Employee was replacing skirt board on stacker belt. Wrench slipped and impacted finger. First aid completed. Employee reported Jan 20 finger was infected and went to clinic. Clinic provided antibiotic/meds; employee released to return on Jan 25.

Employee was removing a tire from the rim using a tire tool. When tire broke loose from the rim the tire tool hit employee on the left cheek, approximately 2" below the eye. The result was a contusion and laceration on his cheek.
Miner using a socket wrench to adjust belt. Tool slipped and miner struck elbow on frame. Miner did not want to go to the doctor on date of incident. Elbow soreness continued & he went to the doctor on March 3, 2011. Medication prescribed. No lost time or restriction.
EE was trimming belt wiper. Knife slipped and cut his left thigh.
Used pocket knife to cut cable tie. When knife passed through cable tie, he stabbed left hand.
Cleaning material chute with punch bar. Hit finger on side of chute.
EE was assisting his foreman remove a damaged spray baiz when the pipe wrench being used to remove the pipe fell and hit employee above his upper lip on the left side of his mouth causing laceration just above his lip.
HE WAS HELPING SET A MUD PUMP INTO THE POND AND THE CHAIN LINK BROKE. THE BROKEN LINK FLEW ABOUT 20' AND HIT HIM IN THE FACE.
USING A WRENCH TO TIGHTEN A BOLT ON THE SAND PUMP.
USING PRY BAR TO ALIGN BOLT HOLES STRAINED RIGHT SHOULDER.
HE WAS CUTTING A PIECE OF BELT WHEN THE KNIFE HE WAS USING SLIPPED AND CUT HIS LEG.
REPLACING TEETH ON EXCAVATOR BUCKET. HAMMER WAS USED TO UNWEDGE TEETH. METAL SLIVER CAME OFF TEETH STRIKING EE IN STOMACH.
EUIPMENT NORMALLY USED TO LIFT SCREENS (SHAKER SCREENS. APPROX. 60 LBS) TO SECOND DECK OF PLANT WAS OUT OF ORDER. EMPLOYEE WAS ATTEMPTING TO LIFT SCREENS FROM GROUND TO SECOND BY USING ROPE TO PULL SCREEN UP. IN THE PROCESS OF LIFTING SCREENS EMPLOYEE STRAINED HIS LOWER ABDOMEN ON RT. SIDE.
EMPLOYEE ASSISTING REPAIR TO GRAVEL RETAINING WALL AT PLANT. AS ANOTHER EMPLOYEE NAILED BOARD INTO PLACE A ROCK HIT I.NAME IN EYE, CUTTING HIS EYE CAUSING A CORNEAL ABRASION OF HIS RIGHT EYE.
EMPLOYEE WAS ST9IKING A CONVEYOR FRAME WITH HAMMER THE HAMMER MISSED THE FRAME TEH MOMENTUM CARRIED HIS HANDS AND THE HANDLE AGAINST THE CONVEYOR FRAME PINCHING FINGER

HITTING ROCK WITH HAMMER AND CHIIP BRUISED EYE.RUBBED EYE AT NIGHT CAUSING SCRATCH ON PUPIL.
