He didn't initially report incident. Date of initial injury is unknown date filed is approx. The scrape became infected and employee then notified his supervisor on 2/8/2012
WHILE CUTTING BELTING WITH A UTILITY KNIFE THE KNIFE SLIPPED AND HE CUT HIS WRIST
EMPLOYEE WAS USING A WRENCH TO LOOSEN A BOLT. THE WRENCH SLIPPED AND HE HIT THE PALM OF HIS LEFT HAND ON THE EDGE OF A PIECE OF STEEL CAUSING A LACERATION. HE WASN'T WEARING GLOVES
The employee decided to use a different piece of equipment to do the task other than what had been discussed with the supervisor. The equipment he chose wasn't capable of supporting the load he was handling.
WHILE HOLDING SKIRT BELT EE NOTICED THE HOLES PUNCHED WERE NOT LARGE ENOUGH, SHE USED HER UTILITY KNIFE TO ENLARGE THE HOLES, WHILE DOING THISTHE KNIFE SLIPPED, BLADE CUT THUMB.
EE HAD CLEANED OUT ANVIL COVER ON CRUSHER WHEN ATTEMPTING TO PRY COVER UP, THE PRY BAR SLIPPED AND STRUCK IN MOUTH.
