Mostrando los 28Removing small center shaft on loader with come-along. Shaft got hung up on bolt and slid to strike forehead. Cut on forehead, cleaned and placed band-aid on cut. Sent to care center to just have them evaluate. They place two stitches and returned EE to work with no restrictions.
Employee was removing a bolt on a 330CL Excavator to change out attachments and when the bolt broke loose his left thumb was smashed between the wrench and the boom. His left thumb began to swell after a while. A co-worker and the employee talked and decided to be safe to have his left thumb looked at.
During disassembly of transmission pump, a hammer and punch were used to remove the shaft from the housing. On the third strike a chip flew off the punch and struck the upper lip. Employee requested a band-aid and declined a trip to the doctor.
Employee was assisting with the assembly of the upper frame of a Pegson cone crusher. A driving maul tool and 20 pound sledge were being used to help drive the concave wedges in place as a metal chip struck the employee in the left hand. The employee was taken to a local healthcare facility and stitches were required to treat lacerations in his left index & middle fingers.
EE was working on vehicle 22-311. While working on suspension he heard a popping type sound. He immediately pulled his left arm away from bolts being tightened. The leaf springs may have shifted. Springs were not under load.
Prying on a bearing rack with a heelbar inside a gear box. The heelbar slipped which caused my right forearm to hit the gear box housing. The edge of the housing was fairly sharp.
While driving a stake into a wattle, employee's hand & stake moved causing sledge hammer to hit his left hand.

EE WAS SHOVELING OUT SAND THAT HAD BUILT UP INSIDE THE GRIZZLEY TUNNEL. HE STOOD DOWN UNDER A BELT AND CAUGHT HIS LEFT SHOULDER ON A STEEL BAR SUPPORT.
LIFTING GENERATOR HALF AND SHACKEL SLIPPED AND LET PART FALL ABOUT 4" AND SMASHED 2 FINGERS ON LEFT HAND.

PULLING CABLE ON DRAGLINE USING A 3 FOOT PRY BAR TO PRY CABLE FROM SHEAVE.
EE WAS ADJUSTING A ROLLER WITH A PIECE OF ANGLE BAR. THE BAR SLIPPED AND PUNCTURED HIS RIGHT INDEX FINGER.
HOLDING PUNCH & HAMMER GLANCED OFF PUNCH AND HIT HAND.
EE WAS USING A FENCE POST HAMMER TO INSTALL A GRUND ROD AT A PORTABLE FEEDER. WHILE DRIVING THE GROUND ROD, HE LIFTED THE HAMMER TOO HIGH AND ON THE NEXT STROKE MISSED THE ROD AND LET THE HAMM ER FALL ON HIS FOOT.
LABORER WAS CUTTING FLASHING WITH A SHEETROCK KNIFE. THE KNIFE SLIPPED AND CUT HIS LEFT LEG ABOVE KNEE HE RECEIVED THREE STITCHES.

CHANGING LINERS. PIECE OF METAL HIT WORKER IN LEG.
A ROCK GOT STUCK IN JAW ON MAIN PLANT AND EMPLOYEE WAS USING A PRY BAR AND ANOTHER ROCK CAME DOWN AND HIT THE BAR, AND HIS FINGERS GOT SMASHED BETWEEN THE BAR AND THE SIDE OF METAL PLATE ON IN SIDE JAW.
