EE was performing maintenance on cone crusher replacing sheave. Sheave shifted off motor shaft. EE's left thumb got caught between impact driver handle & steel frame. Sought med. treatment same day, received stitches & removed from work. Reporting within 10 days.

Mostrando los 25Employee was performing maintenance on conveyor belt. While removing a bearing with a mallet, a piece of metal broke off, became embedded into the EE's right pec. On 5/13/25 we were notified the employee had a piece of metal removed and was able to continue working. Reporting within 10 days.
Employee inside Screen deck removing frozen agg. material from belt with pry bar. Chunk came free & struck the employee in the jaw, chipping a tooth. We were informed on 12/3/24 that the employee had dental work done. Reporting within 10 days.
Employee was repairing blown hydraulic hose on a loader when the wrench slipped off the hose, which caused the employee to strike their right index finger against the hose clamp, causing a laceration. Employee did not seek medical treatment until 12/16/2023. Employee was ref. to a hand specialist and was given a finger splint on 01/11/2024.

Employee was using a sledge hammer to remove a pin from a hydraulic breaker arm. On a back swing, employee felt pull in right shoulder. EE sought medical treatment and has now been referred to physical therapy.

Employee was torching a tooth off a loader bucket. As he used a small sledge hammer to knock the tooth off the loader bucket, a small piece of metal chipped off of the tooth & struck him in his knee. The small metal chip embedded itself into his right knee.
Employee was loosening belt on roller for conveyor & had pain in right elbow & shoulder. He did not report incident until 1/3/14. He sought treatment on 1-26-14 & did consider care. He saw MD again on 2-19-14 & was given removable brace & referred to physical therapy. We did not receive these records until 3-6-14 - therefore we did not knew that was reportable until now.
Employee was crimping on electrical lug & the crimping slipped & pinched his right pinky finger, cutting it & fracturing it
EE was doing maint. on dragline crane which consisted of lifting and pounding on shaft. Next morning EE had low back pain. EE went to chiropractor.
