While attempting to remove a filter cover using a pipe wrench, the cover broke loose and the pipe wrench lost grip and fell onto EE's right great toe. The injury has been identified as a right great toe contusion resulting from a falling object. Employee had medical evaluation and was released for light duty.

EE was performing a drag resocket. EE experienced a laceration to the distal phalanx of the right-hand little finger when a small chip of metal that was embedded in the sledgehammer's handle cut the glove and finger. EE received stitches.

Employee was picking up a backpack that contained a folding knife that had inadvertently deployed. When the employee grasped the backpack, the knife cut the distal phalanx of the right index finger, requiring stitches.

EE was closing the doors on the Finish Mill 2. While using a hammer to help close the door EE accidently hit EE's right index finger. Medical attention was sought, and EE was diagnosed with a fracture and placed on restrictions of no use of the right hand.

Employee was swinging sledge hammer and strained right shoulder. Taken to doctor for examination and EE was placed on light duty.

A cement truck loading operator, with more than 23 years of experience was centering the spout into a cement tanker by using a 52-inch bar that weighs 3 pounds when they felt a pain in right wrist and elbow. After seeking medical attention they were diagnosed with right wrist/elbow sprain and then performed an MRI which came back with no more findings.

2 wrenches smashed finger

EE was using a sledge hammer to drive a pin in. The sledge hammer was harden steel. The pin is soft blow material. As the pin seated the hammer made contact with the area around the pin. This resulted in a small piece of metal entering ee's lip. When the metal was removed it required 2 stitches.

Mobile mechanic was in the process of removing a front end loader bucket tooth which was seized on the bracket so mechanic used a sledgehammer to remove the tooth, when a piece of metal from tooth embedded on left shin.

One electrician and one helper were working on a piece of mining cable. The electrician was stripping the mining cable with a razor knife and the helper was helping to hold the cable. The knife slipped off of the cable and cut the helper on the right forearm.

EE was removing the belly pan off of a 615 Cat water wagon. When EE was removing the last bolt, the pan came down and landed on hand and wrench. This caused a small cut on the palm of hand below the index finger. The cut required 4 stitches. EE returned back to work and hour and a half later.

On 1/14/2016 Mechanic sustained a puncture wound from a fractured piece of metal after striking a crusher hammer with a sledge hammer. Employee taken to doctor and released to Full Duty, No Restrictions. On 1/29/2016, the employee was taken to doctor for follow up and placed on restricted duty, No heavy lifting. On 1/29/2016 this injury became a reportable injury

EE was assisting a crew of maintenance mechanics replacing a link in an elevator chain using a pin driver to drive out a broken pin. While striking the pin driver a sliver of metal became airborne and struck EE on his right cheek causing a cut.

Maintenance mechanic was tightening bottom base nut on finish mill dust collector fan when elbow struck the floor unexpectedly. The nut was located in a tight place under motor and mechanic had to lay on floor to reach the nut.

EE was in the process of attaching two 1" pieces on conduit together using a coupling. EE was holding a pipe wrench with EE's left hand holding the piece of pipe from rotating and treading the piece on the right using a pipe wrench also. While holding the pipe wrench with EE's left hand EE heard a pop from EE's left shoulder.

On 8/21 employee sustained a puncture wound while using a hammer and punch. On 8/25 he was complaining that there was a piece of metal in his left hand and taken to the doctor. The hand was x-rayed and a piece of metal was. He was sent back to work with NO Restrictions. On 9/2 he had the piece of metal surgically removed and placed on restrictions with limited hand use.

Removing liners from a ball mill a liner was stuck, he struck the liner with a sledge hammer to dislodge it causing it to shatter and a piece of metal hit him in the lower right leg shin area causing a laceration and a piece of metal in right leg.

Employee was closing the doors on a rail car. Lever used sprung back and pinched employee's left hand middle finger,

EE was inflating a tire on a 863 bobcat utilizing a ratcheting strap to seat the tire to the wheel. When the strap was released it re-coiled upward striking the employee's forearm requiring 8 stitches to close.

Employee was cutting a piece of rubber, wearing gloves, when he received a small laceration on his left thumb.

Employee was opening a knife valve with a pry bar. The pry bar slipped and the employee fell backwards and hit his head on a hopper causing a laceration to the back of the head.

Fixed Plant Mechanic was replacing a conveyor belt on a Power Screen and sustained a small laceration to the top of his left ring finger while he was trimming the belt. He was taken to the ER and the laceration was cleaned and glued together. No restrictions were given. He was given a tetanus shot and prescribed antibiotics.

EE was removing a packing gland with a com-a-long when the com-a-long hook slipped off its attachment point and re-coiled and struck the employee on the right hand finger causing a fracture.

Employee was working with another employee using a pry bars trying to free a jammed roller in raw mill. One employee was lifting and the other was pushing down on pry bar to move the jammed roller. When the roller unjammed, it rolled over quickly and smashed little finger between pry bar and fixed pipe.

A mechanic from the Mobile Shop received a laceration in his left hand. He was changing a seal on a PTO. The tool that he used slipped off his hand. He received the cut on the palm of his hand close to his thumb.

While cutting steel bands off a pallet, the steel band recoiled back and cut the left arm just above the wrist.

Employee was using hand tools (hammer & steel bar)to strike a metal dust pipe to unplug the flow of dust through it. After striking it numerous times over a 30 minute period, his back became stiff later in shift that became more painful as time passed.

Employee was tightening a bolt on small vibrator mounted on the gypsum hopper. He used two adjustable wrenches to tighten the bolt, one on the top and one on the bottom to prevent the bolt from moving while tightening. He used both hands to push on the bottom wrench when it slipped off. This caused his hand to jerk forward striking metal hopper with top of hand.

Employee was removing a broken piece of a hammer handle from the hammer section using a bolt and another hammer. He struck the bolt twice with the hammer in order to remove the broken handle section when the third strike he hit his left thumb causing impact damage to the thumb.

A 64 year old Miner with over 26 years experience was doing a hoist resocket on a dragline. As he raised the sledge hammer to knock out the wedge from the socket he felt a popping sensation in his right bicep. The employee visited several doctors getting a second opinion and missed his first day of work on 11-5-2012

Employee was installing a 500 lb. raw mill door with another worker. After taking it off the hoist onto door frame, he attempted to use a pry bar to line up the bolt on door hinge. As he was prying it, the door slipped off door frame and fell onto top of foot.

EE was working removing shell liners from raw mill (ball mill). The process required striking the liner with a hammer to loosed to remove. A piece of shrapnel struck EE in the right shoulder embedding under the skin.

On May 22, employee claims he strained his R shoulder and neck opening a rail car door with a pry bar. Supervision was not notified. On May 23 the employee started a 2 week vacation. On May 27 the employee notified his supervisor and said he needed to see a doctor for his injury. On May 28 the employee visited the doctor and was diagnosed with a lumbar strain and prescribed Meds.

While working to install a piece of rail, EE struck a rail clip with a hammer, which glanced off and hit his left index finger.
