Employee was changing screens on the lower deck of the north primary screen in the Wash Plant. Employee used a razor utility knife to cut a rubber gasket down to size. The blade slipped off of the rubber and cut through the glove and the employee's right hand. Employee was taken to the emergency room and received stitches.

Mostrando los 23A bearing was being tapped into place on a head pulley shaft using a hammer and a chisel when a small portion of the chisel cutting edge chipped off striking an employee 6 feet away in the right chest. The piece was removed requiring two stitches to close the wound.
While prying apart a crate, a board broke striking the associate in the face. Splitting EE's lip open.
Employee was stripping wire for main feed when his hand slipped and the knife struck his thumb resulting in two stitches.
Employee was using a pocket knife to cut a zip tie. While doing so, the knife slipped and punctured his hand. This resulted in stitches from the local emergency room. He returned to work, but was instructed to follow up with a hand surgeon for possible nerve issues. At this time, he is on restricted duty, but there has not been any lost time.
While cutting up cardboard boxes to place in the recycling bin the associate cut his left hand with a utility knife. The resulting injury required three stitches to close the wound.
While cutting banding off of a pallet of bags with a utility knife the employee's hand slipped causing a cut requiring three stitches on his left hand fore finger.
Associate was shoveling around skidster that was stuck when he got stuck in the clay around him and cut himself on the skidster. Associate received stitches in the arm.
Associate was helping out at the washplant, raking fines at the dewatering screen when the rake got caught and yanked out of the associates hand.
Associate was cutting belt off fan with knife towards himself and knife slipped cutting hand. Associate was not wearing gloves at the time of the incident. First aid was administered on site and associate went to hospital receiving 6 stitches.
The associate was using a pipe wrench to assemble a 2.5" water line for the plant water truck. When he applied pressure to the wrench it slipped off the threads causing his fingers to strike the back of the truck while still holding the wrench resulting in a fracture to his finger and lacerations.
Employee was performing maintenance on the dredge. He was tightening loose bolts on the frame structure of the dredge ladder. He was holding wrench on the bolt when it slipped off pinching & cutting the end of his first finger right hand between the wrench and angle structure.
The employee was using a spud bar attempting to dislodge jammed rocks from primary crusher jaws, when a large rock fell from top of pile, striking the bar causing it to swing and hit him in the right side of head. The employee was diagnosed with a slight fracture to the lower jaw bone.
Closing R.R car doors with bar the door stuck, when door released, the bar went down fast.
The injured ee was opening a rail car door. He pulled on a large steel bar he was using & felt a pain in the groin area. He went home. Went to ER & discovered a heart flutter. Then they discovered 2 pre-existing hernias. They also noticed a twisted testicle. Surgery was performed to correct everything. Notified on 11/6/06 of surgery on the testicle & is charged as a LTA.
The employee was cutting out a plastic backing plate for a wet location electrical box. He held the plate in his left hand while using a utility knife with his right hand to cut out the tabs. The knife cut through the plastic tab and continued down and away from his body but contacted the tip of his left index finger all in one motion. The laceration needed four stitches to close.
The injured ee was working on a cooler. He was taking off the guide rings for the timing belt sheave. He put a screw driver in behind the ring. The screw driver sliped and cut finger on the guide ring. He recieved stitches to his left hand-ring finger.
CUTTING RUBBER BELTING ON SHOP FLOOR, MISSED FIRST CUT MARK AND CUT LEFT INDEX FINGER.
EE was installing a screen frame in the wet mill using an open end wrench. The wrench slipped off of the bolt head and EE's hand struck the edge of the frame creating a laceration wound that required eight stitches to close.
THIS ACCIDENT OCCURRED ON 4-15-03. WAS NOTIFIED ON 4-21-03 THAT THIS ACCIDENT OCCURRED. THE INDIVIDUAL BELIEVES IT HAPPENED WHILE BRUSHING/SCRUBING TESTING SCREENS VIGOROUSLY. SHE EXPERIENCED SEVERE CRAMPING WHILE PERFORMING THIS TASK. SHE DID SOMETHING TO HER MUSCLE OR NERVE IN HER LEFT UPPER BACK. SHE IS ON RESTRICTED DUTY.
EE WAS DOING SOME CLEAN-UP WORK, BREAKING UPART SOME PALLETS WITH A SLEDGE HAMMER. HE HIT A PALLET BOARD THAT BROKE AND FLEW UP, HITTING HIM IN THE MOUTH AND CUTTING HIS LOWER LIP.
THE INJURED WAS ON A BAGGING PLACING BAGS ON SPOUTS. HE STARTED A CONVEYOR CHAIN AND IT BECAME STUCK. HE SHUT THE POWER OFF AND USED A CROW BAR TO BACK THE CONVEYOR UP THE BAR SLIPPED. HIS HAN D FELL INTO A SHARP PBJECT AND CUT INTO THE TOP SIDE BEHIND THE LITTLE FINGER. HE RECEIVED 3 STITCHES AND WAS ON RESTRICTED DUTY.
EE WAS PULLING ON A 3/4" DRIVE TORQUE WRENCH TIGHTENING THE REAR WHEEL LUG NUTS ON 769-C CAT HAUL TRUCK AND FELT SOMETHING POP IN HIS BACK.
