Miner was replacing liners in a chute. EE was using a portable manual hydraulic ram to help position the liner plate. While positioning the plate the ram slipped and struck the miner in the upper left jaw.

Employee was opening the manual slide gate on the clinker silo with a ratcheting wrench. They applied pressure to the wrench and the ratcheting mechanism wasn't fully engaged, allowing the wrench to slip and smash the employees ring finger between the wrench and the feeder structure. Employee was wearing leather gloves- 4 sutures in fingertip

Employee reported pain in right wrist while using a spade shovel to clean built up material in the tracks of the dozer. While chipping the material the spade hit some very hard material and sprained their right wrist. Employee was given a splint and physical therapy as treatment.

Employee was retrieving garbage bags from a dumpster that had mistakenly been thrown in the wrong roll off. EE was using a grabber tool to retrieve the bags, as EE was pulling a bag towards the front of the dumpster, EE felt a pop and discomfort in EE's neck. Taken to the clinic on 7/7/25 and diagnosed with a strained muscle in EE's neck and placed on light duty.

Laborer was working with the vac truck and needed to cut the old hose off and add a new section. While cutting the bad section, employee was using a utility knife, cutting towards their body when the knife slipped and cut their hand, right above the wrist.

While positioning a feeder for install with three other miners the employee pinched right middle finger when unhooking a chain hoist hook and the feeder moved unexpectedly. The pinch removed the tissue from the tip of the finger. The result was an partial amputation of the tip of the right middle finger performed at the ER.

While swinging a sledgehammer to remove the roll pins on the blow bars of the secondary crusher the employee felt a sharp pain in the lower right back. Was seen by the doctor on 2/26/25 and placed on restrictions for a muscle strain in the lower back.

Employee was using a pipe wrench to break a rotary feeder free. EE was unable to break it free by hand. EE then used EE's body weight by standing on the wrench. EE slipped off and the impact to the flooring caused pain in EE's left knee.

EE utilizing a folding pocketknife in the upside-down position to cut a 6" hose. Knife folded back on itself, pinning the EE's finger between the handle & blade, resulting in a laceration to right index finger.

EE pinned hand between rodding bar and preheater tower, resulting in fractured finger.

Employee was using a sledgehammer to break a mill door free, had to switch tools and when EE reached out to grab the other tool felt a pop in EE's lower back.

Employee was removing the #2 door on the raw mill. The door was tight requiring employees to strike the door to get it to open. On the second swing EE felt a pinch in EE's midback, straining EE's back.

Employee was attempting to clear a blockage in a feed chute. EE sustained a puncture wound/laceration to the left leg requiring 3 stitches.

Employee wearing gloves smashed fingertip with a bar before attempting to close a railcar door. Minor fracturing of fingertip.

Employee was working to clear an air slide, when the air slide cleared material came into contact with the employee's face. Employee was wearing safety glasses, but material came into contact with left eye. The employee was taken to the eye doctor who discovered minor burns to the eyeball and prescribed an antibiotic eyedrop.

The employee was cutting vac hose with EE's pocket knife. EE was not wearing gloves and nicked the bottom of EE's left thumb. 3 stitches were required.

An electrician was striping an electrical cable lead with a striping knife when EE's left hand slipped. EE rolled left palm over the tip of the striping knife lacerating EE's left hand. Employee received 4 sutures to close the wound. Employee was wearing gloves at the time of the accident.

Employee was attempting clear a plug in a chute when an air blaster discharged projecting coal dust in the employee's eyes.

The EE had right pinky hit by a hammer swung by a coworker when they tried to close a door. The EE sustained a crushing injury requiring surgery to close the wound.

On 04/18/2022 the employee was using a pipe wrench to open vessel doors on the HW Kiln Preheat Tower. The employee felt a pop in right shoulder while trying to open a vessel door. On 4/21/2022 employee was placed on restricted duty.

A repairman was loosening a spectacle valve and lacerated a finger when the nut broke free resulting in them smashing a finger between the transfer pipe and the loosening tool. They received 4 stitches and was diagnosed with a chipped bone.

Employee was checking for wear/slop on the carrier rollers using a prybar. Bar slipped from the roller and pinched employee's right ring finger between the bar and the frame of the conveyor.

Injured mechanic hit thumb with deadblow hammer while seating a piston in quarry haul truck engine.

The employee was cleaning down a chute with a pick. The employee hit a chunk of material and felt a pop in the left shoulder.

Employee was scraping down a synthetic gypsum chute of material when employee pushed the scraping tool into a large chunk of material and felt a pain in left shoulder/ left pec.

EE was at a workbench attempting to seat a rubber gasket over a baring race. EE was striking the gasket with a hammer. The tool slipped and hit the tip of EE's left middle finger, causing blood to pool beneath. EE was taken for medical treatment where they drained the blood from under the nail. An X-Ray was performed; no broken bones were present. There were no restrictions.

Injured worker was cleaning out a tire chute with a steel rod out bar. During cleaning, pinched left index finger between the bar and steel port opening. EE was wearing gloves. The pinch caused a laceration requiring stitches.

Employee was cutting the wire tie off a bag of cement with a utility knife. When it cut through the tie, the force continued into employee's thumb on left hand.

Employee was using a porta-power sitting on a stand to push out cooler floor grate plates. The porta-power became cocked on the stand and the stand hit the employee in the mouth, chipping a tooth.

Injured employee (IE) was struck under the chin by a blunt force causing a laceration requiring five sutures; MSHA recordable incident. IE used the portable industrial shotgun, through an eight-inch porthole, to help knock down material in a silo. When material fell it pushed the shotgun barrel down and the stock up. The stock hit the miner in the chin causing a laceration.

The employee was attempting to cut metal strap from a bundle of angle iron with a pair of pliers. When the strap broke it sprung open and caught EE's opposite gloved hand which EE was using to hold the bundle. The strap caught EE's left ring finger causing a laceration which required sutures to treat.

EE reports trying to loosen a bolt on the chem fuel line up at the Down Draft Calciner (DDC) on the tower. EE states they reached up over head and pull hard and jerk on the bolt to get it loose. EE said they didn't have pain that day but it was hurting the following morning. EE did not report the injury until 1/29/20 because they thought it would get better.

Process Utility Repairman strained left shoulder while manually cleaning out a material chute with a handheld scraper. Discomfort continued for a couple of hours afterwards so employee sought medical attention and was placed on restricted duty (keep elbow at side at all times until next visit).

Trying to remove a bolt from an attachment point and when the hammer struck the bolt driver a piece of metal had projected from the point of contact and struck the employees arm.

Employee was driving a re-bar stake into the gravel to set height for concrete forms when they missed and struck their hand on the top of the re-bar.

Injured miner was cleaning on Kiln Feed Shelf. EE was using a T-handle bar to clean built up material. EE broke through built material easier than expected and smashed EE's right pinky finger between handle of metal bar and metal wall of feed shelf.

An employee was attempting to clean a raw material chute with a bar when a gate closed which pulled the bar in causing the gate to close on EE's left arm. This resulted in a fracture to the left arm.

Employee got foreign debris in eye while using a garden hoe

Lying on back holding wrench as a backer. Stood up and got a tiny speck of what we think was metal in left eye. Doctor removed it Monday (3/11/19) afternoon, no restrictions, no medications, no time off. Eye still bothered EE on Wednesday (3/13/19) morning. Went to hospital, doctor did not see anything but prescribed antibiotic. No restrictions, no time off, no follow up.

Worker was removing insulation on an electrical splice when the razor knife worker was using broke causing worker to cut left hand. The laceration required 9 sutures.

An employee was pulling on a wrench trying to break loose a fitting when the employee felt pain in left wrist. The employee decided to change positions and try pushing the wrench rather than pulling it. Employee started to feel a sharp pain in wrist when employee started to bear down on the wrench as it was loosening.

The employee was removing a bucket tooth and retainer pin on a Cat 988 loader, employee missed the driver tool used to remove pins striking right thumb with a 3lb hammer. The thumb broke in two places near the mid-knuckle.

Employees were using a hammer bit on a mini-ex to extract large bolts from a raw mill tire. A washer from a bolt shot out in the process striking an employee on the upper lip which required two stitches.

A welder / repairman was using a pry bar to lift up a welded bearing mount plate. The bar slipped and struck the employee in the mouth, causing a small, superficial lip cut and a cracked canine tooth.

Employee was using a kiln ring blaster eight-gauge shotgun to remove a ring in the back of the kiln. After approximately thirty shots, three small pieces of material or shotgun slug projectile ricocheted back and struck face resulting in two sutures on the bridge of nose.

Employee was using a come along and choker to pull the HDR. The HDR had become stuck and would not move in either direction. When EE pulled with the come along the choker failed and the hook came back and struck the EE. EE sustained a broken pinky finger on EE's right hand.

Injured employee (IE) was assisting in the retraction of an inlet probe by means of a hand operated chain lever hoist. During the retraction process a ¼" eye and eye wire rope sling being used failed/broke and the rope snapped back and struck the IE's right wrist and IEE also lost balance and fell onto left buttocks resulting in wrist and coccygeal strain and contusion.

Struck by metal bar while performing tower rodding duties. Material fell inside the preheat tower and struck the end of the bar causing the other end to raise and strike the employee.

Employee was working in the Raw Mill trying to loosen an Allen head bolt when ee felt pain in ee right arm near the inside of ee elbow.

Employee was attempting to remove the keeper pin on a D-8 Cat blade with a 2 pound Sledge hammer and a punch. As EE swung the hammer, it glanced off of the punch, hitting EE in the left hand between the thumb and pointer finger. EE sought medical help on 4/13/16, but there were no fractures. EE returned to the doctor on 4/18/16 and was placed on light duty.
