Employee was performing equipment setup on water tank. While walking to hook up a hose, EE slipped on limestone slurry and fell down. EE sought medical treatment and was DX with a right shoulder strain. On 11/18/24 we were notified that EE was referred to physical therapy. Reporting within 10 days.

Employee was performing maintenance on a crusher. EE was changing shoes. As it was being lowered, EE's finger got pinched between the shoe and the edge of the crusher. Employee sought medical treatment and was DX with a subungual hematoma. On 9/16/24 we were notified that the hematoma was drained. Reporting within 10 days.

Contractor employee was removing bolts for breaker and the load shifted, pinching EE's right index finger.

Employee was lifting screen to place in screen deck & felt pain in the shoulder. EE eventually sought medical treatment & has now been diagnosed with a tear. Surgery is scheduled for 9/20/2021. The employee continues to work.

Employee removed seatbelt to exit the broom and got tangled in the seatbelt. Employee grabbed the side handle and injured right shoulder. Employee first sought treatment on 7-14-2021. Employee had an MRI on 7-20-2021. We received the MRI report on 7-26-2021 which showed a partial tear.

An employee was repositioning a lime bag and strained EE's right shoulder. EE initially did not seek medical treatment. EE has now sought medical treatment and received a steroid injection and was referred to physical therapy.

EE was pushing a wheelbarrow of mud out of the surge pile tunnel to go dump it. In the process the EE stepped on a piece of dry mud which caused them to lose their footing and strain their back because they continued to hang onto the wheelbarrow. They sought treatment with a chiropractor.

On 7/18/17, EE was setting up a crane with the crew. While pulling the cable thru the block of the crane EE injured EE's shoulder. EE sought medical treatment & was diagnosed with a strain. EE recently went back to the MD & was diagnosed with a rotator cuff tear. EE has continued to work regular job. EE will be having surgery in Nov.

Employee was carrying a screen up to the screen box when ee strained left shoulder. Injury was initially treated with first aid measures. On August 31 ee saw an orthopedic MD & was given a cortisone injection as well as referred to physical therapy.

Employee was welding on 3/15/16 & hot slag fell down sleeve causing a burn. On 3/23/16, our employee notified us of this incident & sought medical treatment. EE was diagnosed with a burn & an infection.

Employee was grinding on a piece of metal flashing that was resting on the metal saw horses. One side of the flashing vibrated off of the saw horse causing the flashing to slide sideways off of the other saw horse. The flashing cut the employees finger as it was falling. The employee sought medical treatment and received stitches.

Employee was walking to personal vehicle at the end of shift & fell on ice in parking lot that had dislodged during work shift. Employee did not seek medical treatment immediately. Employee has now sought medical treatment with chiropractor.

Frost was coming out of haul road truck hit rough spot in road. Operator jammed right hand on steering wheel. Bone fracture above pinky finger on right hand.

Passing motorist intentionally drove thru locked quarry fence & vehicle got stuck. He walked to nearby top edge of high wall. Waukesha County Sheriff responded at 8:03 PM. 2 Sheriff Deputies tried to "talk him down," but he jumped at 8:24 PM landing 80 feet below on quarry floor. We were notified at approx. 10:35 PM & we reported the incident to MSHA at 10:45 PM CST.

Employee was loosening belt on roller for conveyor & had pain in right elbow & shoulder. He did not report incident until 1/3/14. He sought treatment on 1-26-14 & did consider care. He saw MD again on 2-19-14 & was given removable brace & referred to physical therapy. We did not receive these records until 3-6-14 - therefore we did not knew that was reportable until now.

While climbing down equipment, employee caught ring finger on framework. Ring finger was cut and employee received stitches.

Employee was crimping on electrical lug & the crimping slipped & pinched his right pinky finger, cutting it & fracturing it

Employee was taking track off drill & removing small roller. Roller slipped & pinched employee's right ring finger. Employee did not seek treatment until 11-8-12 & was diagnosed with an infection.

EE was changing screens and pinched left ring finger. He received stitches.

EE was blowing off equipment with air hose. He tried to move the air hose to his other hand and he lost control of it. The air hose struck his safety glasses, knocking them off and then hit his eye. He was given prescription eye drops.

Walking on jobsite slipped on snow and fell on left shoulder. He went to MD and was treated conservatisly-pain persisted. He has now been diagnosed with a torn rotator cuff. He continued to work, but may has surgery in the future.

Employee was shoveling and pushing wet limestone and felt pain in mid back. Went to md was diagnosed with a strain and given light duty restriction we did not have light duty work available with restriction.

Employee cut hand on stationary bag slide plate. He went to walk in clinic and received stitches.

Employee stepped up concrete step to obtain fuel log paperwork. When he stepped down off concrete step he rolled his ankle. Employee is currently not working.

Employee was using pipe wrench to loosen pipe and felt pull in lower back.

Hitting bearing off with hammer and 3 pieces of steel broke off and flew into arm and arm pit.

Employee complained of back pain & instally said it was not work related. Then on 6/12/07 He said this is work related and his back pain in caused by driving the guarry truck over the past year. *We are denying work relatedness of this claim*.

Loading 55 lbs cases of explosives onto truck, employee picked up his 3rd box and felt pull in back.

Employee was cleaning out the hopper and was sliding large frozen chunks of lime. He was bending and turning and felt a pull in his lower back.

Employee was grinding on a skid loader bucket that he was repairing. A piece of steel flew into his eyes.

Employee was getting out of vehicle & his back popped. He had just arrived at the job site-had not started working yet. We are still investigating whether this is work related.

Employee had rock stuck between tires of truck. He parked the truck and used an air chisel to get the rock out. The rock flew out and cut/hit his lip.

Employee was walking down conveyor catwalk and stumbled. When he reached for the handrail he missed and jammed the fingers on his left hand in the catwalk.

EE REPORTED PAIN IN HER HAND FROM DATA ENTRY AND SCALE OPERATIONS. INITIALLY HER CLAIM WAS DENIED AS NOT WORK-RELATED BUT FURTHER INVESTIGATION AND MEDICAL EVALUATION HAS LED TO THIS BEING COV ERED AS WORK-RELATED. SHE IS TO HAVE SURGERY ON FEB 2003. SHE HAS NOT LOST ANY TIME AS OF YET.

WELDING ON PLANT, WELDING ROD BECAME STUCK ON METAL. HIT ROD WITH HAND TO LOSOEN IT AND PUNCTURED HAND.

PULLING PUMP & HOSES @ THE WLS WATER STATION- STRAINED LOWER/LUMBAR BACK MUSCLES. CHIROPRACTICTREATMENT.

SHOVELING GRAVEL ONTO CONVEYOR BELT WHEN BACK GAVE OUT. EE HAD MULTIFUSION TO HIS BACK IN FEBRUARY AND WAS RELEASED W/NO RESTRICTIONS.

LIME BAG PLANT MOISTURE IN AIRLINE OF BAGGING MECHANISM, PULLED OF LINE TO LET MOUSTURE ESCAPE AND ATTEMPTED TO RE-ATTACH LINE CAUSING CAUSING BAG TO ENGAGE AND STRIKE OTHER HARD.

CUTTING STEEL, PIECE OF METAL DEBRIS BECAUSE OF EMBEDDED IN EYE. SAFETY GLASSES WERE ON AT THE TIME.
