While walking to enter forepersons pickup truck, miner was watching footing and not looking ahead. Miner struck forehead on corner of pickup truck forcing hard hat up and off of head and causing laceration to forehead. 8 stitches required; employee released to full duty same day.

Mechanic removed cutting edge from loader bucket. Following removal and placement of old cutting edge inside bucket, they pushed the cutting edge out of the way. In doing so, the edge lacerated glove through to left index fingertip. 4 stitches required.

While cleaning ground below and adjacent to a troughing belt, a 4 inch diameter stone fell from a height of 11 feet and struck the employee on the right shoulder. Contusion to right shoulder resulted.

The employee retrieved overalls and was walking away from personal vehicle when they stepped on a patch of ice. Employee lost footing, slipped and fell landing on left shoulder.

EE using bottle jack to remove plate from crusher. Plate came free and threaded rod struck employees left hand. .25" skin avulsion on hand.

While walking on cat walk preparing to clear it off, employee stepped on a golf size stone and fell against the steel top rail causing injury to EE's ribs and abdomen

While cleaning off cat walk next to 6x16 screen on secondary a rock popped out and hit employee on the right shoulder

While attempting to lift a 200 lb roll of belting with another employee injured employee strained back

While cutting rubber roll "nose rubber" with utility knife, the knife slipped and cut employee's left hand (palm)

Employee while greasing loader tripped on grease house and strained employee's right knee

Failure to follow company SOP. Failure to complete JHA. Incorrect tool used. Freezing temperatures. Injury - pinch point, struck by - Laceration to finger requiring stitches.

Employee was clearing snow and ice from loader, was overreaching and pulled a muscle in back

While using a push scraper to clean up he unexpectedly hit the edge of a metal plate. The scraper kicked back and the handle hit his left cheek causing a left bottom tooth to break and need removal.

Employee was using a rake to clear the buildup of dust under a conveyor and strained his back muscle

While lifting a head pulley guard to reattach he felt a pull on the right side of his back.

While operating a haul truck debris came out of heater vent and imbedded in eye.

Employee exited catwalk and climbed down ladder. He hit his right knee on a hook that used to hold a hanging ladder. The hanging ladder had been removed from service but the hook wasn't. The hook fell between the rungs of the ladder the employee was using. The hook had a sharp edge that lacerated employees knee.

Employee states he had pain in shoulder for past 7-8 months and didn't report it. He couldn't stand it anymore and went to the doctor on 9/17/12. Doctor diagnosed bursitis from repetitive overhead motion at work.

Employee was using punch on belting, he let go of punch after he had it seated and then hit it with a hammer. The punch came back up and hit him in the left cheek causing a laceration requiring 5 stitches

Employee had lifted hard hat to wipe sweat from brow. Stone came off of catwalk went behind his safety glasses and hit lower lid of left eye causing a contusion that required a prescription

Employee was using hammer to hit a chisel, head of hammer came free. Head then hit side of concave ring, deflected off ring hitting employees leg. Employee states he received a small scrape. He didn't initially report incident. Date of initial injury is unknown date filed is approx. The scrape became infected and employee then notified his supervisor on 2/8/2012

WHILE CUTTING BELTING WITH A UTILITY KNIFE THE KNIFE SLIPPED AND HE CUT HIS WRIST

Hearing test showed recordable standard threshold shift.

While changing out a screen, employee hit his hand on a dust cover that came loose. He attempted to grab it and instead, he caught his finger between it and the screen, causing a laceration.

EMPLOYEE WAS USING A WRENCH TO LOOSEN A BOLT. THE WRENCH SLIPPED AND HE HIT THE PALM OF HIS LEFT HAND ON THE EDGE OF A PIECE OF STEEL CAUSING A LACERATION. HE WASN'T WEARING GLOVES

The employee decided to use a different piece of equipment to do the task other than what had been discussed with the supervisor. The equipment he chose wasn't capable of supporting the load he was handling.

Employee states he was climbing fixed ladder located on water tank when he hit his head on the cage causing him to fall backwards.

Employee states while driving haul truck he hit a pothole, seat bottomed out, and his back was strained.

Eployee states he twisted left knee while throwing debris in dumpster.

Employee claims he strained back from riding in the haul truck.

While carrying a hole cover (2' by 3' rubber mat) tripped and fell to the ground.

Claims he unbuckled seatbelt ,while haul truck was being loaded with shot rock, to get a water bottle. The force of the rock being loaded caused him to be bounced out of the seat and he hit his head on the top of the cab. Strained neck muscles. Also claims he hit his head earlier on a piece of conduit he forgot to duck.

Employee lost grip on manhole cover while moving it causing it to drop on his toe. Broken toe on right foot (big toe).

WHILE APPLYING BELT DRESSING EE STATES HE PLACED HIS HAND ON BELT JUST BEFORE IT WAS STARTED UP. BELT STARTED, PULLED HIS FINGERS AND CAUGHT THEM BETWEEN BELT DRIVE AND SHEAVE. RECEIVED RX, FRACTURED TIP OF FINGER.

WHILE CLIMBING STAIRWAY TO WASH PLANT, EE NOTICED STAIRS WERE GETTING ICY. HE STATES HE SLIPPED AS HE WENT TO CLIMB BACK DOWN STAIRS.

WHILE HOLDING SKIRT BELT EE NOTICED THE HOLES PUNCHED WERE NOT LARGE ENOUGH, SHE USED HER UTILITY KNIFE TO ENLARGE THE HOLES, WHILE DOING THISTHE KNIFE SLIPPED, BLADE CUT THUMB.

WORKER RECEIVED A LACERATION TO HIS FINGER WHEN A SHEET OF STEEL (FLIGHT) SLIPPED OUT OF GROOVE,CAME DOWN ON FINGER. STITCHES WERE REQUIRED.

WHILE CLIMBING UP RUNGS OF LADDER EE WAS HANDING UP A SCREEN TO FELLOW EES. EE CLAIMS FOOT SLIPPED OFF RUNG AND HE STRAINED HIS BACK WHILE HOLDING UP SCREEN.

EE ATTEMPTED TO DISLODGE 2 ROCKS FROM THE FEEDER HOPPER. HE DIDN'T TURN OFF BELT FIRST. HE REACHED IN & THE ROCKS CAUGHT HIS FINGER REMOVING SKIN FROM TIP. EE RECEIVED ONLY FIRST AID AT FIRST. HIS FINGER WASN'T HEALING PROPERLY & HE HAD TO THEN HAVE SKIN GRAPHS ON 9/16/02.

EE HAD CLEANED OUT ANVIL COVER ON CRUSHER WHEN ATTEMPTING TO PRY COVER UP, THE PRY BAR SLIPPED AND STRUCK IN MOUTH.

EMPLOYEE WAS STANDING ON THE 2ND STEP OF LADDER AND LOST HIS FOOTING. HE SLIPPED BACK AND STRUCKTHE BACK OF HIS HEAD.

EE WAS EXITING THE 980 LOADER FACING AWAY FROM THE MACHINE WHEN HE SLIPPED AND FELL BRUISING HIS TAILBONE.

EE WAS WORKING ON PRIMARY CRUSHER. WHEN ANOTHER EMPLOYEE WAS REMOVING A END CAP FROM FLYWHEEL WHEN HE DROPPED IT ON THE INJURED EE'S RIGHT PINKYCAUSING A LACERATION WHICH REQUIRED 8 STITCHES.

EMPLOYEE WAS REMOVING FEEDER GRIZZLEY-SUDDEN MOVEMENT OF PART BEING LIFTED CAUSED HAND HOLDING PRY BAR TO BE PINCHED AGAINST STATIONARY PART OF GRIZZLY.

EMPLOYEE STATS THAT HE WAS STEPPING ONTO A LADDER WHEN HIS KNEE TWISTED & HE FELT PAIN.

WHILE LOOSENING SOME RETAINER NUTS HE HIT HIS HAND WITH A WRENCH AND HURT HIS FINGER.

EMPLOYEE WAS REMOVING PLATES UNDER THE CRUSHER WITH A PINCH BAR STRUCK THE BAR WITH HIS HAND INJURING HIS RIGHT HAND.

WHILE SETTING A 1/2" X 4' X 6'PIECE OF STEEL ONTO A CONCRETE WALL APPROX 5'HIGH. THE PLATE WAS RESTING ON THE CONCRETE AND THE CHAIN BECAME SLACK WHEN THE LOADER WER WERE USING TO SET IT, JERKED.
