Miner was helping lower a chute on the secondary plant, EE and two other employees were using come-alongs to lower the chute into place. They let the chute drop halfway down, and it landed on the Miner's right hand, fracturing it.

Mostrando los 14EE was moving lines under a 100-ton conveyor. The Allen key EE was using to hold the bolts, wrapped around EE's glove and twisted EE's pinky finger resulting in a sprain to the finger.
While repairing a fender in the shop, EE missed the fender while breaking it loose with the sledgehammer which caused EE's hand to strike the fender. This caused an injury to EE's left pinky nail.
While replacing flashing EE was not wearing gloves and was using a box cutter and was cutting towards self. A witness stated to the employee: shouldn't be cutting towards self.
A site foreman was struck in the face by a chisel while trying to separate a crusher assembly from its associated hopper feeder.
Employee with opening a package with a knife. He was cutting away from himself and hit his hand on the table, sliding the knife into his index finger.
Went up to oil chains on jaw belt and saw that belt that feeds tyrock screen had a lot of build up. Locked out equipment. Got a pick ax and scraper. Walked down the 1-1/4" belt. Scraping the sides of the bin. Used pick ax in corners. Crouched down low and swinging pick ax and got it hooked on an angle. Arm stopped mid swing/felt like a stabbing in front shoulder
EE was splicing a cord (straight line down & away from himself with a circle cut around cord at the end) and he grabbed the cord to pull it off. He was pulling the cord off & towards himself the knife in hand. His hand slipped off cord, & the knife cut his thumb & 1/3 of his tendon. EE got a few stitches on the tendon about 9 on the outside.
EE WAS WORKING W/COWORKER TO REPAIR THE CON CRUSHER. EE WAS HOLDING A METAL BAR SLAB WHILE COWORKER POUNDED ON IT WITH THE SLEDGEHAMMER. COWORKER MISSED THE BAR AND HIT EE'S LEFT ARM. EE DID N OT SEEK MEDICAL ATTENTION AT THIS TIME. EE WENT TO SEE THE DR. ON 8-31-98 AFTER FEELING MORE PAIN & BRUISING OVER THE NEXT FEW DAYS.
EE WAS WORKING WITH A SKID LOADER FIXING THE BUCKET WHEN THE 500 LBS BUCKET LOOSENED AND FELL ON HIS LEFT FOOT. EE WAS THEN TAKEN TO HIS DR AND X RAYED. THE RESULT WAS TWO FRACTURES TO HIS LEF T FOOT AND A CAST WAS PUT ON HIS FOOT.
20 POUND SLEDGE HAMMER BOUNCED OFF OF I-BEAM HITTING THE FOOT.
EE WAS SHOVELING OFF CONVEYOR BELT THERE IS A WOODEN EDGE WHILE SHOVELING HE GOT A SLIVER OF WOOD IN THE LEFT ARM THE SLIVER HAD TO BE REMOVED WITH A SCAPER AND ONE SUTURE.
EMPLOYEE WAS BREAKING UP A BOULDER WITH A HAMMER. THE ROCK HIT HIM IN THE RIGHT LEG CAUSING A CONTUSION.
WHILE REPAIRING CRUSHER, EMPLOYEE WAS HOLDING TOOLS AND A PART SLIPPED HITTING HIS FOREHEAD.
