Employee was changing screens on the lower deck of the north primary screen in the Wash Plant. Employee used a razor utility knife to cut a rubber gasket down to size. The blade slipped off of the rubber and cut through the glove and the employee's right hand. Employee was taken to the emergency room and received stitches.

A bearing was being tapped into place on a head pulley shaft using a hammer and a chisel when a small portion of the chisel cutting edge chipped off striking an employee 6 feet away in the right chest. The piece was removed requiring two stitches to close the wound.

While prying apart a crate, a board broke striking the associate in the face. Splitting EE's lip open.

Employee was stripping wire for main feed when his hand slipped and the knife struck his thumb resulting in two stitches.

Employee was using a pocket knife to cut a zip tie. While doing so, the knife slipped and punctured his hand. This resulted in stitches from the local emergency room. He returned to work, but was instructed to follow up with a hand surgeon for possible nerve issues. At this time, he is on restricted duty, but there has not been any lost time.

While cutting up cardboard boxes to place in the recycling bin the associate cut his left hand with a utility knife. The resulting injury required three stitches to close the wound.

While cutting banding off of a pallet of bags with a utility knife the employee's hand slipped causing a cut requiring three stitches on his left hand fore finger.

Associate was shoveling around skidster that was stuck when he got stuck in the clay around him and cut himself on the skidster. Associate received stitches in the arm.

Associate was helping out at the washplant, raking fines at the dewatering screen when the rake got caught and yanked out of the associates hand.

Associate was cutting belt off fan with knife towards himself and knife slipped cutting hand. Associate was not wearing gloves at the time of the incident. First aid was administered on site and associate went to hospital receiving 6 stitches.

The associate was using a pipe wrench to assemble a 2.5" water line for the plant water truck. When he applied pressure to the wrench it slipped off the threads causing his fingers to strike the back of the truck while still holding the wrench resulting in a fracture to his finger and lacerations.

Employee was performing maintenance on the dredge. He was tightening loose bolts on the frame structure of the dredge ladder. He was holding wrench on the bolt when it slipped off pinching & cutting the end of his first finger right hand between the wrench and angle structure.

The employee was using a spud bar attempting to dislodge jammed rocks from primary crusher jaws, when a large rock fell from top of pile, striking the bar causing it to swing and hit him in the right side of head. The employee was diagnosed with a slight fracture to the lower jaw bone.

Closing R.R car doors with bar the door stuck, when door released, the bar went down fast.

The injured ee was opening a rail car door. He pulled on a large steel bar he was using & felt a pain in the groin area. He went home. Went to ER & discovered a heart flutter. Then they discovered 2 pre-existing hernias. They also noticed a twisted testicle. Surgery was performed to correct everything. Notified on 11/6/06 of surgery on the testicle & is charged as a LTA.

The employee was cutting out a plastic backing plate for a wet location electrical box. He held the plate in his left hand while using a utility knife with his right hand to cut out the tabs. The knife cut through the plastic tab and continued down and away from his body but contacted the tip of his left index finger all in one motion. The laceration needed four stitches to close.

The injured ee was working on a cooler. He was taking off the guide rings for the timing belt sheave. He put a screw driver in behind the ring. The screw driver sliped and cut finger on the guide ring. He recieved stitches to his left hand-ring finger.

CUTTING RUBBER BELTING ON SHOP FLOOR, MISSED FIRST CUT MARK AND CUT LEFT INDEX FINGER.

EE was installing a screen frame in the wet mill using an open end wrench. The wrench slipped off of the bolt head and EE's hand struck the edge of the frame creating a laceration wound that required eight stitches to close.

THIS ACCIDENT OCCURRED ON 4-15-03. WAS NOTIFIED ON 4-21-03 THAT THIS ACCIDENT OCCURRED. THE INDIVIDUAL BELIEVES IT HAPPENED WHILE BRUSHING/SCRUBING TESTING SCREENS VIGOROUSLY. SHE EXPERIENCED SEVERE CRAMPING WHILE PERFORMING THIS TASK. SHE DID SOMETHING TO HER MUSCLE OR NERVE IN HER LEFT UPPER BACK. SHE IS ON RESTRICTED DUTY.

EE WAS DOING SOME CLEAN-UP WORK, BREAKING UPART SOME PALLETS WITH A SLEDGE HAMMER. HE HIT A PALLET BOARD THAT BROKE AND FLEW UP, HITTING HIM IN THE MOUTH AND CUTTING HIS LOWER LIP.

THE INJURED WAS ON A BAGGING PLACING BAGS ON SPOUTS. HE STARTED A CONVEYOR CHAIN AND IT BECAME STUCK. HE SHUT THE POWER OFF AND USED A CROW BAR TO BACK THE CONVEYOR UP THE BAR SLIPPED. HIS HAN D FELL INTO A SHARP PBJECT AND CUT INTO THE TOP SIDE BEHIND THE LITTLE FINGER. HE RECEIVED 3 STITCHES AND WAS ON RESTRICTED DUTY.

EE WAS PULLING ON A 3/4" DRIVE TORQUE WRENCH TIGHTENING THE REAR WHEEL LUG NUTS ON 769-C CAT HAUL TRUCK AND FELT SOMETHING POP IN HIS BACK.

EMPLOYEE WAS REMOVING A LINER FROM A FEEDER PAN.DURING THIS MAINTENANCE ACTIVITY, HIS PRYING TOOL SLIPPED, CAUSING HIS LEFT WRIST TO HIT THE BOTTOM OF THE FEED BIN. HE CUT HIS LEFT WRIST.

THE EE WAS SHOVELING SAND-HE WAS USING ONLY HIS LEFT HAND TO PUSH THE SHOVEL INTO A PILE OF SAND- THEN PULL THE SHOVEL BACK WHEN IT WAS FULL AND DISPOSE OF IT. ON THE 3RD PUSH ON THE SHOVEL HE RAMMED HIS LEFT HAND INTO A GUARD WHICH HAD A SHARP EDGE CULLING HIS LEFT HAND BETWEEN THE THUMB AND INDEX FINGER HE RECEIVED 5 STITCHES AND A TETANUS SHOT.

EE WAS HELPING TO CHANGE CONVEYOR IDLERS, DURINGTHIS MAINTENANCE ACTIVITY METAL WENT INTO HIS EYE. HE WAS USING A HAMMER TO LOOSEN THE IDLERS.

EE WAS PRYING ON THE DRIVE SHAFT OF A 980G WHEELLOADER WITH A 5 LONG BAR AND PINCHED THE 5TH FINGER OF HIS LEFT HAND BETWEEN BAR AND FRAME OF LOADER. EE RECD 1 SUTURE TO CLOSE THE LACERATION.

WHILE EE WAS USING A HAMMER TO UMPLUG THE PIPE ON THE HYDROSEEDER, THE HAMMER SLIPPED & HIT EE'S RIGHT ARM [WRIST].

EMPLOYEE WAS WRENCHING ON TURN BUCKLE TO REMOVE OLD BELTS ON ROTARY DRYER DRIVE. THE WRENCH SLIPPED OFF TURN BUCKEL. EMPLOYEE TWISTED WHEN WRENCH CAME OFF TURN BUCKEL, STRAINING HIS LOWER BACK . SUPERVISOR NOTICED EMPLOYEE WAS HURT WHEN HE HANDED HIM THE NEW BELTS AND HE STOOD UP AND SAID HIS BACK WAS HURTING. EMPLOYEE THOUGHT HE PULLED SOMETHING.

EE WAS HELPING UNPLUG A 6 " DIAMETER HOSE WHICH CARRIES GRAVEL FROM A SCREEN TO A CONVEYOR BELT. THE HOSE WAS PLUGGED WITH DEBRIS. EE WAS RAMMING A 10' LONG PIECE OF 1/2" CONDUIT DOWN THE HOSE , TRYING TO FREE THE DEBRIS. HE FELT A BURNING SENSATION IN HIS LEFT SHOULDER, DIAGNOSED AS A SPRAIN/STRAIN.

INDIVIDUAL WAS USING A SHOVEL TO POKE A RUBBER SHIELD IN PLACE WHEN THE SHOVEL MISSED THE RUBBER SHILED. HIS LEFT HAND CAME DOWN AND LANDED ON EXPANDED METAL. HE CUT HIS LEFT THUMB AND RECEIVE D 7 STITCHES AS OF TODAYS DATE STITCHES HAVE BEEN REMOVED AND THERE WERE NO RESTRICTIONS.

THE EE WAS CLOSING THE BOTTOM DOOR ON A COVD HOPPER RAIL ROAD CAR. THE DOOR NEEDED TO BE CLOSED TO INSERT THE LOCKOUT & SEAL. HE WAS USING 80-100 LB BAR TO CLOSE THE DOOR ON THE RAIL CAR. WHEN PRESSURE WAS APPLIED TO THE BAR. THE DOOR CLOSED& WHEN PRESSURE WAS RELEASED THE BAR CAME UP & PINCHED HIS HAND BETWEEN CAR & BAR. HE WAS ON RESTRICTED DUTY 3/14/97. ON 3/20/97 WENT TO WORK

EE WAS PULLING PACKING FROM A PUMP WHEN A TOOL HE WAS USING BROKE CAUSING HIM TO TWIST HIS BACK.

EE WAS CHANGING A SCREEN ON THE PEBBLE PLANT. USING A BAR TO LOOSEN THE SCREEN WHEN THE BAR SLIPPED AND CUT EE BETWEEN THE INDEX FINGER AND THUMB OF HIS RIGHT HAND. IT TOOK 3 STITCHES TO CLO SE THE WOUND.

TWO EE WORKING ON A CONVEYOR ONE ON EACH SIDE. ONE EE WAS PULLING ON SLACK END OF CHAIN FROM COME A LONG TO CLEAR IT OF CONVEYOR FRAME. HE W. PULLED CHAIN FROM ABOVE WHICH FORCED INJURED HAND INTO THE END OF THE SHAFT CREATING A DEEP LACERATION WHICH REQUIRED 3 STITCHES. THE NEXT DAY HE HAD BLOOD POISONING AND WAS HOSPTIAL FOR 4 DAYS.

EMPLOYEE WAS PRYING & LIFTING A PIPE WITH PRY BAR.

WHILE EE WAS CLOSING BOTTOM DOOR ON RAILROAD CAR STEEL BAR USED FOR CLOSING DOORS SLIPPED STRIKING EE IN THE RIGHT KNEE.

EE WAS REPLACING FEEDER WEAR PLATES AT JEFFREY SAND DRIVER WHEN ACCIDENT OCCURRED EE WAS APPLYING DOWNWARD FORCE ON A SCREWDRIVER HANDLE USING HIS ABDOMINAL MUSCLES WHEN HE SUSTAINED A HERNIA TO THE STAMCH MUSCLE IN THE NAVEL AREA.

WHILE EMPLOYEE WAS HITTING LOCK OF BOTTOM DOOR OF RAILCAR WITH PRYBAR, EMPLOYEE FELT PAIN IN WRIST .

EMPLOYEE WAS OPENING THE BOTTOM DOORS ON RAIL CARS. THE INJURED PLACED A LONG STEEL BAR IN THE KNUCKLE TO OPEN THE DOOR. THE DOOR WAS FROZEN SHUT-ALL THE INJUREDS WEIGHT WAS ON THE BAR. THE BA R SLIPPED OUT OF THE KNUCKLE. THE INJURED FELL TO THE R.R. TRACKS-SMASHING HIS FINGER BETWEEN THE BAR & RAILROAD TRACKS. THE GROUND WAS FROZEN & LOTS OF ICE.

CUT L MIDDLE FINGER WHILE INSTALLING PANEL

EMPLOYEE WAS USING A COMBINATION WRENCH TO LOOSEN BOLT. THE OPEN END SLIPPED AND THE EMPLOYEE'S THUMB WAS PINCHED BETWEEN THE WRENCH AND TRUNION BASE.

EMPLOYEE WAS STRIPPING WIRE WITH A POCKET KNIFE AND SLIPPED AND CUT LEFT HAND.

INDIVIDUAL WAS CUTTING SHIM STOCK WITH PAIR OF TIN SNIPS. TIN SNIPS SLID OFF SHIM STOCK & SHIM STOCK SLID TO RIGHT INDEX FINGER & CUT IT AT SECOND JOINT. INDIVIDUAL RECEIVED 4 STITCHES TO RIGH T INDEX FINGER. NO RESTRICTED DUTY. WOUND WAS TOBE KEPT CLEAN & DRY STITCH REMOVAL IN 10 DAYS.

MECHANIC WAS IN AN AWKWARD POSITION BENT OVER WHILE PULLING HARD ON WRENCHES TO LOOSEN MOTOR BOLTS, AND FELT A STRAIN ON HIS BACK. HE WENT TO CHIROPRACTOR FOR TREATMENT.

EMPLOYEE WAS PUSHING ON PIPE WRENCH TO LOOSEN PIPE JOINTS. INJURED BACK BY REMOVING OLD PIPE AT THE DRYER TRAPS.
