Employee went to dentist and a crown on EE's tooth was broken. 4-15-2016 it was brought to my attention that a crown was broken and replaced on EE's teeth after seeing a dentist.
Employee was pushing on leveling bar with channel locks, with mechanic. When my back got tight, and started to hurt. When he went to stand up straight he had a sharp pain in my lower middle back.
Employee was driving a pin out of a ride with a hammer and chisel, when a piece of the chisel broke off and lodged under the skin on the left index finger. The employee received two stitches.
