Employee was standing on a piece of metal that was being bend so it wouldn't move and with hands on the frame of the conveyor above. An employee struck the metal with a hammer causing employee # 1 to lose balance. Hand was holding on the conveyor frame which put weight on wrist (left) injuring it.

Injured miner was using a pry bar to lift a bearing on a LOTO'd pump to remove shims under it. Lifted bearing and used fingers to remove shim, pry bar slipped dropping bearing, catching left index tip of finger cutting it off.

He was cutting a rope and the utility knife slipped and cut his left leg.

While trying to remove a bolt from the hydraulic fluid reservoir the wrench slipped and cut his right hand.

Installing the last tooth on an excavator bucket with a sledge hammer. A sliver of steel from the tooth broke off striking him in the left leg.

He had attached a hook and rope to the bottom screen in order to pull it out through the top. The hook slipped and his right elbow struck against the frame of the screen deck. No expected impairment.

Removing a bearing from the housing by striking a hammer with another hammer. A sliver of metal from one hammer struck him in the abdomen.

She was cutting off the top of a box of tickets when the box cutter kinfe slipped and cut the middle finger of her left hand.

EMPLOYEE WAS CUTTING A PIECE OF OLD CONVEYOR BELT WITH A UTILITY KNIFE. HE LET THE KNIFE SLIP AND CUT HIMSELF ON THE LEFT HAND BETWEEN HIS THUMBAND FIRST FINGER. INJURY REQUIRED TWO STITCHES.

BOLT HEAD BROKE WHICH CAUSED A WRENCH TO SLIP AND HIT ARM AND CHEST ON A PIECE OF SHAKERS - SQUARE CORNER CUT IN AT SHOULDER - 2" LONGCUT AT CHEST & SHOULDER - 6 STITCHES TO REPAIR.

CONTUSION/ABRASION TO LEFT SHIN STRUCK BY 8 LB, SLEDGE HAMMER USED TO BREAK-UP A LARGE PIECE OF SANDSTONE LODGED IN FEEDER HOPPER.

PULLNG HEAD PULLEY WITH COME-A-LOGN HOOK BROKE HIT EMPLOYEE UNDER CHIN.

INJURED WAS CHANGING SCREENS ON WASHER. HIS WRENCH SLIPPED AND HE HIT HIS RIGHT ARM ON PIPE.WHILE HIS ARM HURT HE DIDN'T THINK IT WAS ANY THING SERIOUS. 12-9 HE APPLIED PRESSURE TO WRENC H AND PAIN SHOT THRU RIGHT ARM. HE WENT TO DR. WHO SAID A BONE WAS CRACKED ON 11-17 AND HAD STARTED TO HEAL. NOW HAS ARM IN CAST.

LACERATION TO LEFT HAND WHILE CLEANING FEEDER BELT.

EMPL WORKING ON DRAG BUCKET AND HAMMER CHIPPED OFF AND PIECE OF STEEL WENT INTO HAND.
