Employee was installing drive belts on CV22. EE was adjusting the motor mounts when EE's right index finger tip was pinched between the wrench and the mount.

Maintenance crew was removing pinion sheeve for crusher repairs. While removing off the shaft, Tech's Left index finger was struck by assisting Tech with 2 lb hammer.

Employee was under the crusher pulling on wrench while removing the lube line - when they felt a burning in their stomach.

Employee was rambolting a discharge flange on a 12" pit pump - employee was tightening up bolts with an impact in right hand and a combination wrench in left hand & while applying pressure to the bolt employee heard a pop and shoulder began to hurt

Employee was installing a side panel on top deck - striking panel with the sledge hammer it bounced back and hit employee on the cheek -

Employee was installing a skirt rubber with a sledge hammer - hammer slipped out of employee's hand and jammed into the conveyor frame

employee was using the come-a-long - that was attached to the feeder - cable broke causing it to hit ee on shoulder, back, wrist -

TRYING TO OPEN A RAIL SWITCH/SWITCH WAS NOT WORKING AND HAD TO USE A PULLEY BAR AND A SLEDGE HAMMER TO MOVE TRACK OVER.

Employee was swinging a sledge hammer while doing repairs to a crusher

Trying to put drive belts on conveyor #5. The conveyor belt rolled backwards and the wrench mashed my finger against the guard.

2 employees trying to loosen on motor base of crusher. Employee was holding the wrench on a bolt and co-worker was hitting the wrench with the sledge hammer. The wrench slipped, employee fell forward and pinched his finger between the wrench and the motor base. His finger is broke

Am EE was tightening the 3/4 ton come-a-long on a loaded train door to close it. This caused too much tension on the chain. The chain broke and something hit IE in the leg causing a small cut and bruising. Job duties were restricted for healing process.

Employee was trying to close a gate on a bin. He was using a wrench. His hand slipped off the wrench and fell between the gate and the bin. The weather was in the 20's with the wind chill factor. He was not wearing his gloves. He had cut his pinky from the knuckle to the end - requiring a few stitches. He has not missed any work.

Employee was trying to knock the manganese out of the crusher with a sledge hammer. The hammer head came off and fell on his finger. Busting his finger. Finger required stitches. Employee has not missed any days.

employee was swinging sledge hammer to know out anvils out of anvil ring. repetitive motion caused shoulder strain.

The employee was using a wrench to tighten a bolt on a Hitachi excavator and the wrench slipped off and hit him in the lip.

Employee was trying to loosen a bolt on a piece of equipment JCD 309 haul truck. He was using all of his body weight. The wrench slipped of the bolt causing him to fall against the equipment. His elbow hit the bracket on the equipment.

EE (injured)was spraying lubricant on screen deck while installing new screen, another EE was taping in the new screen with a sledge hammer. He missed the screen and hit EE in the left side of face above the left eye.

Employee reported soreness in shoulder after swinging 16 lb sledge hammer. Employee states shoulder has been hurting for some time (a year or more) but is hurting worse now.

Plant operator was cutting a piece of skirt board rubber with a knife. The knife came out of the rubber and cut his thumb. Knife sliced the top of his thumb requiring minor surgery to repair the tendon.

Employee breaking down crusher, swinging sledge hammer, pulled shoulder. Employee has not missed any time but is still having trouble, may end up missing time.

Putting bushing in crusher with wood and hammers. By hitting the wood with the hammer caused vibration which inflammed a 3 yr old injury to his wrist.

He pulled a muscle in right arm while using a hammer to tighten bolts.

It was raining. Employee was tightening a bolt on the screen deck. He slipped and fell on the step.

Employee was tightening a drill chuck with pipe wrench. Wrench slipped causing a laceraton to his right arm. 2 cm laceration required stitches.

EE WAS CLEANING PIPE OF DEBRIS BY BEATING ON PIPE WITH HAMMER - HIT FINGER WITH HAMMER.

The chute was stopped up. Employee took a hammer and hit the chute. It came back on his left hand - jamming his middle finger. This caused a sprain finger.

CREW WAS CLEANING TOP SHELF OF CRUSHER WITH WIRE BRUSHES PIECE OF WIRE CAME OUT OF BRUSH ANDHIT HIM IN HE EYE.

HE WAS INSIDE THE ROCK CRUSHER TRYING TO UNSTOP IT AND ROCK ROLLED DOWN INTO THE CRUSHER AND MASED HIS LEFT INDEX FINGER ON ONE OF THE HAMMERS.

STRUCK RIGHT FOOT WITH SLEDGEHAMMER.

ANOTHER EMPLOYEE WAS DRIVING A PIN OUT OF A BUCKET, A PIECE OF METAL SHEARED OFF AND HIT HIS ARM. EE WAS STANDING ABOUUT 10 FEET AWAY.

HE WAS HITTING DRAGLINE TOOTH WITH HAMMER TRYING TO KNOCK TOOTH OFF OF SHANK. A PIECE OF TOOTH SHEARED OFF AND WENT INTO LEFT ARM. THE PIECE OF STEEL HAD TO BE SURGICALLY REMOVED.

EE WAS USING SCREWDRIVER AND A PIECE OF IT BROKE OFF HITTING HIM IN THE EYE.

HITTING BACK OUT HAMMER WITH SLEDGE HAMMER TO KNOCK WEDGE OUT OF ROPE SOCKET TO CHANGE DRAG CABLE A PIECE OF STEEL SHEARED OFF OF BACK OUT HAMMER AND ENTERED TOP OF HIS HAND.

I NAME WAS ASSISTING IN KNOCKING LOCK IN TEETH OF CRUSHER ANDERSON WAS HOLDING A 4# BNO HAMMER WHEN 16# SLEDGE HAMMER HIT LOCK & FLIPPED UP HITTING HIM IN THE LEFT EYE HITTING SAFETY GLASSES B REAKING THEM & BRUISING EYE & CHEEK

EMPL WAS ATTEMPTING TO LOOSEN BOLT WHEN WRENCH THAT HE WAS USING SLIPPED OFF HAND STRIKING ANOTHER PART OF EQUIPMENT CAUSING FRACTURE TO RIGHT HAND.

EMPLOYEE WAS TR2ING TO DISLODGE ROCK IN CRUSHER HE WAS USING A SLEDGEHAMMER & HIT HIS KNEE
