An employee struck finger with a hammer while using a drift pin to install a pinion gear onto a shaft. The employee struck the index finger on left hand. The employee's finger suffered a laceration/bruising and a broken finger tip bone during the incident. The employee was placed on restricted duty

While attempting to climb into a front end loader(CAT 966H),an employee slipped, hurting both knees. The employee was climbing in to the front end loader to load the hoppers with barite ore. The employee stated that as he was e entering the cab, his left leg slipped causing his right knee to impact the top of the cab landing.

Two fingers on an employees right hand were pinned between two pieces of a journal assembly as a result of an improper suspension method being used to make repairs to the final assembly.

Employee aggravated a previous back injury while performing regular shift duties. Employee states he did some light shovel before a pre-shift inspection. After the inspection the employee states he sat down in the break room to rest and upon rising from the sitting position experienced a sharp pain in his lower back. The injury has been diagnosed as lower lumbar strain.

A short service employee (SSE) reported being injured while operating a forklift as he performed his warehouse duties. The SSE reported that he drove over a small crack in the concrete pad. The employee reported that the resulting jolt caused him to experience pain to the neck and lower back.

The injury/illness status being reported has been diagnosed as cumulative over a period of time, perhaps 10 years. The employee's job tasks include repetitive work with the use of hand and power tools. This employee appears to have overuse symptoms from either a strain to the elbow and shoulder region or due to bursitis.

While operating a power drill a contractor broke a bone in his right hand. The drill caught metal causing the drill to twist out of his grip. The drill immediately swung around struck the back portion of his right hand fracturing it.

Employee injured his back when he stepped off the last rung of a 966G CAT front end loader ladder. When the employee reached the bottom rung of the ladder he released his hand hold and stepped off with his left leg. He reported feeling a sharp pain on the left side of his back and found it difficult to put pressure on his left leg.

Operator was pushing an air release valve lever in order to de-air railcar braking system when his hand slipped striking his wrist on the valve lever guide (support).

EMPLOYEE STATES THAT WHILE BAGGING, HE TRANSFERRED A 50 POUND BAG OF BARITE FROM THE BAG SCALE TO A PALLET AND FELT A PINCHING SENSATION IN HIS LOW BACK.

Employee was attempting to better the flow of material in the mill hopper by using a long rod to stab the raw ore. He changed the angle of the rod to reach a different spot in the hopper. His left ring finger struck the guard on a belt conveyor. The impact cracked his nail completely and pushed his cuticle and skin up to the root of the nail.

Employee alleges he was shoveling barite tailings from under grizzly/ crusher for about 20-25 minutes when he suddenly felt a sharp pain in his groin.

EE alleges that as he opened the door of the mill office with his left hand, he stepped in and planted his right foot first. As he did, his foot slipped and twisted and his whole body twisted left and he lost his balance and fell with his back against the door.

EMPLOYEE ALLEGES HE WAS PERFORMING HIS NORMAL DUTIES OF BAGGING. HE TOOK HIS HARD HAT OFF FOR A MOMENT AND THEN PLACED IT BACK ON HIS HEAD. SHORTLY AFTER HE FELT A STING ON THE BACK OF HIS HEAD.

INJURED WORKER & OTHERS WERE REPLACING A TAIL PULLEY ON A BELT CONVEYOR. A CO-WORKER LIFTED THE TAIL PULLEY WITH THE FORKLIFT AND DROVE THE FORKLIFT INTO POSITION & LOWERED THE PULLEY. INJURED WORKER WAS HOLDING THE PULLEY WHILE FORKLIFT WAS BACKED OFF TO PREVENT IT FROM SLIDING BACK. WEIGHT OF PULLEY SHIFTED; PULLEY ROLLED AND CAUGHT FINGER.

EE WAS BREAKING APART A CARDBOARD AND WOOD CRATE WITH HIS BARE HANDS. AS HE APPLIED PRESSURE WITH HIS HAND AND FOOT TO BREAK A BOARD IT SUDDENLY BROKE AND SNAPPED ON HIS RIGHT HAND NEAR HIS TH UMB CAUSING EXTREME PAIN.
