Tightening bolts on jaw crusher and felt knee strain.

Tightening bolts under screen when wrench slipped off and hit back of right hand on piece of metal. Sprained back of hand and broke 3rd finger.

Wheel crusher had a small rock lodged in it. Put a spanner wrench on main shaft to spin backwards. EE pulled on wrench and wrench slipped off. He fell backwards into crusher upright support and hurt his back side. Doctor gave him medication which made him drowsy.

Changing a screen. Prying old screen up and bar slipped and handle struck just above right eye. 1" gash required 3 stitches to close.

Rosk wedged in bottom of hopper door vibrating Grizzly Feeder. One employee was hitting the rock with sledge hammer and a piece of rock broke off and hit another employee in the eye standing adjacent on catwalk causing damage to right eye. Had PPE off to wipe eyes.

EE WAS USING A PRY BAR, ATTEMPTING TO DISLODGE AN OVERSIZED ROCK FROM THE FEEDER, WHEN PRY BAR SLIPPED & EE FELL BACKWARDS OFF OF CATWALK.

WHILE PATHCING A HOLE IN A CONVEYOR BELT AND CUTTING RUBBER PATCH RAZOR BLADE SLIPPED AND CUT INTO LEFT LEG.

HE WAS PULLING ON A WRENCH TO TIGHTEN THE MOTOR ON THE SCREEN DECK AND PULLED HIS LOWER BACK, RUPTING TWO DISKS IN HIS BACK.

EMPLOYEE WAS HOLDING PIN WHEN OTHER WORKER SWUNG A SLEDGE HAMMER AND IN THE PROCESS MISSED THE PIN CAUSING THE SLEDGEHAMMER TO HIT EMPLOYEE RIGHT CHEEK AND EYE AREA.

IT WAS A COLD MORNING 40 DEGREE WITH THE WIND COMMING OUT OF THE SOUTH. HE WAS SPLICING A BELT WHEN HE CUT HIS RIGHT THUMB.

TIGHTENING BOLTS ON CRUSHER. BACKED UP AND HIT SHOULDERS ON BIG PIECE OF ANGLE IRON.

MAN CUT LEFT KNEE WITH A UTILITY KNIFE WAS CUTTING A PIECE OF RUBBER TO PUT ON SCREEN LIP MAKE WHEN THE KNIFE SLIPPED AND CUT KNEE. RECEIVED FOUR STITCHES.
