EE was placing shims on a dryer trunnion. EE was working from a ladder and was about 6 feet off the ground. The wrenches EE was using slipped and EE lost EE's balance. EE fell from the ladder and landed on EE's feet, but received a stress fracture in right heel.

EE was cutting piece of skirting with safety knife. Skirting moved and EE cut left forearm with knife resulting in laceration. EE received 3 stitches resulting in MSHA reportablity.
EE was cutting flexible sealing tape into stripes to repair a leaking pipe. The utility knife slipped and lacerated EE's finger.

EE was closing valve under slurry tank using 2' pipe wrench for leverage and felt a pain in back. EE was seen by a doctor and returned to full duty with a follow-up a week later. At follow-up, EE was referred to a physical therapist for an evaluation. On April 30th the EE attended the first of 6 PT sessions, thus constituting MSHA reportability.

Employee and partner were replacing section of cable. Employee was holding cable while his partner was cutting with a hacksaw. His partner cut through the cable and the hacksaw struck employee in the upper lip resulting in a laceration which required stitches.
Employee was using a J-wrench (40 - 50 lbs) to break the lock on the thread between the hammer and the back head on the drill. He was using the supports of the broom as a block to push the wrench against, and was next to the cab when the wrench broke loose under pressure and shot back at him, striking him in the nose. This resulted in a laceration and a fracture to the nose.

LIFTIN G EYES ARE PROVIDED FOR USE WITH A HOIST, BUT WAS NOT USED BY THE EE. THIS INJURY BECAME A LOST TIME INCIDENT ON AUGUST 5, 2002.

EE STATED HE WAS REMOVING DUST PIPING ON #7 ROLL. HE FIRST LOOSENED THE TOP NUTS AND REMOVED THEM. WHILE REMOVING THE LOWER NUTS & BOLTS THE TOP END OF THE DUST PIPE CAME LOOSE, STRIKING EE ON THE TOP OF HIS HEAD. HE WAS WEARING HIS HARD HAT AT THE TIME.
EE STATED HE WAS REMOVING A HAMMER FROM DRILL. WHEN HAMMER SEPARATED FROM DRILL STEEL EE HAD HISHAND UNDERNEATH THE J-WRENCH. THE WRENCH FELL TO THE DRILL TABLE SQUASHING HIS FINGER. EE WAS WE ARING GLOVES AND HAD BEEN PROPERLY TRAINED. THE LAST SAFETY TALK ATTENDED WAS 4-24-99.
