Employee was rodding out Mill #1 with a pry bar when they smashed their left index finger between the pry bar and an I-beam behind them.

Employee was trying to turn a shaft with a pipe wrench above his head. It broke loose and struck the employee in the head with the handle in the head.

Conditions contributing to the accident involved a combination of inexperience and incorrect usage of tools.

Injured and co-worker were cleaning out the crusher hopper when a slight material slide struck the end of the pry bar forcing the other end into his face.

EE was utilizing a pry bar through an inspection hole on a rock hopper trying to free a jammed rock. When the rock came free, the rock fell against the pry bar in the inspection hole. EE still had his hands around the pry bar. EE's finger was lacerated when the pry bar moved up and into a steel support for the hopper.

WHILE USING A WRENCH TO LOOSEN NUTS ON DISCHARGE VALVE, THE WRENCH SLIPPED CAUSING THE HANDLE OF THE WRENCH TO STRIKE EMPLOYEE IN THE FACE

EMPLOYEE WAS PRYING ON BRASS BUSHING WITH PRY BAR, WHEN PRY BAR SLIPPED AND SMASHED EMPLOYEE FINGER.

CLEANING SOME METAL WITH A WIRE BRUSH AND A PIECE OF METAL FLEW UP INTO HIS RIGHT EYE.

EMPLOYEE WAS SWINGING A SLEDGE HAMMER AND DISLOCATED HIS RIBS.
