip of the finger.
Employee was helping fix a shuttle car chain. The boom was being lowered and employee saw a shovel. EE reached in to retrieve the shovel and accidentally got EE's other hand pinched by the shuttle car boom causing injury.
While employees were preparing for a belt move they noticed that the belt was not level. While trying to level the belt the belt came off onto the belt structure that the employees were standing on causing employee to come off of the structure and hyper extending a knee.
Employee was helping to hook up a trailer that was in a crosscut due to the highness of the truck they had to lift the tongue of the trailer, So while in the process of lifting the tongue on the compact trailer to hook it onto the truck they strained lower back.
EE was removing the CM pot and while trying to pry the pot free the bar slipped and pinched EE's left hand ring finger between the bar and the CLA tip.
On September 23rd at approximately 9:00pm, EE was monitoring the LW belt, a 20' flapper was noticed traveling into the take-up. EE shut the belt down and locked out the drives and winch. EE then removed the guarding and began cutting out the flapper. While pressing up on the belt, the knife slipped and punctured their left wrist.
The employee was standing on the roof bolter. While moving the hung miner cable by hand, they lost balance falling backwards hitting the drill mast, resulting in a 1-inch laceration on the right side of their neck.
On 08/30/2025 at 14:15 pm a roof fall was discovered in the intersection of crosscut 17 #2 entry in 12 right tailgate. It was determined at that time the roof fall was above the anchor point for the primary roof support. The Msha Hotline was notified of the roof fall at 14:22 pm 08/30/2025
On August 18th at approximately 4:00pm employee was advancing a shield over the shearer in a effort to support rock that was falling out from the roof and face. They lowered the shield adjacent to the one they were operating from, when rock that was approximately 8"x8"x 12" thick struck them on the left hand causing injury to their left hand.
EE was working in the beltline and stated that EE was not feeling good, after sitting down EE collapsed EE's Co-workers checked EE and found no pulse at which time they began CPR, an AED was retrieved. EE was brought outside with CPR in progress. Around 3:50 AM EE was transferred to Sevier County EMS and taken to the Medical Examiner.
Working in a 2-man crew, driller opened rod clamp while chuck was not engaged with 300 ft of drill rod in the horizontal borehole. Water flow from the borehole pushed the free rods backwards into the helper behind the drill rig, striking their right side and causing bruising.
On 6/27/2025 EE was hit by a softball size piece of coal coming off shearer drum. CT scan was negative, and EE returned to work. On 7/5/2025 EE was diagnosed with pleurisy. Workers Comp investigated alleged diagnoses and could not determine if it was related or not. They accepted the claim making the incident reportable on 7/22/2025.
Employee lost the balance of the doors on the forks of the skid steers and got hand caught between the door and headache rack on skid steer causing injury.
Employee was proceeding to advance TG shields from transition as face was advancing and a chunk of coal fell out and hit in the back causing them to accidentally fall forward causing injury to their leg.
Employee was changing out gas bottles and put one on a dolly. It started to fall off and employee tried to catch from falling and accidentally caused injury to fingers.
Employee was carrying 3 drill steels bound together, as they walked through a known slick area, they transitioned between solid floor and mud which caused them to slip and fall with steels in hand, they reached back with right hand to break their fall, crushing right pinky finger.
Tasked with pre-use inspection of a high voltage switch gear. Injured noticed dust on the receptacle and went to wipe with a rag. A capacitive discharge entered EE's hand and body. This resulted in a small burn to EE's left and where the discharge exited and muscle soreness in forearms, shoulder, back.
Employee was cleaning up a belt splice area and employee was lowering the rollout of the belt core. Hook came off end and the core fell which caused injury to their pinky finger
While Climbing off the machine, Employee Placed EE's right hand on the engine cover Resulting in EE's fingers contacting the cooling fan.
While pulling old conveyor belt off and pulling new one on the a frame pulled loose landing on employee's foot, this caused two metatarsal fractures.
While building a Kennedy overcast, injured was kneeling on the solid, left for overcast crossing, over a 1-2' gap above the completed overcast wall below. The injured lost balance and slid between the gap. Injured stuck left arm out to brace against impact, catching the Kennedy decking before sliding approximately 6' between the wall and solid ground below.
EE was trying to hook the knuckle to the pan. The draw bar needed to be coma longed into place. There was no D ring on the draw bar so ee hooked the coma long around the draw bar and then hooked the body of the coma long to the canopy. EE instructed another EE to raise the shield as EE was watching the draw bar the chain broke and struck EE in the mouth/nose.
Employees were loading a butterfly valve onto an underground truck. The valve shifted and accidentally rolled onto employee's hand that was on the bed of the truck. Employee pulled EE's hand out from underneath the valve causing injury to two fingers.
Surface Miner was walking to the forklift. EE stepped over something buried in the snow. When EE did, EE stepped on a belt bottom roller buried in the snow. EE fell back and to the side landing on a roller. This caused broken ribs on the left side.
While pulling a skid out of crosscut EE noticed a bolt that needed to be moved and as EE attempted to move it the skid hit EE's right foot.
While troubleshooting fan shroud on equipment, Employees thumb was struck by fan.
Once underground employee got out of mantrip to check equipment. When getting back in EE shut their thumb in the door.
EE was roof bolting mesh to the roof as EE was raising the ATRS to the roof EE noticed that the mesh needed to be adjusted. EE lowered the mesh and was adjusting the mesh when it kicked upwards causing EE's finger to be caught between the mesh and the mat pulling EE's middle finger nail back.
Unloading Richwood impact clusters off the cookie sheet of an M2 shield hauler. Unloaded two cluster without incident where one man would push and the other would pull. The injured pulled the third cluster into a stationary cluster, smashing EE's finger between the two. EE sustained a small fracture in the tip of EE's right middle finger. The wound required 8 sutures to close.
Employee was pouring drain cleaner and it came out faster than expected and accidentally splashed on EE's arm and then running down under EE's glove causing injury to arm. Employee did not miss work until Dec 26 when EE received additional medical treatment for EE's burn and then was put off work.
EE was hanging line brattice in xc20 between #4 and #5 entry with a insulated hook and when EE lowered EE's hand the end of the hook had caught EE's glove tearing into the back of EE's left hand, between EE's index finger and thumb
Employee was fusing pipe and trying to line up pipe with fuser. The pipe slid forward and accidentally caused employee's finger to get caught between two pipes causing injury
Employee was throwing blocks and smashed EE's finger between two blocks causing injury to EE's finger. Note: This employee works a 4-10 (4 days a week) schedule and this MSHA 7000-1 was submitted within 10 working days of the incident.
IE and co-worker were hanging tubes in #1 entry of the 7 North 3 Left Tailgate, they had hung the tube and IE was holding it up while co-worker was fixing the chains and hook. They switched positions and when they did, the tube rolled causing a latch back to come loose and catching IE's nose which caused a laceration.
Employee was operating the left side of roof bolter, when the canopy got hung up in the roof mesh, they tried lowering the canopy and it would not come down, then they tried lowering the whole boom and it broke loose abruptly striking their hard hat/head against the canopy.
While traveling to check on pumps in 7Rt Bleeder, the roof fall was found.
During the non-producing shift, 12RTG belt went off on remote. Miners walked the belt and found the roof fall on the belt.
While EE was running the miner the face was popping and a piece of coal came off the face and hit EE in the mouth causing one of their front teeth to get chipped.
Employee was operating a fuel truck underground and accidentally hit a rib causing EE's knee to hit against the dash in the cab causing injury.
EE was on Shield #6 and was asked to come into the pan line to help move the mesh out of the way of the bolter. When EE went to go into the panline they lost their balance causing them to fall backwards at which time they hit their head on the relay bar.
Two EE's were retrieving their bolting steels from the Kitchen toolbox. One EE opened the lid and was holding it opened as EE moved towards EE, EE moved around EE, due to someone standing up the table shifted, and the lid swung down striking EE on the right pinky finger resulting in the injury.
