Employee was repairing a Raymond Roller Mill Journal assembly. EE was lifting the part with a sling and forklift to position it on a work table. EE attempted to reposition the part before placing on the table when the assembly came loose and a portion fell crushing EE's right thumb.

Mostrando los 33Employee was loosening a bold on a Derrick Screen system during a screen change. The nut gave way while EE was using a wrench and the employee felt a "pop" in EE's shoulder.
Employee was cleaning the floor with a broom when EE's hand contacted a cabinet that had a sharp object sticking out. EE had a small laceration on EE's right pointer finger knuckle that required stitches.
IE was cutting open Supersack of material, IE rested supersack on guard & cut open bag w/ knife, plastic liner inside SS slid out & pinched IE's right hand between liner and filter grate.
IE was attempting to unclog finish bin by striking with hammer. After several hits, hammer glanced off bin and IE's hand holding hammer struck metal support frame causing a bruise to right hand. IE was taken to health clinic and x-rays were negative for broken bones. IE was given lifting and grasping restrictions requiring IE to be placed on light duty job.
Employee reported right shoulder pain while breaking concrete with a sledgehammer. EE documented that EE had a previous injury to the shoulder. EE did not seek medical treatment until 5/11/23 at which time EE reported treatment beyond normal first aid. Diagnosis is "joint pain". No lost time, no restrictions to date, treatment is ongoing.
EMPLOYEE WAS REMOVING ICE FROM A CRUSHER DISCHARGE CHUTE USING A DIGGING BAR WHEN EE'S THUMB STRUCK A METAL BEAM.
Was removing a pipe nipple from Jet Mill 2 cooler system with a pipe wrench using my legs to hold me in place.
employee was opening a unloading door on the bottom side of a hopper car. EE was using the tanker mans bar and when EE went to open it, the position EE was in caught a pinch in EE's back.
Employee was helping to position a gear box onto a screw conveyor when the gear box fell from the chain fall it was being lifted by contacting the employee on the left thigh.
Employee was cutting a gum rubber boot with a folding knife and no cut gloves on. The employee was cutting in a upward motion and knife blade turned in EE's hand, at this point the knife blade closed on EE's finger.
Employee was working at Ball mill #1 tightening the door on the top. When EE went to turn the bar, EE "felt a pop and a tear in the groin area". EE was taken by ambulance to the ER. released "off work" until seen by a surgeon. EE's first appointment is on 4-12-19.
Cutting bottom out of pint cup to tape together
Employee was using a sledge hammer to put 3-4" anchoring bolts into a concrete floor. While swinging the hammer the employee injured their left shoulder. This resulted In a left shoulder sprain and the employee is estimated to be out of work for 2 weeks per the doctors request.
Was cutting a bag of material on Scr. 1 pad/pit
Employee was using a dead blow hammer to install pipe in a trench. Employee attempted to swing the hammer to strike a 2x4 piece of wood. The hammer deflected off the side wall of the trench striking the employee in the left hand fracturing their middle finger.
Production worker was cutting a bulk back and cut right index finger. EE left work early to seek a doctors opinion, they glued finger closed and bandaged it. EE returned to work for the next shift.
Cutting shot coke sacks into loader guys bucket with co-worker; co-worker was on L side and I was on Right; trying to get sack into bucket; was steadying left hand on loader bucket below sack & then we were both cutting sides & it fell down - weight of sack appr. 2700 lbs. material fell on left hand & couldn't pull it out. Called co-worker over & dug material & yanked my hand out.
Employee was using a hook bar to reposition a large piece of Graphite that was jammed in the Single Roll Crusher. It did not move on his first attempt. When he pulled harder on the bar he felt a sharp pain in his groin area.
EE was tightening a bolt on the end of the rotary dryer. Co-worker was holding the nut in place with a large wrench to assist EE in tightening the bolt. The large wrench slipped off the nut and co-worker lost control of the wrench. It struck EE in the upper lip breaking his tooth.
EE was sampling sacks of material with a thief sampler. When he turned the thief shut. The skin on his left index finger got caught in the pinch point of one of the openings of the thief which resulted in a hunk of skin being cut off.
EE was preparing to put a stack of 4"x21' steel plates in storage. When EE cut the banding the plates sprung loose and fell into a puddle which caused a splatter and some of the fine material and rust slashed in his face. Later in shift his right eye became irritated.
EE was using a utility knife to cut open the bottom of a sack. He was pulling the knife toward himself. When the knife completed cutting the sack, he lost control of it and it stabbed him in the right calf muscle.
Employee was cutting the bottom out of a supersack of raw material. The bag bulged out when cut & pinned his left arm & shoulder between the hopper and bottom of sack. He then jerked his arm out of the pinch point.
Employee was placing .sprocket on the elevator shaft on the roller mill. His knee came in contact the back elevator's door flange. This caused pain and swelling in the right knee.
Employee was welding a piece of steel into place at the end of the Rotary Dryer. He tried to hit the steel with a hammer to position it properly. When he was swinging the hammer downward it hit a piece of steel causing it to glance off the steel into his index finger on his left hand.
While attempting to open a thief sampler, the employee's pinky finger slipped into the slot on the sampler. While sliding it apart, it stuck and when the employee pulled harder, the tool sliced off the tip (1 centimeter) of his pinky finger
EE WAS CUTTING LIDS OFF OF PLASTIC 5 GAL. BUCKETS WITH A BOX CUTTER. HE CUT HIS HAND WITH BOX CUTTER. (CUT 2 TENDONS IN HIS LFT. HAND).
EE WAS SHOVELING UP GRAPHITE IN ACM ROOM WHEN HEGOT GRAPHITE DUST IN HIS EYES CAUSING THEM TO BECOME IRRITATED
EE WAS USING CRESCENT WRENCH TO TIGHTEN A NUT & BOLT & EE STRAINED ELBOW
EE STUCK A METAL BAR THROUGH A GRATE TO LOOSEN WET MATERIAL. THE BAR GET CAUGHT ON THE SCREW CONVEYOR AND SLID ALONG THE EDGE OF THE BIN AND HIT HIM ON THE RIGHT SIDE OF THE HEAD.
IT BECAME NECESSARY TO REMOVE/RELOCATE 26" PVC WATERLINE FOR TRUCK ROUTE SO SAND COULD BE REMOVED FROM CEILING PILE. THE PIPE SNAPPED LIKE A BOWAND IT'S ENDS CAUGHT MY FINGER. 9 STITCHES REQUI RED.
EMPLOYEE WAS CHANGING BLADE IN UTILITY KNIFE ANDCUT THE INDEX FINGER ON HIS RIGHT HAND.
